CAMT 2013 Registration Form Unlocking the Mathematical Mind Unlocking the Mathematical Mind Registration Confirmation A registration receipt will be mailed to you during the month of June. If you do not receive a copy of your receipt, please contact the CAMT office; we may not have received your registration materials from your school. Name Tag A name tag will be mailed to your home address in late June. The name tag is your ticket of admission to the conference. You will not be granted admittance to the conference without your name tag. Cancellations/Refunds/Transfer of Registration If you register in advance and cancel by June 1, half the registration fee will be returned. No refunds will be returned after June 1 regardless of the reason for cancelling. However, registration may be transferred to another participant. If transfer is made by June 1, 2013, you may contact the CAMT office by email or phone with the original participant’s name and the new participant’s name, home address, and email address. After June 1, transfer can only be made by giving the original participant’s mailed name tag to the new participant. Special Needs Attendees with special needs as defined by ADA should send requests by fax to (512) 355-8517 or by email to [email protected]. To ensure that your request is fulfilled, please contact us before June 10, 2013. DEADLINE: MAY 15, After May 15 – Register when you arrive at the conference.
