Description and Requirements

Proof of Installation is required for all orders containing any Product Line that is Instrument or Parts or DEMO order

Proof of Installation must be provided on IDR letter head and acceptance must be signed, stamped and dated by the End User.

Incase if any End-user does not have the stamp – Then the proof of Installation must be provided on End User letter head.

The template must be filled with the following mandatory information in the Product Information section

– Agilent SAP Order Number

– All parts, Instruments, sub parts of the order listed under the table of Product Information

– Serial No. and Model Number of each part / material listed against the item.

• The only case where Serial No. and Model Number would be N/A when Agilent has not provided the same. For e.g sub items of GC/GCMS may not have a Serial No. or Model No. available. However all items must be listed in the Product Information category

Situation & Requirement

Situation-1

If the order (containing any Product Line for Instrument / parts) was placed to Agilent with End User data.

• Proof of installation must be provided with a maximum of 4 weeks of Installation

• Proof of Installation must be provided on IDR letter head and acceptance must be signed, stamped and dated by the End User.

• Incase if any End-user does not have the stamp – Then the proof of Installation must be provided on End User letter head.

• The template must be filled with all mandatory information in the Product Information section described in 1st slide.

Situation & Requirement

Situation-2

If the order (containing any Product Line for Instrument / parts) was placed to Agilent as Instrument Stock Order.

Proof of installation must be provided within a maximum of 3 months of delivery or when it is sold to a customer wherever is earlier.

• If the Instrument Stock order is sold to a customer, IDR must provide the end user data prior to selling instrument to the CSR (to include PO number where the instrument belongs) along with complete details of End User, Ship to (if any other than end user) and details of any other intermediate ship to party involvement. The instrument must not be sold till the CSR provides an approval for the same

• Incase if the Instrument is still not sold to any customer and is still at the IDR’s facility then they must fill the same template on their letter head with mandatory Product information and remove the customer acceptance section (Page No. 2) from the template and specify “The order is still at IDR site and end user information will be made available once the order is sold – maximum in additional 3 months time frame.

If the order was placed to Agilent as DEMO Stock Order.

Proof of installation must be provided within a maximum of 6 months of delivery or when it is sold to a customer wherever is earlier.

• If the DEMO Stock order is sold to a customer, IDR must provide the end user data prior to selling instrument to the CSR (to include PO number where the instrument belongs) along with complete details of End User, Ship to (if any other than end user) and details of any other intermediate ship to party involvement. The instrument must not be sold till the CSR provides an approval for the same

• Incase if the DEMO Instrument is still not sold to any customer and is still at the IDR’s facility then they must fill the same template on their letter head with mandatory Product information and remove the customer acceptance section (Page No. 2) from the template and specify “The order is still at IDR site and end user information will be made available once the order is sold – maximum in additional 6 months time frame.

Important Info. for Stock Orders

For all Stock orders (Instrument as well as DEMO) wherein the Instrument is proposed to be sold to a customer.

Please DO NOT SELL the instrument, instead provide the end user data prior to selling instrument to the CSR

(to include PO number where the instrument belongs) along with complete details of End User, Ship to (if any other than end user) and details of any other intermediate ship to party involvement.

The instrument must not be sold till the CSR provides an approval for the same
