• smaller operators with 1 well – 1,300 operators• 2-20 wells – 1,900 operators• 21-110 wells – 400 operators• > 110 wells - 150 operators - reminder letter only
If no letter received by early March – DEP may not have good address.

• File Electronically via web site (preferred)
First Time Sign-in - confirm agreement for electronic submission
Option to complete report on-line with the on-line form;
OR,
If you have over 100 wells you have the additional option to download an ACCESS database of your inventory to your computer and either key the information into each record or import your data for all wells.

Upload XML file in schema format.

Completion Reports - 2
1. Pursuant to Section 212(b) of the Oil and Gas Act and Section 78.122(a) of Chapter 78 of the Oil and Gas Regulations, a Well Record must be submitted to the Department within thirty (30) days of cessation of drilling or altering a well.
2. Pursuant to Section 212(b) of the Oil and Gas Act and Section 78.122(b) of Chapter 78 of the Oil and Gas Regulations, a Completion Report must be submitted to the Department within thirty (30) days of completion of the well.
