1. At least one meaningful picture2. Your favorite quote – think of this as

“words to live by,” like really good advice

3. The name of a person who inspires YOU and WHY they are your inspiration

4. One dream you have for your future – think of this as a realistic, long-term goal

5. What is one thing “holding you back”? Think of this as a challenge you may have to overcome in order to make your dream come true.

• Describe yourself in ONLY 3 words.• When your children, are the age you are today, what is
