ARIZONA

Counselor, Professional or Associate License Description: Major Requirements for License: By Examination -- 1) Master's or higher degree in counseling or a related field from: a) regionally accredited college or university in a program that consists of a minimum of 48 semester credit hours, or a program accredited by C.A.C.R.E.P. (Council on Accreditation for Counseling and Related Educational Programs) or C.O.R.E. (Council on Rehabilitation Education) in a program that consists of a minimum of 48 semester credit hours.; 2) Beginning on January 1, 2008, the program of study required shall include a minimum of 60 semester credit hours; 3) An applicant for professional counselor licensure shall be deemed to meet the curriculum requirements in this Section if the applicant holds an active associate counselor license in good standing; 4) An applicant for professional counselor licensure who received a master or higher degree before July 1, 1989, and whose program of study did not offer a practicum, shall have completed three years of post-master or higher degree work experience in counseling under direct supervision. One year of a doctoral-clinical internship may be substituted for one year of supervised work experience; 5) Applicant who did not attend a college or university in Arizona shall provide: a) a university or college catalogue course description for the year and semester the applicant was enrolled in the course for every course the applicant submits to meet the curriculum requirements and any additional documentation the Counseling Credentialing Committee determines is necessary to evaluate an applicant's curriculum; 6) Beginning on January 1, 2008, an applicant with a master or higher degree in counseling or a related field from a program accredited by C.A.C.R.E.P. or C.O.R.E. whose program of study did not include a minimum of 60 semester credit hours may submit coursework obtained outside of the degree from a regionally accredited college or university; 7) Applicants must receive a passing score on an approved licensure examination from one of the bodies:: a) National Board for Certified Counselors, Commission on Rehabilitation Counselor Certification 8) Applicant must complete a minimum of 3200 hours of supervised work experience in the practice of professional counseling in no less than 24 months. Supervised work experience in the practice of professional counseling is limited to the use of psychotherapy for the purpose of assessment, diagnosis and treatment of individuals, couples, families and groups. The 3200 hours of supervised work experience in professional counseling shall include a minimum of 1600 hours of direct client contact. in the discipline for which the applicant is requesting licensure in Arizona and the license or certification is in effect and in good standing; 2) Passage of an examination was required for the certification or licensure and the applicant passed the required examination or the applicant has passed the licensure examination required for the discipline and level license sought by the applicant; 3) The credentialing committee determines that the applicant's qualifications substantially meet the current requirements for Arizona licensure; 4) Prior to applying for licensure in Arizona, an applicant was engaged in the practice of behavioral health for a minimum of 800 hours each year for at least five out of the last seven years in the state issuing the license; 5) Prior to applying for licensure in Arizona, an applicant for licensure as a bachelor social worker or a master social worker has practiced as a social worker for a minimum of 800 hours each year for at least five out of the last seven years in the state issuing the license used by the applicant to qualify for licensure by reciprocity; and the applicant is not the subject of a pending complaint before the Board or any other state behavioral health regulatory entity and has not had a license or certificate to practice a health care profession suspended or revoked by another state behavioral health regulatory entity. Licensed Professional Counselor (LPC) License Description: Licensed Professional Counselors provide counseling to individuals, groups, organizations, and the general public. Counseling may include appraisals, consultation, referral, and research. Most counselors help individuals or groups of people in developing an understanding of their personal problems, defining and reaching goals, and bringing out their interests, abilities, aptitudes, and needs as related to personal-social concerns, educational progress, occupations, and careers. Counselors perform appraisal activities that may include selecting, administering, scoring, and interpreting assessments of an individual's aptitudes, interests, abilities, attitudes, achievements, and personality characteristics. Requirements: 1) Must possess a graduate degree that is primarily professional counseling in content from a regionally accredited institution. The graduate semester hours must meet the national academic and training content standards adopted by the Board and the Council for the Accreditation of Counseling and Related Educational Programs (CACREP/CORE) or equivalent. 2) Must demonstrate professional competencies by passing written, oral, and situational examinations as prescribed by the Board. 3) Must provide evidence of three years (3,000 clock hours) of supervised full-time experience, as defined by the Board, in professional counseling beyond the Master's Degree. One year (1,000 hours) of experience may be gained for 30 semester hours of graduate work beyond the Master's level, provided the hours are clearly counseling in nature and acceptable to the Board. Hours earned may be substituted for no more than two years of supervised professional experience. The Board of Examiners in Counseling does not have power to waive any required period of supervised experience. 4) Must complete a criminal background check. Written and oral examinations required. Counselor, Mental Health License Description: For MFT Interns and LMFTs: Must have a master's (at least 60 hours) or doctoral degree from a Marriage and Family Therapy Program or certificate from a post-graduate training institute accredited by COAMFTE or substantially equivalent to COAMFTE standards. Must pass the National Marriage and Family Therapy Exam Must document qualified post-graduate work experience in marriage and family therapy and that includes at least 3,000 hours of clinical services to individuals, couples, families, or groups. The supervisee shall complete his or her internship in not less than two and no more than seven years from the date the intern is registered with the board. 1. At least 2,000 hours must qualify as direct work experience. a. Up to 500 hours of direct work experience received during the completion of a graduate program that is systemically oriented as determined by the advisory committee may be counted toward the required 2000 hours. 2. The remaining 1,000 hours may be indirect work experience or other professional activities that may include but are not limited to qualified supervision, workshops, public relations, administrative tasks, consulting with referral sources, etc. as approved by the advisory committee. 3. The internship must include at least 200 hours of qualified supervision, of which at least 100 hours must be individual supervision. The remaining 100 hours may be group supervision. a. Up to 100 hours of face-to-face supervisor contact received during the completion of the applicant's qualifying academic experience graduate program that is systemically oriented as determined by the advisory committee may be counted toward the required 200 hours of qualified supervision. i. Of the 100 hours, only 50 hours may be counted as individual supervision.For Counselor Interns and LPCs: Must have a master's degree to include 48 graduate hours with eight prescribed content areas, a mental health counseling practicum of 100 hours, and a mental health internship of 300 hours. Beginning September 1, 2015, all applicants whose academic background has not been previously approved by the board, must have accumulated at least 60 graduate semester hours as part of the graduate degree plan containing the eight required areas, the supervised mental health practicum and supervised internship in mental health counseling. Applicants may apply post-masters counseling courses towards licensure if their degree program consisted of less than 60 hours. Licensed Professional Counselor (LPC) License Description: Three application methods are currently available to persons seeking LPC licensure. The three options are: Licensure by Examination By Examination (Minn. Stat. § 148B.53, subd. 1) Master's or Doctoral degree program in counseling or a related field, from a program that is CACREP (Council for Accreditation of Counseling and Related Educational Programs) accredited or by an accrediting agency recognized by CHEA (Council for Higher Education Accreditation). Degree upon which license application is based must include: -at least 48 semester or 72 quarter credits; -a minimum 700 hour supervised field experience; and -specific academic course content in ten subject areas. However, Minn. Stat. § 148B.531 allows up to 20 semester or 30 quarter credits and any of the field experience hours to be completed post-degree for licensure purposes. If more than 20 semester or 30 quarter credits are earned post-degree, they must be earned in an organized sequence of study. The ten core content areas are:  1. The helping relationship, including counseling theory and practice;  2. Human growth and development;  3. Lifestyle and career development;  4. Group dynamics, processes, counseling, and consulting;  5. Assessment and appraisal;  6. Social and cultural foundations, including multicultural issues;  7. Principles of etiology, treatment planning, and prevention of mental and emotional disorders and dysfunctional behavior;  8. Family counseling and therapy;  9. Research and evaluation; and 10. Professional counseling orientation and ethics. 2,000 hours of supervised professional practice; these hours may be (1) hours that were completed prior to licensure, if the Board finds the past supervision acceptable, or (2) they may be completed after licensure, based on a supervision plan that is accepted by the Board. Successful completion of the National Counseling Examination (NCE) or National Clinical Mental Health Counselor Exam (NCMHCE). Administered by the National Board of Certified Counselors (NBCC) or an alternate examination, determined by the Board to be equivalent to the NCE. $400 check or money order to cover the application fee and the initial license fee. All fees are non-refundable. Licensure for Licensed Psychologists (LPs) currently licensed in Minnesota LP (Minn. Stat. § 148B.53, subd. 1[c] and 1[d]) Current Minnesota LP licensure. Supervision: 2,000 hours of supervised professional practice. LP: Supervision completed for the LP license is accepted as meeting the 2,000 hours. True and correct copies of supervision verification submitted to the Minnesota Board of Psychology should be submitted to the Minnesota Board of Behavioral Health and Therapy (BBHT). Note that the LPC license will be based upon the degree that the LPP or LP license is based upon. $400 check or money order to cover the application fee and the initial license fee. All fees are non-refundable. Licensure by Reciprocity Reciprocity (Minn. "Provisional licensed professional counselor" (P-LPC) shall mean and is restricted to any person who holds himself or herself out to the public by any title or description of services incorporating the words provisional licensed professional counselor or psychotherapist and who offers to render professional counseling or psychotherapy services under the supervision of a board qualified supervisor to individuals groups organizations corporations institutions government agencies or the general public for a fee monetary or otherwise implying that he or she is licensed."Practice of counseling/psychotherapy" shall mean rendering offering to render or supervising those who render to individuals groups organizations corporations institutions government agencies or the general public any service involving the applications of counseling procedures and other related areas of the behavioral sciences to help in learning how to solve problems or make decisions related to personal growth marriage family or other interpersonal or intrapersonal concerns."Counseling/Psychotherapy procedures" shall mean the application of mental health psychological or human development principles through cognitive affective behavioral or systematic intervention strategies that address wellness personal growth or career development as well as pathology. Counseling/ Psychotherapy involves diagnosis assessment and treatment by use of the following (i) Counseling/psychotherapy methods and techniques both verbal and nonverbal which require the application of principles methods or procedures of understanding predicting and/or influencing behavior and motivation; (ii) Informational and community resources for personal or social development;(iii) Group and/or placement methods and techniques which serve to further the goals of counseling;(iv) Designing conducting and interpreting research on human subjects or any consultation on any item above; and(v) Appraisal techniques including but not limited to testing of achievement abilities interests aptitudes and personality."Fees for licensed counseling services" shall mean any form of compensation received for the practice of counseling."Board" shall mean the Mississippi State Board of Examiners for Licensed Professional CounselorsAny person who represents himself or herself by the title "Licensed Professional Counselor" or "Provisional Licensed Professional Counselor" without having first complied with the provisions of this chapter shall be guilty of a misdemeanor and upon conviction thereof shall be punished by a fine of not less than Five Hundred Dollars ($500.00) nor more than One Thousand Dollars ($1000.00) for each offense.The board shall adopt the code of ethics of The American Counseling Association. The immigration document must be current and issued by the United States Immigration Bureau.(e) The applicant is not in violation of any of the provisions of the rules and regulations adopted hereunder.(f) The applicant shall have a minimum acceptable graduate semester hour or acceptable quarter-hour masters degree as determined by applicable laws rules and regulations primarily in counseling or related counseling field from a regionally or nationally accredited college or university program in counselor education or a related counseling program subject to Mississippi State Board of Examiners for Licensed Professional Counselors approval. All applicants shall provide official transcripts of all graduate work.(g) The applicant must pass the examination approved by the board as set forth by all applicable rules and regulations(h) A provisional license issued under this section shall require that the individual confine ones practice to a board-approved site and accrue counseling experience under the supervision of a board-qualified supervisor. (i) The limited license shall be renewable for not more than four (4) years with a nonrefundable license fee of Fifty Dollars ($50.00) per year. Licensees may appeal to the board for an extension of the renewal period.(j) Each applicant for licensure and renewal shall apply to undergo a fingerprint-based criminal history records check of the Mississippi central criminal database and the Federal Bureau of Investigation criminal history database. Each applicant shall submit a full set of the applicants fingerprints in a form and manner prescribed by the board which shall be forwarded to the Mississippi Department of Public Safety and the Federal Bureau of Investigation Identification Division for this purpose. Occupational Description Independent Mental Health Practitioner: Independent mental health practice is the provision of treatment, assessment, psychotherapy, counseling, or equivalent activities to individuals, couples, families, or groups for behavioral, cognitive, social, mental, or emotional disorders, including interpersonal or personal situations. Independent mental health practice includes diagnosing major mental illness or disorder, using psychotherapy with individuals suspected of having major mental or emotional disorders, and/or using psychotherapy to treat the concomitants of organic illness, with or without consultation with a qualified physician or licensed psychologist. Independent mental health practice does NOT include the practice of psychology or medicine, prescribing drugs or electroconvulsive therapy, treating physical disease, injury, or deformity, or measuring personality or intelligence for the purpose of diagnosis or treatment planning. Mental Health Practitioner: Mental health practice means the provision of treatment, assessment, psychotherapy, counseling, or equivalent activities to individuals, couples, families, or groups for behavioral, cognitive, social, mental, or emotional disorders, including interpersonal or personal situations. Licensure Requirements 1. Have either: A. Graduated with a master's or doctoral degree from an educational program which is accredited, at the time of graduation or within four years after graduation, by the Council for Accreditation of Counseling and Related Educational Programs, the Commission on Accreditation for Marriage and Family Therapy Education, or the Council on Social Work Education; -OR- B. Graduated with a master's or doctoral degree from an educational program deemed by the board to be equivalent in didactic content and supervised clinical experience to an accredited program; 2. Either: A. Is licensed as a licensed mental health practitioner; -OR- B. is licensed as a provisional mental health practitioner and has satisfactorily passed an examination approved by the board pursuant to subdivision 3 of section 38-2122; 3. Either: A.Has 3,000 hours of experience supervised by a licensed physician, a licensed psychologist, or a licensed independent mental health practitioner, one-half of which is comprised of experience with clients diagnosed under the major mental illness or disorder category; -OR- B. All of the following: (a) Graduated from an educational program which does not meet the requirements of subdivision A of this subsection; (b) Is licensed as a licensed mental health practitioner, or is licensed as a provisional mental health practitioner and has satisfactorily passed an examination approved by the board pursuant to subdivision 3 of section 38-2122; (c) Has 7,000 hours of experience obtained in a period of not less than ten years and supervised by a licensed physician, a licensed psychologist, or a licensed independent mental health practitioner, one-half of which is comprised of experience with clients diagnosed under the major mental illness or disorder category. The experience required under this section shall be documented in a reasonable form and manner as prescribed by the board, which may consist of sworn statements from the applicant and his or her employers and supervisors. The board shall not in any case require the applicant to produce individual case records. Applicants for a Mental Health Practitioner license must: 1. Have received a master's degree, a doctoral degree, or the equivalent of a master's degree, as determined by the board, that consists of course work and training which

was primarily therapeutic mental health in content and included a practicum or internship and was from an approved educational program. Practicums or internships completed after September 1, 1995, must include a minimum of 300 clock hours of direct client contact under the supervision of a qualified physician, a licensed psychologist, or a licensed mental health practitioner; 2. Have successfully completed 3,000 hours of supervised experience in mental health practice of which 1,500 hours were in direct client contact in a setting where mental health services were being offered and the remaining 1,500 hours included, but were not limited to, review of client records, case conferences, direct observation, and video observation. For purposes of this subdivision, supervised means monitored by a qualified physician, a licensed clinical psychologist, or a certified master social worker, certified professional counselor, or marriage and family therapist qualified for certification on September 1, 1994, for any hours completed before such date or by a qualified physician, a psychologist licensed to engage in the practice of psychology, or a licensed mental health practitioner for any hours completed after such date, including evaluative face-to-face contact for a minimum of one hour per week. Such 3,000 hours must be accumulated after completion of the master's degree, doctoral degree, or equivalent of the master's degree; 3. Have satisfactorily passed an examination approved by the board. An individual who by reason of educational background is eligible for certification as a certified master social worker, a certified professional counselor, or a certified marriage and family therapist shall take and pass a certification examination approved by the board before becoming licensed as a mental health practitioner. All applicants must be at least 19 years of age and is a citizen of the United States, an alien lawfully admitted into the United States who is eligible for a credential under the Uniform Credentialing Act, or a nonimmigrant whose visa for entry, or application for visa for entry, is related to such employment in the United States.>h3>Fee Information Mental Health Practitioner license (MHP)- $155.00 Independent Mental Health Practitioner license (without MHP)- $155.00 Independent Mental Health Practitioner license (with MHP)- $50.00

Counselors, Professional License Description: Prerequisites for licensure by examination. Every applicant for licensure examination by the board shall: 1. Meet the degree program requirements prescribed in 18VAC115-20-49, the course work requirements prescribed in 18VAC115-20-50 or 18VAC115-20-51, and the experience requirements prescribed in 18VAC115-20-52; and 2. Submit the following to the board: a. A completed application; b. Official transcripts documenting the applicant's completion of the degree program and coursework requirements prescribed in 18VAC115-20-49 and 18VAC115-20-50 or 18VAC115-20-51; c. Verification of Supervision forms documenting fulfillment of the experience requirements of 18VAC115-20-52 and copies of all required evaluation forms; d. Documentation of any other professional license or certificate ever held in another jurisdiction; and e. The application processing and initial licensure fee. 18VAC115-20-45. Prerequisites for licensure by endorsement. A. Every applicant for licensure by endorsement shall submit the following: 1. A completed application; 2. The application processing fee; 3. Verification of all professional licenses or certificates ever held in any other jurisdiction. In order to qualify for endorsement the applicant shall have no unresolved action against a license or certificate. The board will consider history of disciplinary action on a case-by-case basis; 4. Documentation of having completed education and experience requirements as specified in subsection B of this section; 5. Verification of a passing score on a licensure examination in the jurisdiction in which licensure was obtained; and 6. An affidavit of having read and understood the regulations and laws governing the practice of professional counseling in Virginia. B. Every applicant for licensure by endorsement shall meet one of the following: 1. Educational requirements consistent with those specified in 18VAC115-20-49 and 18VAC115-20-51 and experience requirements consistent with those specified in 18VAC115-20-52; or 2. If an applicant does not have educational and experience credentials consistent with those required by this chapter, he shall provide: a. Documentation of education and supervised experience that met the requirements of the jurisdiction in which he was initially licensed as verified by an official transcript and a certified copy of the original application materials; and

b. Evidence of post-licensure clinical practice for five of the last six years immediately preceding his licensure application in Virginia. 3. In lieu of transcripts verifying education and documentation verifying supervised experience, the board may accept verification from the credentials registry of the American Association of State Counseling Boards or any other board-recognized entity. 18VAC115-20-49. Degree program requirements. A. Programs that are approved by CACREP or CORE are recognized as meeting the definition of graduate degree programs that prepare individuals to practice counseling and counseling treatment intervention as defined in 54.1-3500 of the Code of Virginia. B. The applicant shall have completed a graduate degree from a program that prepares individuals to practice counseling and counseling treatment intervention, as defined in 54.1-3500 of the Code of Virginia, which is offered by a college or university accredited by a regional accrediting agency and which meets the following criteria: 1. There must be a sequence of academic study with the expressed intent to prepare counselors as documented by the institution; 2. There must be an identifiable counselor training faculty and an identifiable body of students who complete that sequence of academic study; and 3. The academic unit must have clear authority and primary responsibility for the core and specialty areas. 18VAC115-20-50. (Expired.) 18VAC115-20-51. Coursework requirements. A. The applicant shall have completed 60 semester hours or 90 quarter hours of graduate study in the following core areas with a minimum of three semester hours or 4.0 quarter hours in each of the areas identified in subdivisions 1 through 12 of this subsection: 1. Professional identity, function and ethics; 2. Theories of counseling and psychotherapy; 3. Counseling and psychotherapy techniques; 4. Human growth and development; 5. Group counseling and psychotherapy, theories and techniques; 6. Career counseling and development theories and techniques; 7. Appraisal, evaluation and diagnostic procedures; 8. Abnormal behavior and psychopathology; 9. Multicultural counseling, theories and techniques; 10. Research; 11. Diagnosis and treatment of addictive disorders; 12. Marriage and family systems theory; and 13. Supervised internship of 600 hours to include 240 hours of face-to-face client contact. B. If 60 graduate hours in counseling were completed prior to April 12, 2000, the board may accept those hours if they meet the regulations in effect at the time the 60 hours were completed.

18VAC115-20-52. Residency. A. Registration. 1. Applicants who render counseling services shall: a. With their supervisor, register their supervisory contract on the appropriate forms for board approval before starting to practice under supervision; b. Have submitted an official transcript documenting a graduate degree as specified in 18VAC115-20-49 to include completion of the internship requirement specified in 18VAC115-20-50 or 18VAC115-20-51; and c. Pay the registration fee. 2. After September 3, 2008, applicants who are beginning their residencies in exempt settings shall register supervision with the board to assure acceptability at the time of application. B. Residency requirements. 1. The applicant for licensure shall have completed a 4,000-hour supervised residency in counseling practice with various populations, clinical problems and theoretical approaches in the following areas: a. Counseling and psychotherapy techniques; b. Appraisal, evaluation and diagnostic procedures; c. Treatment planning and implementation; d. Case management and recordkeeping; e. Professional identity and function; and f. Professional ethics and standards of practice. 2. The residency shall include a minimum of 200 hours of supervision between supervisor and resident occurring at a minimum of one hour and a maximum of four hours per 40 hours of work experience during the period of the residency. No more than half of these hours may be satisfied with group supervision. One hour of group supervision will be deemed equivalent to one hour of individual supervision. Supervision that is not concurrent with a residency will not be accepted, nor will residency hours be accrued in the absence of approved supervision. 3. The residency shall include 2,000 hours of face-to-face client contact. 4. A graduate-level internship completed in a program that meets the requirements set forth in 18VAC115-20-49 may count for no more than 600 hours of the required 4,000 hours of experience. The internship shall include 20 hours of individual on-site supervision, and 20 hours of individual or group off-site supervision. In order to count toward the residency, internship hours shall not begin until completion of 30 semester hours toward the graduate degree. 5. A graduate-level degree internship completed in a CACREP- ,CORE- or COAMFTE-approved program may count for no more than 900 of the required 4,000 hours of experience. 6. In order for any graduate-level internship to be counted toward a residency, either the clinical or faculty supervisor shall be licensed as set forth in subsection C of this section. 7. The board may consider special requests in the event that the regulations create an undue burden in regard to geography or disability which limits the resident's access to qualified supervision. 8. For applicants enrolled in an integrated course of study in an accredited institution leading to a graduate degree beyond the master's level, supervised experience may

begin after the completion of 30 graduate semester hours or 45 graduate quarter hours, including an internship, and shall include graduate course work in the core areas as prescribed in 18VAC115-20-50 or 18VAC115-20-51. 9. Residents may not call themselves professional counselors, directly bill for services rendered, or in any way represent themselves as independent, autonomous practitioners or professional counselors. During the residency, residents shall use their names and the initials of their degree, and the title "Resident in Counseling" in all written communications. Clients shall be informed in writing of the resident's status and the supervisor's name, professional address, and phone number. 10. Residents shall not engage in practice under supervision in any areas for which they have not had appropriate education. General examination requirements; schedules; time limits. A. Every applicant for initial licensure by examination by the board as a professional counselor shall pass a written examination as prescribed by the board. B. Every applicant for licensure by endorsement shall have passed a licensure examination in the jurisdiction in which licensure was obtained. C. A candidate approved to sit for the examination shall take the examination within two years from the date of such initial approval. If the candidate has not taken the examination by the end of the two-year period here prescribed: 1. The initial approval to sit for the examination shall then become invalid; and 2. In order to be considered for the examination later, the applicant shall file a new application with the board. D. The board shall establish a passing score on the written examination.
