Dear Constituent, This new directory highlighting organizations from the Eastern Shore and Southern Maryland represents a new beginning in the community engagement strategy of the Office of Minority health and Health Disparities (MHHD). In terms of context and history, the Office of Minority Health and Health Disparities (MHHD) was established in 2004 by statute, under the Maryland Health General Article, Section § 20-1001 to § 20-1007, to address minority health disparities in Maryland. Our mission is to address the social determinants of health and eliminate health disparities by leveraging the Department’s resources, providing health equity consultation, impacting external communications, guiding policy decisions and influencing strategic direction on behalf of the Secretary of Health. Our vision is to achieve health equity where all individuals and communities have opportunity and access to achieve and maintain good health. Our directory extends our history to reality and springs our mission and vision into actions and produces collaborative partnerships. During 2020 and onwards, additional organizations on the Eastern Shore and Southern Maryland as well as from other regions of the State will be added to form a statewide platform for transformative collaboration and coordination. Combined and comprehensive efforts, directed towards reducing and eliminating health disparities and advancing health equity, will help achieve our goal: improvement in minority health everywhere in the state of Maryland. I present to you this inaugural edition of community and faith-based organizations. It is dedicated to enable you to sit at the table, generate resources and engage in public education decisions which are keys to increasing health prevention efforts. It also enables efforts in screenings, referrals, treatment and recovery; and opportunities for linking to clinical and social services. These combined strategies provide solutions to infant and maternal mortality, asthma, prediabetes and diagnosed diabetes and their social determinants such as housing, transportation, food security and skills for jobs. Thanks for joining the MHHD in improving health and wellbeing through transformative actions.
