Included is verification that the required annual CAER Eye exam/s have been obtained. If the CAER is recorded in the OFA database, certificate date can be listed on this update; if not, a copy of the certificate must be submitted. For either case, yearly CAER exam reports must be included with submission. Eye exams must be current before any breeding takes place. Marking "New" indicates this is an addition to my/our breeding program. For all additions, the "Breed Book Information and Documentation" form (on page 5 and in separate attachment) must be completed and submitted with photos and relevant documentation.
