The need for good health continues to propel our business strategies at Dr. Reddy’s. We are in the business of facilitating good health – driven by a companywide imperative to accelerate access to affordable and innovative medicines to people who need them. With this conviction, we continue to invest in product development, acquisitions, and advances in research, processes, and technologies that help us deliver on our promise. We don’t see medicines as just molecules, but as tools that will help people get back to health and promote wellness among them.

As we look ahead, we’re committed to meet the needs of consumers by resolving challenges. Our goal is to make good health possible.

Important NoticeThe Brand-to-Generic-to-Brand Conversion Guide has been produced to provide an easy-to-use reference source of generic names, brand names, and other selected drug information. While the Brand-to-Generic-to-Brand Conversion Guide is an extensive list, it is not a complete list of all the generic and brand-name drugs currently available.

The Brand-to-Generic-to-Brand Conversion Guide should not be relied upon in making generic substitution determinations. It is the healthcare professional’s responsibility to consult and follow all applicable federal and state laws and regulations when prescribing or dispensing pharmaceuticals and, as always, the full prescribing information should be consulted. In reaching a professional judgment, healthcare professionals should thoroughly understand the options available for any clinical application, the potential effectiveness of each product, the associated risks and side effects, and possible interactions. This knowledge should be considered in light of the special circumstances of the patient, for each patient is unique.

Good Health Can't Wait. Access to many of today’s expensive, life-changing medicines remains a hurdle for countless Americans. Helping overcome these barriers to good health and wellness is what drives each and every one of us who work at Dr. Reddy’s North American Generics.

Each day, we strive to do what matters most to patients, determined to find the best way to answer their unmet and under-met needs innovatively and with agility.

Our 5 promises: • We bring expensive medicine within reach • We address unmet patient needs • We help patients manage disease better• We enable partners to ensure our medicines are

available where needed• We work with partners to help them succeed

Uncovering new opportunities for good health has helped us build one of the most expansive offerings in the industry and bring value to the pharmacy and its customers.

Dr. Reddy’s products span across major therapeutic categories including gastrointestinal, cardiovascular, pain management, oncology, anti-infectives, pediatrics, and dermatology. More than 80 prescription products are marketed under the Dr. Reddy’s label in over 350 dosing presentations, supported by an aggressive product pipeline.

One reason that we’re able to make our medicines affordable is because we manage the entire value chain – from producing the active ingredients to developing formulations to distributing them through our streamlined supply chain.

Dr. Reddy’s manufacturing facilities follow cGMP guidelines in the production of API’s and finished dosage forms. Five finished dosage facilities and 8 API facilities currently serve North American Generics. The strategic benefits of vertical integration help ensure that Dr. Reddy’s products are delivered on time.

Measuring creatinine clearance using serum concentration level and timed urine collection provides an estimate of glomerular filtration rate (GFR):

Creatinine Clearance (ClCr) = (urine creatinine x volume) ___________________________

serum creatinine

CREATININE CLEARANCE FORMULA

The estimated creatinine clearance rate using Cockcroft-Gault equation is as follows:

ClCr(est) (mL/min) = [140 – age (yrs)] x weight (kilograms) x 0.85 if female __________________________________ 72 x serum creatinine (mg/dL)

COCKCROFT-GAULT EQUATION

The original MDRD Study Equation – suitable for use with creatinine methods that DO NOT have calibration traceable to isotope dilution mass spectrometry (IDMS):

GFR(est) (mL/min per 1.73 m2) = 186 x [serum creatinine (mg/dL)]-1.154 x [age (yrs)]-0.203 x (0.742 if female) x (1.212 if African American)

The MDRD Study Equation – re-expressed for use with standardized serum creatinine levels:

GFR(est) (mL/min per 1.73 m2) = 175 x [standardized serum creatinine (mg/dL)]-1.154 x [age (yrs)]-0.203 x (0.742 if female) x (1.212 if African American)

Codes For Therapeutic Equivalence Evaluations

Codes

“A” codes are used for drug products considered to be therapeutically equivalent to other pharmaceutically equivalent products.

"B” codes are used for drug products that FDA, at this time, considers not to be therapeutically equivalent to other pharmaceutically equivalent products.

"A" CODES

AA Products in conventional dosage forms not presenting bioequivalence problems

AB Products meeting necessary bioequivalence requirements [AB1, AB2, AB3…]

AN Solutions and powders for aerosolization

AO Injectable oil solutions

AP Injectable aqueous solutions and, in certain instances, intravenous non-aqueous solutions

AT Topical products

"B" CODES

B* Drug products requiring further FDA investigation and review to determine therapeutic equivalence

BC Extended-release dosage forms (capsules, injectables and tablets)

BD Active ingredients and dosage forms with documented bioequivalence problems

BE Delayed-release oral dosage formsBN Products in aerosol-nebulizer drug delivery systems

BP Active ingredients and dosage forms with potential bioequivalence problems

BR Suppositories or enemas that deliver drugs for systemic absorption

BS Products having drug standard deficienciesBT Topical products with bioequivalence issues

BX Drug products for which the data are insufficient to determine therapeutic equivalence

High-alert medications are drugs that bear a heightened risk of causing significant patient harm when they are used in error. Although mistakes may or may not be more common with these drugs, the consequences of an error are clearly more devastating to patients. We hope you will use this list to determine which medications require special safeguards to reduce the risk of errors. This may include strategies such as standardizing the ordering, storage, preparation, and administration of these products; improving access to information about these drugs; limiting access to high-alert medications; using auxiliary labels; employing clinical decision support and automated alerts; and using redundancies such as automated or independent double-checks when necessary. (Note: manual independent double-checks are not always the optimal error-reduction strategy and may not be practical for all of the medications on the list).

Based on error reports submitted to the ISMP National Medication Errors Reporting Program (ISMP MERP), reports of harmful errors in the literature, studies that identify the drugs most often involved in harmful errors, and input from practitioners and safety experts, ISMP created and periodically updates a list of potential high-alert medications. During June and July 2018, practitioners responded to an ISMP survey designed to identify which medications were most frequently considered high-alert. Further, to assure relevance and completeness, the clinical staff at ISMP and members of the ISMP advisory board, were asked to review the potential list. This list of medications and medication categories reflects the collective thinking of all who provided input.
