JEFF BAUMANN ANNUAL CARDBOARD CANOE RACE PARENTAL WAIVER AND CONSENT FORM I, the undersigned, as the parent or legal guardian of ____________________ (print name), a minor child, age _____, do hereby give my full consent and approval for him/her to participate as a paddler in the Jeff Baumann Annual Cardboard Canoe Race on July 17, 2015. I understand that providing my consent means that my child will be racing a watercraft constructed from cardboard and packing tape in the Granger Meadows Park Koi Pond. I understand that there are certain risks of injury inherent to this activity, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating as a paddler in the Jeff Baumann Annual Cardboard Canoe Race and that my child is healthy and has no physical disabilities or infirmities that would restrict full participation in this activity. In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless DeWitt Charter Township, Granger and all sponsors of the Meadows Celebration, as well as all officers and representatives of these entities, for any injury that may be suffered by my child in the course of participation in this event, whether the result of negligence or any other cause.
