I t i s s u g g e s t e d t h a t d o s a g es c h e d u l e s s h o u l d b e f l e x i b l e a n d t a i l o r e d t o t h e n e e d s o fe a c h i n d i v i d u a l p a t i e n t , a n d t h a t treatnent s h o u l d b e con-t i n u e d f o r s i x m o n t h s b e f o r e p r e s u m i n g i t t o b e i n e f f e c t i v e .

I t i s c o n c l u d e d t h a t L - d o p a o f t e n a m e l i o r a t e s P a r k i n -s o n i s m f o r l o n g p e r io ds , b u t i t s e f f e c t on t h e n a t u r a l h i s -t o r y o f t he d i s e a se i s u n d e t e r m i n e d .

I n t r o d u c t i o nDopamine i s w i d e l y d i s t r i b u t e d i n t h e b o d y , a n d i t s v a r y i n gc o n c e n t r a t i o n s i n t h e t i s s u e s r e f l e c t i t s u s u a l r o l e a s a p re -cursor o f n o r a d r e n a l i n e . The c a u d a t e n u c l e u s a n d p u t a m e n ,h o w e v e r , a re r i c h i n d o p a m i n e ( C a r l s s o n , 1 9 5 9 ) , t h o u g h t h e s es t r u c t u r e s c o n t a i n l i t t l e n o r a d r e n a l i n e ( V o g t , 1 9 5 4 ) . T h i s d i s -crep ancy s u g g e s t e d t h a t d o p a m i n e i n t h i s r e g i o n m i g h t i t s e l fb e a c t i v e p h y s i o l o g i c a l l y . E h r i n g e r a n d H o r n y k i e w i c z ( 1 9 6 0 )s h o w e d t h a t t h e d o p a m i n e content o f t h e c a u d a t e n u c l e u s a n dp u t a m e n was g r e a t l y r e d u c e d i n p o s t - m o r t e m s p e c i m e n st a k e n f r o m p a t i e n t s w i th P a r k i n so n is m .

T r e t i a k o f f ( 1 9 1 9 ) f i r s t s u g g e s t e d t h a t t h e p r i n c ip a l l e s i o nc a u s i n g P a r k i n s o n i s m l a y i n t h e s u b s t a nt i a n i g r a . T h i sh y p o t h e s i s was s u p p o r t e d b y H a s s l e r ( 1 9 3 8 ) . Many f i b r e s runf r o m t h e s ub s t a nt i a n i g r a t o t h e p a l l i d u m ( C a r p e n t e r a n dM c M a s t e r s , 1 9 6 4 ) , t h o u g h f e w , i f a n y, run i n t h e reversed i r e c t i o n . E f f e r e n t f i b r e s from b o t h p a r t s o f t h e s t r i a t u m , t h eput a m e n a n d c a u d a t e n u c l e i , c o m m u n i c a t e w i t h t h e g l o b u sp a l l i d u s , a n d a d i r e c t c o n n e x i o n b e t w e e n t h e s t r i a t u m a n d t h es u b s t a n t i a n i g r a h a s b e e n d e f i n e d ( R u n d l e s a n d P a p e z , 1 9 3 7 ) .

Bloom e t a l . ( 1 9 6 5 ) s h o w e d t h a t d o p a m i n e i n h i b i t s t h e f i r i n go f c e l l s i n t h e c a u d a t e n u c l e u s a n d t h a t a c e t y l c h o l i n e i n c r e a s e st h e f i r i n g r a t e .

T h e s e b i o c h e m i c a l a n d a n a t o m i c a l s t u d i e s d o n o t com-p l e t e l y c l a r i f y th e p h y s io l og ic a l d e f e c t s w h i c h r e s u l t i n t h es y n d r o m e o f P a r k i n s o n i s m , b u t t h e y do i n d i c a t e a r a t i o n a la p p r o a c h . They suggest t h a t b e t w e e n t h e s u b s t a n t i a n i g r a , t h ep a l l i d u m , a n d t h e s t r i a t u m a c e t y l c h o l i n e i s an e x c i t a t o r yt r a n s m i t t e r , w h i l e d o p a m i n e t h e r e a c t s i n an i nh i b i to r y f a sh -i o n . P a t i e n t s

T h i r t y - t w o p a t i e n t s ( 1 8 men a nd 1 4 women) were t r e a t e dw i t h L - d o p a . T h e i r ages r a n g e d from 42 t o 7 6 , average 6 1 y e a r s .S i x p a t i e n t s were o v e r 7 0 a n d f o u r were l e s s than 5 0 . F a r k i n -s o n i a n s y m p t o ms ha d been p r e s e n t f o r p e r i o d s r a n g i n g from 3t o 4 7 y e a r s .

The l i k e l y a e t i o l o g y o f P a r k i n s o n i s m was a s s e s s e d i n e a c hcase. Many causes a re r e c o g n i z e d , b u t i t i s g e n e r a l l y a c c e p t e dt h a t m ost cases ca n b e c a t e g o r i z e d under t h e t h r e e h e a d i n g so f p o s t e n c e p h a l i t i c , i d i o p a t h i c ( o r p a r a l y s i s a g i t a n s ) , a nda t h e r o s c l e r o t i c , t h o u g h t h e e n t i t y o f a t h e r o s c l e r o t i c P a r k i n -s o n i s m h a s b e e n d i s p u t e d . I t i s s o m e t i m e s d i f f i c u l t t o p l a c e i n d i -v i d u a l p a t i e n t s i n t o one o f t he s e c a t e g o r i e s w i t h c e r t a i n t y . Of t h ep r e s e n t group, s i x c o u l d c o n f i d e n t l y b e l a b e l l e d as p o s t -e n c e p h a l i t i c on t h e b a s i s o f t h e ir h i s t o r i e s and c l i n i c a l f e a t u r e s .The e a r l y a ge o f onset o f s y m p t o ms s u g g e s t e d an e n c e p h a l i t i ca e t i o l o g y i n a f u r t h e r t h r e e p a t i e n t s , b u t t h e l a c k o f c o n f i r -m at ory h i s t o r y a n d o f c h a r a c t e r i s t i c s i g n s l e a v e s t h e d i a g n o s i si n d e t e r m i n a t e i n t h e s e p a t i e n t s . Most p a t i e n t s ( 2 4 ) werel a b e l l e d i d i o p a t h i c p a r a l y s i s a g i t a n s , b u t i n a t l e a s t f o u r a ndp o s s i b l y i n some o f t h e o t h e r e l d e r l y p a t i e n t s c e r e b r a la t h e r o s c l e r o t i c c h a n g e s may w e l l have p l a y e d a p a r t i n p r o d u c -i n g t h e i r d i s a b i l i t i e s . Four p a t i e n t s had u n d e r g o n e p r e v i o u s sur-g i c a l treatment f o r t h e i r P a r k i n s o n i s m . There was no s e l e c t i o no f p a t i e n t s b e c a u s e o f age o r s e v e r i t y o f a f f e c t i o n , b u t t h o s ewho were d e m e n t e d or w ho gave a c l e a r h i s t o r y o f c e r e b r o -v a s c u l a r d i s e a s e were e x c l u d e d from t h e t r i a l .

Denny Brown ( 1 9 4 9 ) e m p h a s i z e d t h e i n t e r r e l a t i o n -s h i p b e t w e e n t h e s e f e a t u r e s . He h a s shown t h a t b o t h tremor a n dr i g i d i t y r e p r e s e n t c o n t r a c t i o n i n th e same m u s c l e g r o u p s . T h e r e a f t e r , t r e a t m e n twas c o n t i n u e d i n t h e o u t p a t i e n t c l i n i c , where p a t i e n t s werereviewed a t f o r t n i g h t l y i n t e r v a l s . In a l l b u t t h r e e p a t i e n t so t h e r a n t i - P a r k i n s o n i a n d r u g s were s t o p p e d w h i l e L - d o p awas g i v e n .

At t h e b e g i n n i n g o f t h i s s t u d y t h e d r u g used was g i v e n i nf r e s h l y m a d e d o s e s from a b u l k p o w d e r which was p r e s e r v e di n a w a t e r - t i g h t c o n t a i n e r . T h e p h a r m a c e u t i c a l s t a b i l i t y a n dp o t e n c y o f t h i s p r e p a r a t i o n could not b e g u a r a n t e e d . S e v e np a t i e n t s i n a l l were t r e a t e d with t h i s p r o d u c t f o r s h o r t p e r i o d s .L a t e r , a s u p p l y o f s t a b l e L - d o p a was o b t a i n e d . This w ass u p p l i e d i n c a p s u l e s , each o f which c o n t a i n e d 1 5 0 mg. o f L -d o p a o f a c o n s i s t e n t l y h ig h d e g r e e o f p u r i t y ( n o t l e s s t h a n99 ' o , ) . R e c en tl y , c a p s u l e s c o n t a i n i n g 5 0 0 m g. o f t h e d r u g h a v eb e e n a v a i l a b l e , a n d t h i s i s now t h e standard p r e p a r a t i o n u s e d .Each p a t i e n t was s t a r t e d on 0 . 9 g . o f L - d o p a d a i l y , g i v e n i nt h r e e d i v i d e d d o s e s . The d r u g was administered a f t e r m eals t ominimize nausea. T h e dose was i n c r e a s e d b y a m o u n t s o f a 'f u r t h e r 0 . 9 g . a t i n t e r v a l s o f t h r e e t o f o u r d a y s . As t h e t o t a ld a i l y d o s a g e i n c r e a s e d , t h e num b e r o f d o s e s was a l s o i n -c r e a s e d up t o a maximum o f e i g h t d a i l y . When n ause a a n dv o m i t i n g were d i s t r e s s i n g t o p a t i e n t s t h e y were a l s o g i v e nc y c l i z i n e 5 mg. t h r i c e d a i l y .

T h e i n c r e m e n t s o f L - d o p a were c o n t i n u e d u n t i l an a d e -q u a t e t h e r a p e u t i c response was a c h i e v e d , o r u n t i l t r o u b l e s o m es i d e - e f f e c t s s u p e r v e n e d . When p a t i e n t s d e v e l o p e d d i s t r e s s i n gs i d e - e f f e c t s treatment sometimes ha d t o be d i s c o n t i n u e d ,o t h e r w i s e t h e d o s e was reduced t o t h e h i g h e s t t o l e r a t e d l e v e la nd p a t i e n t s were t h e n t r e a t e d f o r an i n i t i a l p e r i o d o f s i xw e e k s . I f no s i g n i f i c a n t i m p r o v e m e n t was s e e n a t t h e e nd o ft h i s t i m e , t h e treatment was d i s c o n t i n u e d . When a w o r t h -w h i l e response was o b t a i n e d L - d o p a was c o n t i n u e d . T he e f -f e c t i v e d a i l y d o s e v a r i e d b e t w e e n 1 . 5 a nd 9 g . At t h e time o fd i s c h a r g e from h o s p i t a l m os t p a t i e n t s were t a k i n g b e t w e e n 4a n d 6 g . d a i l y .Twenty p a t i e n t s were i n c l u d e d i n a d o u b l e - b l i n d " c r o s s -over" t r i a l . P a t i e n t s a t ra n d o m were g i v e n a p l a c e b o f o rp e r i od s v a r y i ng from t w o t o s i x w e e k s . T he p l a c e b o wasi de nt i c a l i n form t o t h e a c t i v e d r u g , a nd n e i t h e r p a t i e n t n o ro b s e r v i n g p h y s i c i a n was aware o f t h e d r u g r e c e i v e d . Thougha l l p a t i e n t s were aware t h a t t h e y were t a k i n g p a r t i n a t r i a l o ft h e new d r u g none was t o l d t h a t h e m i g h t b e g i v c n 1 ^ n i n a c t - v ep r e p a r a t i o n d u r i n g t h e p e r i o d o f t r e a t m e n t .

o f s i x m o n t h s . T he m i n i m u m d u r a t i o n o f treatment i n t h i sgroup w a s t w o months ( t w o p a t i e n t s ) . E i g h t e e n p a t i e n t s re -c e i v e d L - d o p a f o r more t h a n t h r e e months and f i v e o f t h e s ewere t r e a t e d f o r a t l e a s t s i x m o n t h s , f i v e o t h e r s f o r n i n em o n t h s , and two f o r a l m o s t a y e a r . I n 1 2 p a t i e n t s treatmentwas d i s c o n t i n u e d , f o r reasons a na l y s e d b e l o w , a f t e r p e r i o d sv a r y i n g from t w o t o s i x w e e k s .

In each p a t i e n t an o v e r a l l assessment o f b e n e f i t was e s t i -mat ed. We found t h a t o n l y t h i s o v e r a l l e v a l u a t i o n g i v e s ana d e q u a t e i n d e x o f t h e r a p e u t i c response i n P a r k i n s o n i s m . AsH o e h n a nd Yahr ( 1 9 6 9 ) p o i n t e d o u t , t h e amount o f d i s a b i l i t ya r i s i n g from t h e n e u r o l o g i c a l a b n o r m a l i t y o f P a r k i n s o n i s mo f t e n d e p e n d s on p a t i e n t s ' v a l u e s a n d n e e d s . The r e l a t i v eimport ofsuchfeatures as a defect in gait, deficits in finefinger movements, andspeech impairment varies w i d e l y fromindividualtoindividual.Thuswebelieve that the use of anindexofdisability achievedby summationof the rankings ofvariouscriteriadoesnot adequately reflect either the degree

disability or the change therein. In each case weassessedthec l i n i c a lfeaturesando bj ect i v e criteria listed a b o v e, and

The d o s e w a s t h e r e f o r e r e d u c e d t o4 5 g . d a i l v , a n d o n t h i s d o s a g e s h e was d i s c h a r g e d . A t t h i s t i m es h e w a s a b l e t o r i s e f r o m a c h a i r u n a i d e d a n d t o w a l k f a i r l v e a s i l yw i t h t h e a i d o f o ne s t i c k . A f t e r a m o n t h , h i s s p e e c h s h o w e d s t r i k -i n g i m p r o v e m e n t . I t was o b v i o u s l y l o u d e r , a n d h i s average v o i c ev o l u m e was 6 4 d e c i b e l s , w h i c h represents a h i g h l y s i g n i f i c a n t d e -

gree o f i m p r o v e m e n t . H i s d y s a r t h r i a was g r e a t l y r e d u c e d , a n d t h ep r o f o u n d d i f f i c u l t y i n i n i t i a t i n g s p e e c h v i r t u a l l y d i s a p p e a r e d . H i sh a n d w r i t i n g i n c r e a s e d t o n o r m a l s i z e , a n d h i s d e x t e r i t y t e s t sshowed minor i m p r o v e m e n t s . He c o n t i n u e d on tr eat m ent f o r s i xmonths, though d u r i n g t h i s t i m e h e ha d a p e r i o d o f f o u r w e e k sw h e n h e was t a k i n g t h e i n a c t i v e p r e p a r a t i o n . D u r i n g t h e p e r i o d onp l a c e b o h i s s p e e c h d e t e r i o r a t e d , and t h i s was o b v i o u s b o t h t o hi mand t o t h e o b s e r v i n g c l i n i c i a n , b u t even a f t e r f o u r w e e k s i t d i d no tregress to t h e p r o f o u n d i m p a i r m e n t w h i c h was present b e f o r etre a t m e n t s t a r t e d . H i s s p e e c h r e m a i n e d i m p r o v e d . S i n c e s t a r t i n gon L - d o p a h e h a d s u f f e r e d no f u r t h e r o c u l o g y r i c c r i s e s . H i ss a l i v a t i o n was much l e s s e v i d e n t . He c o n t i n u e d t o work as al a b o u r e r , and was r e l i e v e d o f a p r e v i o u s l y e m b a r r a s s i n g d i f f i c u l t yw h e n t a l k i n g w i t h h i s w o r k m a t e s .

B o t h p a t i e n t s showed i m p r o v e m e n t which q u a l i t a t i v e l y wasvery d i f f e r e n t b u t w h i c h i n e a c h case was m a n i f e s t to am a r k e d d e g r e e . I n C a s e 1 4 t h e s l i g h t i m p r o v e m e n t i n s p e e c hwas i r r e l e v a n t w h e r e i n C a s e 1 8 i m p r o v e m e n t i n s p e e c h a n da b o l i t i o n o f o c u l o g y r i c c r i s e s p r o f o u n d l y a f f e c t e d t h e p a t i e n t ' se f f i c i e n c y , a n d t h e m i n o r g a i n i n motor f u n c t i o n was t o himi n s i g n i f i c a n t . I t was u s u a l l y p o s s i b l e t o m i n i m i z en ausea b y g i v i n g c y c I i z i n e , b y r e d u c i n g t h e i n d i v i d u a l d o s e s o fL - d o p a a n d g i v i n g t h e s e a f t e r f o o d , o r b y r e d u c i n g t h e t o t a ld a i l y d o s e . I n s i x p a t i e n t s n ause a was one f a c t o r l e a d i n g t ot h e stoppage o f t r e a t m e n t , b u t i n o n l y t w o was v o m i t i n g t h ep r i n c i p a l reason f o r d i s c o n t i n u i n g t h e d r u g .

I n d u c e d i n v o l u n t a r y m o v e m e n t s were common. N o p a t i e n td e v e l o p e d t r u e c h o r e i f o r m m o v e m e n t s . The commonest a n du s u a l l y t h e e a r l i e s t d y s k i n s i a s were s t e r e o t y p e d g r i m a c i n gm o v e m e n t s o f t h e m o u t h , c h e w i n g movements o f t h e j a w s , o rr o t a t a r y m o v e m e n t s o f t h e tongue. O f t e n t h e s e oral d y s k i n e s i a sw er e a c c o m p a n i e d b y a l t e r n a t i n g f l e x i o n a n d e x t e n s i o n a t t h ea n k l e s , p r o d u c i n g " p a d d l i n g " f o o t m o v e m e n t s . S i x p a t i e n t s s h o w e d s i g n i f i c a n t p o s t u r a l h y p o t e n s i o n d u r i n gt h e f i r s t f e w w e e k s o f treatment. Two o f t h e s e com plain ed o ff a i n t n e s s a n d d i z z i n e s s . On e o f t h e asymptomatic p a t i e n t sd e r i v e d l i t t l e b e n e f i t a f t e r s i x w e e k s ' t r e a t m e n t on L - d o p a ,w h i c h was s t o p p e d . T w o weeks l a t e r he ha d a myocardiali n f a r c t i o n . The r e l a t i o n b e t w e e n t h e i n f a r c t a n d h i s e a r l i e rp o s t u r a l h y p o t e n s i o n i s u n c e r t a i n , b u t c l e a r l y t h e r e i s a t l e a s ta p o t e n t i a l d a n g e r f r o m p o s t u r a l h y p o t e n s i o n i n t h o s e p a t i e n t swho s u f f e r f r o m c e r e b r o v a s c u l a r o r coronary a r t e r i a l d i s e a s e .

A f t e r a f e w d a y s t h e s e a c u t es y m p t o m s d i s a p p e a r e d . The o t h e r p a t i e n t , a w o m a n, i a db e e n t r e a t e d f o r f o u r m o n t h s b e f o r e s h e d e v e l o p e d f r i g h t e n i n gh a l l u c i n a t i o n s . T h e s e r a p i d l y d i s a p p e a r e d a f t e r a s l i g h t r e -d u c t i o n i n d o s a g e . On e o t h e r p a t i e n t w i t h p o s t e n c e p h a l i t i cP a r k i n s o n i s m was m a r k e d l y i m p r o v e d a f t e r s i x w e e k s ' t r e a t -m e n t ; b u t a t home s h e was e x t r e m e l y a g g r e s s i v e a n d b e g a n t osw e ar c o m p u l s i v e l y a n d o b s c e n e l y . S h e w a s d i s t r e s s e d b y t h i ss y m p t o m , w h i c h r a p i d l y r e g r e s s e d when h e r d o s e o f L - d o p awas s l i g h t l y r e d u c e d . T w o men a n d o n e woman n o t i c e d a ni n c r e a s e i n l i b i d o w h i l e t a k i n g th e d r u g .On e p a t i e n t d e v e l o p e d a d e e p c a l f v e i n t h r o m b o s i s a f t e rt h r e e m o n t h s ' t r e a t m e n t . A n o t h e r s u f f e r e d f r o m f r e q u e n t a n dsevere h e a d a c h e s d u r i n g t h e i n i t i a l p h a s e o f t r e a t m e n t . On ew o m a n, not i n c l u d e d i n t h i s s e r i e s , d e v e l o p e d a n i r r i t a n tw i d e s p r e a d u r t i c a r i a l r a s h a f e w h o u r s a f t e r t a k i n g h e r s e c o n dc a p s u l e o f L - d o p a . S h e was t a k i n g no o t h e r d r u g . L - - D o p awas d i s c o n t i n u e d f o r a w e e k , d u r i n g w h i c h t i m e s h e was g i v e nan a n t i h i s t a m i n e a n d th e r a s h d i s a p p e a r e d . A f t e r a w e e k ,w h i l e s t i l l t a k i n g t h e a n t i h i s t a m i n e , s h e was g i v e n a n o t h e rc a p s u l e o f L - d o p a a n d a g a i n , w i t h i n h o u r s , an u r t i c a r i a le r u p t i o n r e s u l t e d . I t s e e m e d t h a t s h e h a d h a d a h y p e r s e n s i -t i v i t y r e a c t i o n t o L - d o p a , a n d t r e a t m e n t was a b a n d o n e d .D u r i n g th e p e r i o d o f o b s e r v a t i o n no p a t i e n t h a s s h o w n a n ya p p r e c i a b l e c h a n g e i n h i s b l o o d c o u n t , nor h a v e l i v e r f u n c t i o nt e s t s r e v e a l e d a n y a b n o r m a l i t y .

O b s e r v a t i o n s o n D o s a g eA l m o s t a l l p a t i e n t s who h a d b e e n t r e a t e d f o r more t h a nt h r e e m o n t h s a re now t a k i n g a d o s e l o w e r t h a n t h e i r m a x i m a lt o l e r a t e d d o s a g e . The i n i t i a l d o s e was r e d u c e d t o 3 0 0 mg.,a n d i n c r e m e n t s o f 1 5 0 mg. d a i l y were a d d e d a t w e e k l y i n t e r -v a l s . A l l t h r e e p a t i e n t s a re now t a k i n g , w i t h o u t i l l e f f e c t , ad a i l y d o s e o f L - d o p a s u b s t a n t i a l l y greater t h a n t h a t w h i c hp r e v i o u s l y p r o d u c e d d i s t r e s s i n g s i d e - e f f e c t s .
