CAM-ICUFeature 4 – Altered LOC. Anything other than Ramsay 2 scores positive.Feature 1 – Acute Onset or Fluctuating Course. Is patient at their baseline mental status and have they been there for the last 24 hours? If not, then patient scores positive.Feature 2 – Inattention S A V E A H A A R T or pictures. Any score less than 8 is positive.Feature 3 – Disorganized Thinking. 4 yes/no questions and 1 command. Any score > 4 is positive.

CAM-ICU positive if patient is positive for BOTH features 1 AND 2 and EITHER feature 3 OR 4.
