Our MissionThe Cambridge Community Activities Program encourages and promotes life-long wellness by providing a variety of high-quality recreational activities, learning opportunities, aquatics programs, and services to the broad Cambridge area community.

Letter from the Director…

The calendar tells us it’s spring and the sun is shining; surely summer has to be hiding around the corner. We sure hope it shows up soon because we have been busy planning some great opportunities for you this summer! Please take a minute to browse the program guide to see all we have to offer.

In a recent community survey, you told us you’d like additional avenues for adult recreation and we listened! We’ve put together bocce ball, disc golf and matball leagues for adults. We’ve also added to our fitness classes with a new yoga class and a core conditioning class called Hard Core. And you won’t want to miss the Gearing Up for Lake Ripley Ride class, which will not only teach you basic bike maintenance, but get you moving as well! If you prefer something educational, you’ll want to check out the new class for adults called iPads for Beginners; you’ll learn what the tablet craze is all about!

We also hope you will spend some time at Ripley Park this summer. This beautiful 18 acre beachfront park is a gem in our own backyard ~ be sure to enjoy it! Also check out the exciting lineup for concerts in Veteran' Park this summer.

Together with the Village of Cambridge, we have worked over the past several months on promoting our wonderful community. One reflection of this work can be found on pages 19 and 20. Through a collaborative effort, a new bike map has been made available for your use and convenience. These maps will be printed and distributed to surrounding communities to help promote Cambridge. Be sure to tear it out and utilize it this summer!

However you choose to spend your free time this summer, we hope you enjoy all that our wonderful community has to offer. Cambridge has something for everyone so let’s get out and get active!

Let’s head to outer space by visiting the UW Space Place! Students will discuss living in microgravity, why astronauts float in space, and how the space shuttle launches and returns to Earth. Please join in the fun at the monthly commu-nity dinner! This free dinner is open to all who live and work in the Cambridge area. Each evening is sponsored by a local organization or group. Volunteers are needed to help serve, greet, entertain with music, and clean up after the meal. Come warm your soul with home cooking and camaraderie!

A great opportunity for your child to improve their basketball fundamentals! Lead by CHS varsity coaching staff, the camp will empha-size the basic fundamentals of shooting, ball handling, passing, moves with and without a basketball, rebounding, and team defensive con-cepts. Your child will participate in daily league games. Also included in the cost of this camp is a commemorative t-shirt.

Our Softball Pitching Camp offers girls the chance to become a key leader on her team! Drills will teach or solidify fundamental pitching mechanics, while emphasizing game confidence and posture for the tough final inning mo-ments. Participants will also receive instruction on injury prevention to promote a long, healthy career. Players should bring their softball glove and supply their own catcher if available.

Our Baseball Throwing Mechanics Camp will provide its athletes with proper throwing mechanics, as well as injury prevention tips and how to avoid common throwing mistakes. Tech-nique and correction drills will improve your throwing accuracy and power, while improving your confidence! Participants should bring their own glove and baseball hat.

Disc Golf LeagueDates: Wednesdays, July 10 – August 14Time: 12:30 p .m . – 2:00 p .m . Grades: 7 – 12 Location: Ripley Park Cost: $12 R / $18 NR

This recreational disc golf league is designed to allow kids to get out and be active over the sum-mer. Kids will meet at the concession stand at Ri-pley Park to get their weekly match assignments. Kids will play in groups of four, but compete individually. Participants are encouraged to bring their own disc golf equipment; however, CAP has a limited supply that will be available for use. Minimum 6 participants.

Girls Volleyball CampDates: August 5 – 8 Time: 9:00 a .m . – 12:00 p .m . Grades: 3 – 8 Location: NMS New GymFee: $50 R / $65 NR

At this exciting camp, the Cambridge volleyball coaching staff will teach the fundamental skills of volleyball. Your child will learn passing, set-ting, serving, and spiking in a fun learning envi-ronment. Games, team building, and drills will be incorporated within the skill development to keep campers motivated. Also included in the cost of this camp is a commemorative t-shirt.

Recreational Soccer Dates: Saturdays, September 7 – October 12 Location: CES FieldsFee: $25 R / $38 NR*Registration Deadline: August 16

In this fun, exciting, recreational soccer league your child will learn fundamentals, rules and basic plays. Games are played at 8:00 a.m., 9:00 a.m., or 10:00 a.m., depending on registration numbers. Practices are held for the different teams at the discretion of their respective coach-es. You will be contacted by a coach regarding your child’s team assignment. Anyone inter-ested in coaching is asked to attend a coaches meeting on Wednesday, August 21 at 6:30 p.m. in the CES commons.

U6 Open to children currently enrolled in 4K or kindergarten. U6 is intended to be an introduc-tion to soccer, focusing on the fundamentals of the game and having fun. Children will play small sided soccer (3 on 3) on small sized fields (25 X 30 yards). They play four 8 minute quarters; referees and goalies are not used at this level.

U8 Designed for children in grades 1 and 2; chil-dren will play 4 on 4, on a 30 X 35 yard field. They will play four 10 minute quarters; referees and goalies are not used at this level.

U9 The U9 program will be offered to children in grades 3–4 whom are interested in continuing to play recreational soccer. The U9 program will be structured based upon enrollment numbers.

Due to a continued decrease in registration for grades 4–5, CAP will no longer be offering U10 soccer. However, we encourage all soccer enthusiasts to check out the Glacial Drumlin Soccer program. The league offered by CAP is strictly a recreational league, focusing on fun-damentals, equal play, and learning basic plays. Glacial Drumlin United (GDU) is a soccer club comprised of over 200 players from the com-munities of Cambridge, Deerfield, Lake Mills and Waterloo; with ages ranging from eight to nineteen. NFL Flag Football LeagueDate: Mondays & Wednesdays September 4 – October 16Grades: K – 5Location: CES FieldsFee: $40 R / $55 NR*Registration Deadline: August 16

NFL Flag Football will have your child begging for more! Your child will receive a top of the line NFL jersey as part of their participation. As tradition continues, players will participate in training camp on Wednesday, September 4 from 6:30 p.m. – to 7:30 p.m. lead by local coaches and players. A coaches meeting will be held on Tuesday, August 20th at 6:30 p.m. at the CES commons for anyone interested in coaching. Players will be contacted by the coaches as to your child’s team assignment. Due to the reg-istration process of NFL program, late registra-tions will NOT be accepted.

Every month this summer will be an open ended, themed art project for kids of all ages to create. This is a great excuse for the family to come to the beach, create, and enjoy the sun. Come ready to get messy and have fun! Minimum 5 participants.

Young Rembrandts Castle Workshop Date: Monday – Friday, June 17 – 21Time: 12:15 p .m . – 1:45 p .m .Ages: 6 – 12Location: CES CommonsFee: $55 R / $70 NR

We will jump into medieval times as we explore the land of castles, knights and dragons. Boys and girls alike will enjoy this exciting workshop filled with adventure. We will draw a variety of characters and items familiar to this time, including knights in armor, jesters juggling, jousters, catapults and more. We will even take our castle theme and enter the funny land of cartoons as we illustrate the humor of the time. Two large scenes will be created including one of a large castle with a moat and the other of a fiery dragon full of detail and color. Join us this summer for many adventures from this long-ago time. Minimum 6 participants.

Do you love animals? Join us for a trip through the zoo as we learn to cartoon a variety of animals found in the zoo. We will learn cartoon expressions, movement, exaggeration, se-quencing to tell a joke while drawing giraffes, elephants, bears, monkeys and more. Children will complete several cartoon drawings each day. Materials used include pencils, color pencils, Sharpies, and markers. Minimum 6 participants.

Young Rembrandts Enchanted Academy WorkshopDate: Tuesday – Thursday, August 27, 28, 29Time: 10:00 a .m . – 11:30 a .m .Ages: 6 – 12Location: CES CommonsFee: $35 R / $50 NR

Discover the charms of Young Rembrandts’ En-chanted Academy Workshop. Be prepared to be whisked away to an enchanted academy where nothing is as it seems. Experience the majesty of the great dining hall as our students illustrate a beautiful graphic. Amuse yourself in the not-so-ordinary life of the academy with a plethora of cartoon drawings. From drawing an enchanted forest to a day at the races with dragons, it’s nothing but fun in our classroom. No prior experience is needed. Minimum 6 participants.

CAP CARE is a state licensed group child care facility serving children in grades 4K – 4th .

CAP CARE has openings for the Fall 2013. 4K will be available from 11:00 a.m. – 6:00 p.m., Monday – Thursday, 6:00 a.m. – 6:00 p.m., Fridays. If you are interested in CAP CARE 4K please let the school district know and they will schedule your child appropriately.

Kindergarten – 4th grades has openings for before and after school as well, from 6:00 a.m. – 8:00 a.m. and 3:00 p.m. – 6:00 p.m. daily. For more information or questions please call (608) 423-8044.

Summer Day Camp is more than just summer day care, it’s an adventure! This state licensed Day Camp will offer hands on activities that will provide your child with fun, excitement, and learning opportunities. Each day new adventures are planned and exciting activities, games, and crafts are based around our weekly theme. Your child will spend the day using their imaginations and making new friends. Our Day Camp activities also include weekly trips to the Cambridge Library, Cambridge Area Commu-nity Pool, frequent trips to Westside Park, as well as special field trips each week.

Located in the NMS, the Youth Center offers a safe place for youth to participate in positive social, recreational, educational, and com-munity service activities under the supervision of professional staff. Each day includes a free healthy snack.

Activities include: open gym, homework club, crafts, video games, foosball, board & card games, football, movies, billiards, youth council, outdoor games & activities, field trips, ping pong, basketball, computers, socializing with friends, and filtered internet.

The Youth Center will be open this summer as a place for your child to get together with friends in a safe and supervised atmosphere. Students who attend the Youth Center during the sum-mer will enjoy numerous activities including outdoor games, open gym, movies, crafts, a free healthy snack, and more.

Starting this summer every Tuesday, Wednesday and Thursday the Youth Center will be provid-ing theme days. Tuesdays will be "Trekking Tuesdays," where nature walks and hiking will be encouraged. Wednesday will be hence forth known as "Water Wednesday," where students can bring in water related activities to enjoy outside all day. Thursday will be "Try Something New," whether it be a new sport or activity, students will be learning something new every Thursday. Students will need to bring a sack lunch from home. Our volunteer camp will be one your teen will never forget. Participants will feel empowered after giving their time to organizations and people who really need it. Not only will they partake in important community service activities, but they will also have fun and relax with activities like bowling, swimming, and a cook out. Cost in-cludes all camp supplies, transportation, admis-sion for reward trips, and a snack. This introductory cycling class will get you ready physically and mentally for the upcoming cycling season. Whether you are new to cycling or not, this class has something for you. Level 4 – Stroke ImprovementPrerequisites: Passed Level 3, or can swim 15 yards of coordinated front and back crawls using kick and alternating arm movement, perform rhythmic breathing and can jump into deep water. The objective of Level 4 is to develop confidence in the strokes learned and to improve other aquatic skills.

Skills: Swim underwater, perform feet first surface dive, survival swimming, open turns on front and back using any stroke, tread water using two different kicks, perform front crawl, breast-stroke, butterfly, back crawl, elementary back-stroke, sidestroke, streamlined position, head first entry from a compact and stride position, use safe diving rules, perform compact jump with lifejacket, perform throwing and reaching assist, safety rules.

Level 5 – Stroke RefinementPrerequisites: Passed Level 4, or can swim 25 yards back crawl and front crawl, 10 yards elementary backstroke, breaststroke kick and side-stroke kick, standing dive into pool and deep water bobbing.The objective of Level 5 is coordination and refinement of strokes.

Skills: Tread water, shallow dive, sculling, survival swimming, perform rescue breathing, standing dives, pike surface dive, tuck surfacdive, front flip turn, backstroke flip turn, refining and developing endurance for front crawl, breaststroke, butter-fly, back crawl, elementary backstroke, develop sidestroke.

Level 6 – Swimming and Skill Proficiency Fitness SwimmerPrerequisites: Passed Level 5, or can swim 25 yards breaststroke, elementary backstroke and sidestroke, 50 yards front and back crawls. Can do 15 yards butterfly, perform stride jump, open turns, and 2 minutes treading water. The objective of Level 6 Fitness swimmer is to refine strokes so students swim them with more ease, efficiency, power and smoothness over greater distances.

Skills: open turns, flip turns, circle swimming, using a pace clock, using pull bouys and fins. Swim 500 yard continuously, participate in the 12 minute Cooper swim test.

Water Fitness DescriptionsAquacizeJoin this fun, upbeat, and challenging class that will increase your cardio-vascular stamina while building muscle. Routines will include exercises in both the shallow and deep water. Come ready to sweat, work, and have a good time.

Fit after 50Designed to be a nonimpact, cardiovascular workout, this class will help build muscles, increase flexibility and improve functional fitness. Each class will be held in the shallow end of the pool and will end with a stretch-ing session for a total body workout.

*Lap Swim Open to all ages. You may swim at your own pace but you must swim laps and/or perform other aerobic water exercises. During Masters Swim, fitness classes, and lessons there may only be two lap lanes available.

Open SwimOpen to all ages. Children must be accompanied by an adult. At all times a responsible adult must be in the water and on the deck with a child who is under the age of seven. Children ages 7–11 must be accompanied by an adult in the pool area. There will always be at least one lane open for lap swimming during this time.

The Food Pantry provides food assistance, resources, and emergency aid to residents of the Cambridge School District. The Cambridge CAP is pleased to welcome Nancy Brattlie as the Food Pantry Coordinator. Nancy oversees the operations of the Pantry and works with volunteers, community organizations and local businesses to ensure the success of the Pantry.

Cambridge Food Pantry Garden Get involved in growing produce for the Cambridge Food Pantry. Volunteers are needed to help with the garden from May through October. Tasks at the garden include planting, watering, weeding, weighing, harvesting, and general clean up and maintenance of the crops. The garden, located 1.5 miles south of Cambridge on Oakland Road is a great opportunity for ALL ages to gain volunteer experience! Park InformationRipley Park, located on the west shores of Lake Ripley, is an 18 acre park privately owned by the Cambridge Foundation and managed by the CAP. This beautiful park has something for everyone! Ripley Park is home to two tennis courts, a basketball court, a children’s play struc-ture, and wide open space to play. Ripley Park is open seasonally from Memorial Day Weekend to Labor Day. Canoe, kayak and stand up paddle boards available for rental at the beach through CamRock Cafe and Sport.

Disc Golf CourseRipley Park is home to a 9-hole Disc Golf Course. This recreational-to-intermediate level course will be open to all levels of players. A limited number of discs are available for check out at the concession stand.

North Shelter offers 16 picnic tables underneath the shelter, with more tables available upon request. Additionally, North Shelter is in close proximity to the park restrooms and concession stand. The shelter is 30 ft x 40 ft, is equipped with electrical outlets, and has close access to a charcoal grill.

South Shelter . . . . . . . . . . . . . . .$60/DayThe South Shelter is a 24 ft x 36 ft shelter with electrical outlets. This shelter offers 11 – 14 picnic tables, with more available upon request. The South Shelter is located across the field space from the restrooms.

Evinrude Shelter . . . . . . . . . . . .$45/DayThe Evinrude Shelter is located adjacent to the restrooms and bath house. There are 4 – 6 picnic tables located under this 15 ft x 20 ft shelter, which is equipped with electrical outlets. The Evinrude Shelter is also the closest shelter to the parking lot.

Ripley Shelter . . . . . . . . . . . . . . .$45/DayThe Ripley Shelter is adjacent to the restrooms and bath house, and is identical in size and set up to the Evinrude Shelter. Ripley shelter overlooks the beach and playground area while staying in close proximity to the parking lot.

General Information

If you would like to participate in one of our programs, please register in advance of the reg-istration deadline. One week prior to the start date, we determine if enrollment is sufficient to hold the program. By registering early, you will assist us in providing the best organization and administration of all programs. Your coopera-tion in registering early will ensure all partici-pants and instructors are given ample time to prepare for programs.

RegistrationRegistration for enrichment programs is due one week prior to the start date. Please note all beginning dates on programs as to not miss a session. Registration for youth sports leagues is specified in that particular section of the brochure. If registrations are more than seven days late, registration is not guaranteed and may incur additional expenses. No email registra-tions will be accepted; phone registrations will be accepted with credit card payment. Registra-tion is taken on a first come, first served basis and programs may fill up quickly. Spots will not be held without payment.

TripsParticipants may register for trips by logging onto www.cambridgecap.net or by completing the registration form located in the back of this brochure. Space on trips is limited; registra-tions are taken on a first come, first served basis. There is a flat fee for all trips; both residents and non–residents of the Cambridge School District are encouraged to attend. Trip registra-tion is due two weeks prior to the date of each individual trip

If you don’t hear from us, that’s good! Unless you register online, the Cambridge Community Activities Program will not send out confirma-tions of registration. If you do not hear from us, consider yourself registered. We will only contact you in the case of a problem (payment inaccuracy, full program, cancellation). If you would like a confirmation or receipt of payment, please send a self addressed, stamped envelope with your registration. For all youth sports pro-gramming, participants will be contacted by his/her assigned coach.

FeesIt is the policy of the Community Activities Pro-gram that those who participate in Community Activities Programs should help subsidize the overall cost of that activity. Therefore, fees are assessed to activities based on the needs of that particular activity. The first program fees listed in the brochure are for Cambridge School District residents (R). To qualify for the resident fee you must reside in the Cambridge School District. The second fee listed is for non–residents (NR). Non–residents are those who do not reside in the Cambridge School District. Residents support the school district through tax–dollars, while non–residents pay to participate through higher registration fees. All participants must pay in full at the time of registration; however, participation in programs to School District residents will not be denied due to financial constraints.

Sunshine FundA private fund has been established to ensure all children are able to participate in CAP activities. We encourage all School District residents in need of financial assistance contact the Execu-tive Director for further information.

InsuranceThe Cambridge Community Activities Program does not provide medical or accident insurance for any program participants. Participants are encouraged to obtain their own insurance coverage prior to participating in programs.

Program CancellationsThe Cambridge Community Activities Program reserves the right to cancel a program due to low enrollment. Participants will be contacted at least three days prior to an activity being cancelled. Fees for programs cancelled by the Activities Program due to low enrollment will be refunded.Programs are subject to cancellation due to inclement weather, holidays, or facility conflicts. The CAP will attempt to reschedule a makeup session for such occasions, but this can not be guaranteed. For cancellation infor-mation, please call (608) 423–8108, or visit www.cambridgecap.net. Please note the following scenarios:When the School District of Cambridge is closed or dismisses early due to inclement weather, CAP activities will be cancelled. However, in the event of a late start or school cancellation due to inclement weather, 6 a.m. lap swim will operate as scheduled unless otherwise posted on NBC–15, WKOW–27, or WISC–TV3, as well as CAP voicemail by 5:30 a.m.

For public safety, the CAP reserves the right to close the pool should conditions arise that are deemed hazardous to pool patrons or staff. For the protection of all participants, the Cambridge Area Community Pool will be closed from the first sight of lightning or thunder until 30 minutes after the last sighting.

School District CooperationThe Community Activities Program would like to thank the Cambridge School District for their support and cooperation with facility use and scheduling. The extra time and energies from administrative assistants, custodians, teachers, and principals is greatly appreciated.

School District Facility Usage The School District facilities are available for use to members of the Cambridge Community per Board of Education policy 830. Any commu-nity member or group wishing to reserve space within the school district may do so by clicking on the School District of Cambridge website at www.cambridge.k12.wi.us and registering to become a requester. Simply click on the “Click here to reserve a facility” link and you will be prompted to a request page. Once you have been approved by the School District to become a requester, you will be able to use the program to apply for reservations. For additional infor-mation please contact the CAP office at (608) 423–8108.

Photo PolicyThe Cambridge Community Activities Program uses pictures of participants in bulletins and displays to inform others of our recreational op-portunities. If you do not want a picture taken, please inform the director prior to the start of the program.

Volunteer Opportunities Many volunteer opportunities are available with-in the Cambridge CAP, as volunteers are needed to make this program successful. Please list your name, telephone number, and the particular area that you are interested in volunteering with on the registration form. To ensure the safety of our participants, background checks are per-formed on all CAP employees and volunteers. *CAP is always looking for attractive and innovative programming ideas. Refund PolicyThe Cambridge Community Activities Program’s refund policy is as follows:A full refund will be issued in the following cases:•AprogramiscancelledbytheCAPdueto low enrollment or facility usage conflict.If a participant voluntarily withdraws from a program prior to the program’s registration deadline, he/she will have two options:•Receiveaprogramcreditfortheamount paid minus a $3 processing fee or•Receivearefundfortheamountpaid minus a $3 processing fee. All refunds will be mailed to participants in check form.A program credit will be issued in the following case:•Aparticipant’senrollmentiscancelled during the course of a program, unless that participant was needed to meet the minimum enrollment number set for the class.If a participant withdraws after the program has begun or if a participant is unhappy with the program, they may submit to the CAP Director, in writing, their disappointments and CAP will issue either a prorated refund or program credit minus the $3. 100% satisfaction guaranteed.

Behavior PolicyIt is the objective of the Cambridge CAP to provide a positive leisure experience for all participants. Appropriate behavior is expected from all participants and spectators in programs and in school district facilities to protect the en-joyment and safety of all patrons. Behaviors that will not be accepted include, but are not limited to, the following:•Endangeringthehealthandsafetyofother participants, patrons, or staff.•Disruptingaprogramorcreatinga disturbance at a facility.•Continuousrefusaltofollowprogramand/or facility rules and guidelines.•Useofverbalharassment,profanity, vulgarity, obscenity, or racial slurs.

•Blatantdisrespectofstaffandprogramor facility rules and guidelines.•Damage,vandalismortheftofprivateor school district facilities, equipment, or supplies.•Possessionoruseofillegalsubstancesor medication, including the use of alcohol or smoking where prohibited.•Possessionorthreatofaweapon.•Inappropriatedisplayofaffection.

The following outlines steps that may be used with patrons who are not behaving appropriate-ly. Membership or program fees will not be re-funded for any person removed from a program due to violation of this behavior policy. Some programs may have more detailed behavior poli-cies, such as the Blue Fins or the Youth Center, in which a printed copy will be distributed to all participants.

All program participants and spectators are expected to behave appropriately and follow the rules of the program. A participant will receive written notice for inappropriate behavior, and in the case of a minor, a copy of the notice from the CAP Director will be provided to such mi-nor’s parent/guardian. If a participant receives three notices of inappropriate behavior, or if the participant is involved in one severe incident, as determined by CAP staff, the participant may no longer participate in the program until a meeting is scheduled and held. The partici-pant, a parent/guardian if the participant is a minor, the program instructor, the program supervisor, and any other persons whom the CAP considers necessary for a resolution of the inappropriate behavior, shall be present at such meeting. At the meeting, the question of continued participation in the program will be considered and, primarily for minor partici-pants, a Behavior Contract may be prepared as a condition of continued participation. Such a Behavior Contract will be personalized to the circumstances and the participant, and will include specific expectations and consequences for failure to meet such expectations, including possible suspension and/or termination from program participation. Inappropriate behavior by adults may result in more rapid progress toward suspension or termination from program participation.

CamRock ParkCamRock Park

Dane County Parks offers this 422-acre park is located along Koshkonong Creek between the Village of Cambridge and Rockdale. This park has three separate use areas with shelter facilities and play areas. The trails are suitable for hiking, biking, cross-country skiing, and CamRock area 3 has a single-track mountain bike trail. INDEMNIFICATION AGREEMENT

GENERAL RECREATION PROGRAMS

I fully release and discharge the Cambridge Community Activities

Program and its employees, directors and volunteers from any and all

claims or damages, including claims or damages arising from injuries,

death or property damage, which may arise out of or occur in connec-

tion with my use of the school district facilities, the programs offered

by Cambridge CAP or allegedly caused by the negligence of Cambridge

CAP, except for those resulting from the intentional or reckless acts of

the Cambridge CAP or its employees, directors, and volunteers.

I further agree to indemnify and hold harmless the Cambridge CAP

and its employees, directors, and volunteers from any and all claims

or damages, costs, or expenses, incurred by the Cambridge CAP, its

employees, directors, and volunteers which result from or relate to my

use of the school district facilities and the programs offered by CCAP;

except for those resulting from the intentional or reckless acts of the

CCAP or its officers or employees.

I have been offered the opportunity to negotiate the terms and condi-

tions of this liability waiver and indemnification agreement; however,

I choose to accept the terms and conditions of this agreement as they

are, without negotiation.

AQUATICS PROGRAM

In addition to my participation in general activities programs, I

acknowledge that certain risks apply when using the Cambridge Area

Community Pool, under the direction of the Cambridge CAP.

The undersigned hereby recognizes and acknowledges that certain

risks of physical injury and property damage exist when participating

in the programs offered by the CCAP, including but not limited to the

drowning while swimming, and death or serious injury from diving,

swimming for extended periods of time or great distances, or falling

while participating in other activities offered by CCAP. I agree to

release all claims of injury or damage to or for myself or my child/ward

arising from the negligence of CCAP and any of its directors, officers,

employees or volunteers. I acknowledge that I have been provided

with adequate time to read this release, obtain any advice I might need,

and bargain for the opportunity to participate in activities without
