Infant mortality and Postneonatal mortality
Provisional data for the United States for 1992 show that the birth rate per 1,000 population declined from the previous year, while the death rate remained steady. Between 1991 and 1992, a decrease was observed in the infant mortality rate per 1,000 live births. The marriage rate per 1,000 population decreased from the rate for 1991 and the divorce rate was unchanged.
Births
1992. This was a decrease of 5 percent from the provisional number of births reported for the previous December (342,000). The birth rate, 14.9 live births per 1,000 population, was 6 per- cent lower than the rate of 15.8 for December 1991. The fertility rate, 65.1 live births per 1,000 women aged’ 15-44 years, was 5 percent lower than the comparable rate for December 1991 (68.3). The seasonally adjusted fertility rate (66.8) also was 5 percent lower than the comparable rate a year earlier (70.1).

reported for the previous year. The birth rate of 15.9 was 2percent lower than the rate of 16.2 for 1991. The fertility rate for 1992 was 69.3, slightIy lower than the rate for 1991 (69.6). These lower rates continue the gener- ally downward trend observed since early 1991.
Natural increase
As a result of natural increase, the excess of births over deaths, an esti- mated 139,000 people, or 6.4 persons per 1,000 population, were added to the population during December 1992.
During 1992, 1,907,OOO persons were added to the population. This represents a rate of natural increase of 7.4, 4 percent lower than the rate of 7.7 for 1991. The decline in the rate of natural increase is due entirely to a decrease in the birth rate because the death rate remained steady.
Marriages
Marriages performed in Dec- ember 1992 numbered 184,000, a 2-percent increase over the number for December a year earlier (180,000).S The marriage rate per 1,000 popula- tion for December was 8.4 in 1992, 1 percent higher than for the previous year (8.3).
The provisional number of mar- riages for January-December 1992 was 2,362,000, a slight decline from the previous year (2,371,OOO). Despite the small change in the number of mar- riages, the marriage rate for 1992 (9.2) was 2 percent lower than for 1991 (9.4) because of the increase in the population.
Divorces
Divorces granted in December 1992 numbered 105,000, 6 percent higher than for December of the pre- vious year (99,000). The divorce rate per 1,000 population for December increased 4 percent from 1991 (4.6) to 1992 (4.8).
In 1992 there were 1,215,OOO divorces, a 2-percent increase over the number for 1991 . (1,187,OOO). The increase in the number of divorces

matched the increase in the popula- tion, so the divorce rate remained steady at 4.7.
Deaths
The provisional count of deaths estimated for the United States during 1992 totaled 2,177,000, about 12,000 more than in the previous year. The death rate of 8.5 per 1,000 population was the same as the rate for 1991. Approximately 34,400 of these deaths were to infants under 1 year of age. The infant mortality rate was 8.5 per 1,000 live births, 4 percent low,er than the rate of 8.9 for 1991.
Current Mortality Sample, 12 months ending with November 1992- The provisional death rate for the 12 months ending with November 1992 was 854.6 deaths per 100,000 popula- tion compared with a rate of 853.2 for the 12-month period ending with November 1991. The change in the death rate was not statistically signif-
- icant. The provisional age-adjusted death rate for the 1Zmonth period ending with November 1992 was 503.4 deaths per 100,000 population, 1 per- cent lower than the rate of 507.1 for the 1Zmonth period ending with November 1991. Age-adjusted death rates control for changes and varia- tions in the age composition of the population; therefore, they are better indicators than crude rates for showing changes in mortality risk over time and for showing differences between race-sex groups within the population. Among the race-sex groups, the esti- mated age-adjusted death rates decreased for white males and white females. By age, the death rate for the total population decreased for the age groups under 1 year and 85 years and over but increased for the age group 35-44 years.

periods for Chronic obstructive pulmo- nary diseases and Human immunode- ficiency virus infection. The death rate decreased between the two successive 1Zmonth periods for Motor vehicle accidents.

The infant mortality rate for the 12 months ending with November 1992 was 845.5 per 100,000 live births, 6 percent lower than the rate of 897.3 for the same 12-month period a year earlier. For infants under 28 days, the 12-month rate ending with November 1992 was 532.9 compared with a rate of 560.8 for the 12-month period a year earlier. The infant mortality rate for infants 28 days to 11 months was 313.3 compared with a rate of 336.3 for the 12-month period a year earlier. Mortality Surveillance System
Discussed this month are recent trends in infant mortality rates (deaths to infants under 1 year of age per 100,000 live births), neonatal mortality rates (under 28 days), postneonatal mortality rates (28 days to 11 months), and trends in infant mortality rates for Sudden infant death syndrome. In this issue, final mortality data are analyzed for data year 1990 and provisional data from January 1982-April 1992.
In 1990, the latest year for which 1988 1989 1990 1991 1992

total of 38,351 deaths occurred to infants under 1 year. Of these, 24,309 deaths or 63 percent of all infant deaths were to infants under 28 days and 14,042 deaths or 37 percent of all infant deaths were to infants 28 days to 11 months.

infant mortality rate was 922.3 infant deaths per 100,000 live births. The neonatal mortality rate was 584.6 per 100,000 live births, 1.7 times the post- neonatal mortality rate of 337.7. Infant mortality rates, neonatal mortality rates, and postneonatal mortality rates have been generally decreasing since the early 1960’s.

Nature and sources of data
Data in this report are provisionalunless otherwise specified and includeonly events occurring within theUnited States, Mortality data excludefetal deaths.
Beginning with the January 1991issue, birth, death, and infant deathfigures for all years shown in tables 2and 4 for each State are estimates byState of residence. These estimates arederived by applying adjustment ratiosto the actual counts of certificates forall events occurring in the State andreceived in registration offices during al-month period regardless of date ofthe event. The adjustment ratios foreach data year representthe observedrelationship between final State occur-rence and residence figures for the 3most recent years for which final datawere available, expressed as a singleratio for each State. As in previousyears, monthly State marriage anddivorce figures represent the actualcount of all events occurring in theState (State of occurrence) that werereceived in the registration officesduring the l-month period. Beforethe January 1991 issue, provisionaldata for all events were by State ofoccurrence. Delay in the receipt ofcertificates in a registration office mayresult in a low State figure for a givenmonth followed by a high figure for themonth(s) in which the delayed recordsare received. Data for previous monthsand cumulative data include revisedfigures received from the States.
Figures for births, deaths, andinfant deaths for all years for Cali-fornia shown in tables 2 and 4 containadjustments for varying length of Statereporting periods. Beginning with data for January1991, US, totals for births, deaths, andinfant deaths are based on the Stateestimates by State of residence(described above) and, therefore, ineffect, exclude events to nonresidentsof the United States. Events to nonres-idents of the United States areincluded in all marriage and divorcefigures for all years and in the U.S.birth, death, and infant death figuresfor 1990 and previous years. The effectof excluding events to nonresidentsfrom the US, totals is small.
Provisional totals for the UnitedStates include estimates for State datashown as not available. Provisionaltotals for births and marriages for theentire United States include adjust-ments for observed differencesbetween provisional and final monthlyfigures.
Divorce figures include reportedannulments. The monthly nationaldivorce estimate is obtained by multi-plying the total for the reporting areasby the ratio observed between themost recent final annual divorce totalfor the United States and the provi-sional total for the reporting areascombined.
Randonz variation – Although thecounts in this report are not subject tosampling variability (except the Cur-rent Mortality Sample, see below),they may be affected by random varia-tion. When the number of events issmall and the probability of such anevent is small, considerable cautionmust be observed in interpreting thedata. Such infrequent events may beassumed to follow a Poisson proba-bility distribution. For this distributiona simple approximation maybe used toestimate the random variation, asfollows:

Rates
Rates are on an annual basis and,except for infant mortality rates, areper 1,000 or 100,000 estimated popula-tion residing in the United States. Thepopulations used for computing theserates are furnished by the US. Bureauof the Census. The population esti-mates are based on the 1980 Census ofPopulation and do not reflect theresults of the 1990 census enumera-tion. Monthly rates are based on pop-ulations estimated for the specificmonth. Year-to-date rates are aver-ages of the monthly rates weighted bythe number of days in each respectivemonth. Rates for 12-month periods arethe sum of events for the period perpopulation estimated at the midpointof the period.
Infant mortality rates are infantdeaths for the specified period(monthly, year-to-date, 12-monthperiod) per 1,000 or 100,000 livebirths. Births used for computingmonthly and year-to-date infant mor-tality rates are adjusted for monthlyvariation in the number of births;births used to compute 12-month ratesdo not contain this adjustment. Birthsused for computing infant mortalityrates are not corrected for observeddifferences between provisional andfinal monthly figures as described insection on Nature and sources of data.Because monthly infant mortality ratesare based on relatively few events, theyare highly variable. Therefore, com-parisons between monthly infant

mortality rates should be interpretedcautiously see section on Randomvariation.
The age-adjusted rates presentedin this report are computed by thedirect method, that is, by applying theage-specific death rates to the stan-dard population distributed by age.Age groups shown in table 5 were usedto compute the age-adjusted ratesshown in that table, The age-adjustedrates upon which the State maps arebased were computed from age-specific rates in 10-year age groups. Inboth instances the total population asenumerated in 1940 was selected asthe standard. It is important not tocompare age-adjusted death rates withcrude rates.
Current Mortality Sample ,
.The Current Mortality Sample(CMS) is a 10-percent systematicsample of death certificates drawneach month after the certificates arecounted in the State registrationoffices. Deaths and death rates by age,race, sex, and cause are based on thesample. Because of the additional timerequired to select and process the cer-tificates, data based on the CMS arepublished 1 month after publication ofthe U.S. and State counts. Completeinformation concerning the underlyingcause of death is sometimes not avail-able when the sample is drawn. As aresult, estimates based on samplecounts for certain causes are biased,Correction for bias is shown in theannual summary (issue No. 13 in thisseries) for each year.
Estimated numbers of deaths anddeath rates based on the sample wereproportionately adjusted to be consis-tent with estimates based on the countof death certificates received in Stateregistration offices.
HW infection– Beginning withdata for 1987, NCHS introduced cate-gory numbers “042–’044 for classifyingand coding human immunodeficiencyvirus infection (HIV infection), for-merly referred to as human T-cellIyrnphotropic virus-111/lymphadenop-athy-associated virus (HTLV-IIIUV)infection. The asterisk before the cat-egory numbers indicates that thesecodes are not t)art of the Ninth.
Revision of the International Classifi-cation of Diseases. Deaths classified tothese categories are included in Allother infectious and parasitic diseasesin the List of 72 Selected Causes ofDeath as well as shown separately atthe bottom of table 6.
Sampling variability– Because theestimates of deaths and death ratespresented in this report (with theexception of total deaths and deathsunder 1 year) are based on a sample ofdeath certificates, they are subject tosampling variability. The estimated rel-ative standard error shown in the tableis a measure of the sampling error ofthe estimated number of deaths (or ofthe estimated death rate) expressed asa percent of the estimate. The firstcolumn refers to monthly estimates;the second to annual; cumulative year-to-date totals will be between the two.
The chances are about 2 of 3 thatthe percent difference between an esti-mate and the result of a completecount is less than the percent shown.The chances are about 19 of 20 thatthe percent difference is less thantwice the percent shown, Figuresbased on 100 or fewer estimateddeaths have relative standard errors of30 percent or more and are, therefore,considered unreliable. Rates based on100 or fewer estimated deaths are indi-cated by an “*”.

Mortality SurveillartceSystem-TheMortality Surveillance System chartsare based entirely on monthly provi-sional data from th(: CMS. This procedurehas the advantage of avoiding negativeprediction intervals for the model. Themodels, parameter estimates, and sta-tistical tests for lack of fit are availableupon request for the charts publishedin the Mortality Surveillance System.Time series regression models havepreviously been used to describetrends in mortality data (2-4). A list ofcause-of-death topics is presented inthe table on the back of this report.
State Maps
Unlike other data presented inthis report, the State maps are basedon final instead of provisional data.The infant mortality rates used to pro-duce the first State map were com-puted by using 3-year total numbers ofinfant deaths and live births for1987-89. The assignment of the Statesinto the given shadings on the mapswas carried out in two steps: a) deter-mining whether the State infant mor-tality rate differed significantly fromthe corresponding U.S. rate at the 0.05level of significance; b) then groupingthe State rates found to be significantlydifferent from the U.S. rate into thefour categories: 10 highest State ratesof those significantly greater than theU.S. rate, remaining State rates signif-icantly greater than the U.S. rate, 10lowest State rates of those significantlylower than the U.S. rate, andremaining State rates significantlylower than the U.S. rate. Infant mor-tality rates and the corresponding95-percent confidence intervals areshown in the table. The symbols “T”and “tT’> shown in the table are used
to denote State rates that differ signif-icantly from the U.S. rate at the 0.05and 0.01 levels of significance, respec-tively. Different procedures were usedto determine tests of statistical signifi-cance and confidence intervals,depending on the number of infantdeaths,

For 1-49 infant deaths, the lowerand upper 95-percent confidence limitswere estimated as described elsewhere(5). The difference between the Stateand U.S. infant mortality rates wasdetermined to be statistically signif-icant at the 0.05 or 0.01 level if theirrespective 95-percent or 99-percentconfidence limits did not overlap.
To produce the second State map,postneonatal mortality rates instead ofinfant mortali~ rates were used.Except for the substitution of infantdeaths with postneonatal deaths, all ofthe above procedures used in pro-ducing the first State map also apply inproducing the second State map.
