Thank you for your dedication and time you provide supporting your individual
sports. Your role as a parent representative is very important in helping make
your sport’s season successful, informative and fun. Please take the time to
familiarize yourself with this handbook.
Enclosed you will find a variety of helpful information and forms that can assist
you throughout the season. Hold a parent meeting early in the season where
your roster, schedule, budget and team needs can be discussed and handed out.
You should also appoint a secretary to take notes and a treasurer to take care of
your team’s funds.
Getting other parents involved and aware of what is happening with your sport
can create a positive and fun environment for all your Bonita families.
Boosters encourages both you and your parents to attend the nine general
Booster meetings throughout the year. You will find it important in running the
successful program we all strive to have.
Together we can make a difference for all our students and staff at Bonita High
School.

What is Bonita Boosters
Bonita Boosters is a group of volunteer parents and staff of Bonita High School with the primary
purpose of supporting all of the boys and girls athletic programs and holding the funds for the various
sports and other school programs. Through membership dues and fundraising efforts, Bonita Boosters
provides the extras and sometimes the necessities for our school athletic programs, sports awards,
scholarships and special requests from the coaches. The Bonita Boosters are open to all parents of
Bonita High students. The continued support of parents, faculty and students substantially increases the
strength and effectiveness of Bonita Boosters. Bonita Boosters does the following for the students and
athletes at Bonita High School:
Provides Support and Financial Assistance to our Athletic Training Department
Holds monthly meetings to provide updates on the various athletic programs
Executes financial transactions on behalf of all participating sports
Helps facilitate communication between coaches, parents and the school
Helps implement the BHS Athletic Code (Encouraging positive role models from coaches)
Supports the athletic programs by raising funds, providing awards, and scholarships
Provides annual allocation of funds to help sports purchase needed equipment
Provides parents to sit in on coaches hiring panels
Provides game support (scorekeepers)
Supports Bonita’s Beautification committee

Parent Representative Responsibilities
The parent representative is responsible for:
Parent Representative will be a Bonita Booster Member
Organizing parent support to help the coaches with the administrative and communication
aspects of the program. This maximizes the coaches’ time to coach.
(see, Ways Parents Can Help Coaches section)
Attend Booster meetings to report out on sport as well as gain information from other
representatives, the Booster Board, the Athletic Directors, and BHS administration.
Approving Booster requisition requests for sport.
Facilitate communications with parents and coaches. This communication is critical to have a
successful program.
Develop and update a team roster that includes the following for each athlete: name, address,
home telephone, athlete’s cell phone number, athlete’s email address, parents’ names, and
parents’ cell phone numbers. Email list to Booster Membership Rep at:

Ways Parents Can Help Coaches
Parent involvement helps make the sport fun for the kids and
allows the coaches to concentrate on their sport.
Liaison between coaches, parents and Bonita Boosters
Assist in developing and distributing practice and game schedules
Select and order team uniform and merchandise (sweatshirts, t-shirts, bags, etc.)
Prepare team address and contact lists
Send communications to parents
Plan end of season banquet
Remind coach to order awards (including Bonita Boosters’ Bearcat Award)
Plan team dinners and other team events (e.g., father/son or mother/daughter games, alumni
games, camps)
Organize fundraisers
Run snack bars at games
Handle all team finances
Schedule team, yearbook and CIF pictures
Handle team publicity (Communicate scores and stats to local newspapers, notices in school
daily bulletin, manage team website/develop content for website)
Keep game stats
Organize your own spirit group (banners, parent gatherings)

Parent Rep. Checklist
While every sports program runs differently, these are the typical activities that need to be performed by
either the Parent Representative or other parents.
Pre-Season
Develop and update a team roster that includes the following for each athlete: name, address,

Athletic Training
Our Athletic trainer (AT) is a health care professional who collaborates with physicians to optimize
activity and participation of patients and clients across the age and care continuum. Athletic training
encompasses the prevention, diagnosis and intervention of emergency, acute and chronic medical
conditions involving impairment, functional limitations and disabilities. ATs work under the direction of
physicians, as prescribed by state licensure statutes. The Trainers duties include:
Maintaining accurate records of injuries, treatments and providing insurance claim forms for
sports injuries treated by a physician.
Developing and maintaining a budget for the athletic training program.
Scheduling and being present for pre-participation sports physicals
Determining return to play guidelines
Educating coaches and parents about current injuries and treatment protocols
Providing “Athletic Training Services” to student athletes under the direction of the team
physician or by written referral from a physician, and in accordance with National Athletic
Trainer’s Code of Ethics
Maintaining appropriate general treatment orders to be reviewed annually and approved by the
team physician.
Providing athletic training services for all home athletic contests and away varsity football
games.
Acting as a liaison between family physicians and specialists, the school district, athletes and
their parents.
Student Trainer Aides put in at least 135 hours working with an athletic team. Under the supervision of
the Athletic Trainer their duties include:
Attending daily practices and competitions
Assuring water, equipment, and proper supplies are prepared
Assisting with daily taping and treatments
Acting as a first responder and assist coaches/trainers with basic first aid

Booster Finances
All Booster funds are held in an account at Wells Fargo Bank. Each sport has its own subaccount that is
tracked by the Booster Treasurer. It is the responsibility of the parent rep or the team treasurer to
deposit team funds into the Booster account and to prepare and approve requisition forms.
Deposits:
Each Booster rep or team treasurer should have deposit slips and an account stamp.
All checks must be made out to Bonita Boosters, Bonita High Boosters or BHS Boosters.
Checks made out to anything else will be rejected by the bank. Please include on the memo line
the name of the sport (e.g., football, girl’s tennis, etc.).
Upon completing a deposit, the bank receipt and a completed Booster Deposit Form to the Booster
Treasurer. (see, Attachments for copy of form) The team’s individual subaccount will not be
credited with the deposit until these items are received by the Booster Treasurer. You can mail
your deposit forms to the Booster PO Box, put in the Booster Box at BHS, or deliver to the
Booster Treasurer.
Be sure to keep copies for your records.
You will be notified of any “returned checks” and you team account will be debited for the amount
of the check and any bank charges.
All deposits will be verified and posted to your team account once the Treasurer receives the above
noted items. If you make a deposit and do not supply the Treasurer with a copy, your account
WILL NOT be credited. (Deposits received by the bank without the proper paperwork to the
Booster Treasurer will be posted to the Booster’s General Fund until paperwork is received. Keep
your copies!)
Check Requests:
Make sure to complete the entire “requisition form” before submission. (see, Attachments for
copy of form)
Make sure to attach a copy of all invoices and fully describe the item or service that needs to be
paid.
You are responsible for gathering three signatures on each request: the Parent Rep for the sport,
the Coach’s and the Athletic Director’s. The Booster President will review and sign the form
once it is complete and satisfactory.
Checks cannot be processed without all four signatures
Completed requisition forms (excluding the President’s signature) can be mailed to the Booster
PO Box, put in the Booster Box at BHS or delivered to the Booster Treasurer.

ASB Finances
Besides the Booster account, every team also has an ASB account at BHS. The balance in your team’s
ASB account can be checked by contacting the BHS Finance Office. Unlike, Booster funds,
withdrawing funds from your ASB account requires prior approval by ASB before any purchases
are made! If you have the Finance Office collect funds for your banquet, the money is deposited in
your team’s ASB account. It is important to remember, funds cannot be transferred from your team’s
ASB account to your Booster account.
Prior approval of purchases:
1. Complete an ASB Requisition for Funds form before any purchases are made. (see Attachments
for copy of form) If you do not know the exact amount of the purchase, put a "Not to Exceed"
amount on the requisition.
2. Submit the Requisition for Funds to the BHS Finance Office by Monday at noon for approval on
Tuesday or by Wednesday at noon for approval on Thursday. Approvals are made during the
expenditure meeting held every Tuesday and Thursday. If you are requesting a purchase order,
they are filled out and given to the advisor within 2 days. If this is an emergency order, please
let the Finance Office know so they can process a purchase order the same day.
3. Once you receive a copy of the purchase order or approval, you can place the order or make the
purchases.
Reminders:
ASB will not pay any bills that are submitted if there is no prior approval.
Always make sure that the purchase does not exceed the amount of the purchase order or
request for funds.
Make sure that the approval date for the request for funds is before the actual purchase of
the item or date on the invoice or bill.
Documentation of purchases:
1. Upon completion of a purchase, resubmit the approved Requisition for Funds with the
appropriate documentation to the BHS Finance Office.

Team Fundraising
Prior to conducting a fundraiser, you must fill out a BHS Application for Fund-Raising Activity form
and submit it to the ASB Advisor. If there is a conflict with another club or team, the ASB advisor will
contact you within 3 school days. If necessary, the ASB advisor will help come up with an alternative
fundraiser or date.
1. If your fundraiser is going to be held on campus, prior to the event fill out a calendar request,
facility request and a work order.
2. If you are going to need to be reimbursed from ASB funds for items to conduct the fundraiser, be
sure to submit a request for funds prior to spending any funds. The amount does not need to be
exact; you can estimate (best to estimate high).
3. When the fundraiser is complete, you will need to finish filling out the Fund-Raising Activity
form and return it to ASB Advisor.
Team Pictures
While it is recommended that you use Barr’s Studio for team pictures since they work very closely with
the BHS Yearbook staff, it is not required. It is the Parent Reps responsibility to contact Barr’s
Studio at (626) 332-1293 or other photographer of your choice to schedule a date and time for
your team’s picture. Contact them as early as possible to ensure the best availability of dates. If you
use another photographer, please check with the BHS Yearbook staff to ensure that you provide them
the required pictures.
Have the photographer deliver the order form envelopes to the BHS office or you at least a week in
advanced. Once received it is recommended that a short flyer be attached to the order forms specifying
the date, time and location for the team pictures, as well as the team attire that should be worn. These
should be distributed to the athletes at least several days prior to the pictures. It is also advisable to send
an email to the parents letting them know when pictures will be taken.
Barr usually gives Yearbook a team picture. Also, typically, Barr provides 2- 16x20 team photos and a
stack of small team photos to use as thank you gifts.

Publicity - Newspapers
The Varsity head coach is responsible for seeing that scores for all levels are reported. All scores are to
be recorded in the scores notebook in the Athletic Director’s office and reported to the sport’s league
representative. Varsity scores and stats should also be reported in to local newspapers.

reported to. Please check with your coach.
Publicity – BHS
BHS Daily Bulletin: The BHS Daily Bulletin is read at Bonita and is emailed to parents daily. It is a
wonderful way to get information out to students and parents. Contact the BHS front office for details
for submitting information such as banquet ticket sales information, team picture date, team meetings,
upcoming games or fundraising events, etc.
BHS School Newspaper: Information and pictures can be given to the BHS Newspaper class. The
paper comes out numerous times of the year. Contact the Newspaper class for deadlines and details.
BHS Yearbook: The BHS Yearbook staff is always looking for great action shots of the various sports
as well as pictures of students showing off their team and school spirit. Please contact the BHS
Yearbook staff if you want to submit pictures for consideration in the Yearbook.
BHS Website: If you would like information added to the BHS website, please contact Mr. McGarvey

Team Banquets
Team banquets are a great way to end a season and celebrate the accomplishments of the team. Team
banquets come in many shapes and sizes and there is no one way to do them. Some teams have formal
banquets at local country clubs or golf courses; others have casual events at the school or in the park.
The following is a checklist to help you organize your team’s banquet.
□ Select possible dates (make sure coaches are available) – scheduling of banquets must be
coordinated with the Athletic Director.
□ Check possible dates against the BHS school calendar. This can be done by contacting the Front
Office at BHS and filling out a calendar request form.
□ Contact BHS Finance Office to make arrangements for the sale of tickets.
o Bring the tickets, directions, dates and any other information the students may need to the
Finance Office at least two days prior to the sale of the tickets. Also, notify Finance
Office when ticket sales should stop.
o The Finance Office will provide a list of the students and number of tickets they
purchased the day of the banquet.
□ Submit ASB Requests for Funds if any of the banquet costs will covered from the team’s ASB
account. Remember, request must be submitted prior to signing any facility contracts or
purchasing any items. (see, ASB Finance section)
□ Reserve facility. Possible locations include The Meeting House at Hillcrest, Sierra La Verne
Golf Course, San Dimas Golf Course, Mountain Meadows Golf Course, Church of the Brethren,
Church of the Latter Day Saints, Las Flores Park, BHS Student Union.
□ If banquet is going to be on-campus, complete and submit a facilities request form and work
order (can get from front office).
□ Invite guests (e.g., Athletic Director, school administrators, favorite bus driver)
□ Send out invitation to all players and their parents
□ Hire caterer or organize potluck
□ Organize coaches gifts (e.g., framed team pictures, gift certificates)
□ Organize gifts for parent volunteers (e.g., flowers, gift certificates)
□ Organize gifts for seniors (e.g., framed pictures, Bonita blankets, memory books)
□ Make sure coach picks up certificates/letters from Athletic Director and trophies from Majestic
Awards. (see BHS Athletic Awards section below)
□ Decorations
□ Save receipts to turn in for Booster or ASB reimbursement

Bonita Boosters’ Bearcat Award
Program Policies
Program Purpose:
The Booster Club Bearcat Award Program provides a way to recognize Bonita High School’s student
athletes whose overall conduct in “support” of their team has brought credit to Bonita High School and
its athletic programs. In other words, the Bearcat Award is intended to recognize those student athletes
who could be called “the Most Valuable Support Player.” All student athletes that are selected for
recognition under the program will receive an appropriate commemorative certificate. All varsity-level
student athletes that are selected for recognition under the program will receive a special
commemorative patch. The Bearcat Awards will be presented to the winners at the respective teams
sport banquet by the affected coach.
Award Requirements:
1. Must be a student athlete member of a Bonita High School athletic team (i.e., athletic players
only).
2. Only one student athlete may be selected per sport and per level (i.e., V, JV, Frosh/Soph or
Frosh) each season.
3. Bearcat Awards are coach-selected. Coaches are encouraged to select a Bearcat Award winner
for their respective teams.
4. Student athletes selected for this award must, in the coach’s opinion, have demonstrated
excellence in the following five areas:
Desire,
Enthusiasm,
Courage,
Effort, and
School Spirit.
5. To ensure that the Bearcat Awards are timely given, each coach who wishes to grant a Bearcat
Award, must submit the names of the award winners to the Athletic Directors not less than two
(2) weeks prior to the perspective team’s sports banquet.
6. At the conclusion of each season, the Athletic Directors will report the names of each Bearcat
Award winner to the Boosters Club Board of Directors.
7. The Booster Club shall provide the Bearcat Award certificates and patches.

BHS Athletic Department Awards
It is important to remind the coach to provide updated rosters to the Athletic Director by the date
requested. ASB provides a budget allocation for team awards for each level (Varsity, JV, and
Frosh/Soph). Check with the Athletic Director to find out amount. It is the responsibility of the
coaches to pick the team awards. The Varsity coach must coordinate the ordering of the awards with the
Athletic Director.
Majestic Trophy does most all of BHS’s sports awards, plaques, trophies, etc. The Athletic Director will
email the names of the award winners to Majestic Trophy for the engraving, etc. It is the responsibility
of the coach to get the Athletic Director the awards roster in a timely manner before the team’s sports
banquet. The coach or a parent representative is responsible for picking up the trophies.
Majestic Trophy is located at 215 N Laurel Ave. Ontario (just west of Euclid between B and C streets).

If the coaches want to have additional awards, they may use their Booster funds for the awards. These
additional awards need to be ordered directly through Majestic Trophy and paid for before they can be
picked up.

Boosters Athletic Scholarship
Program Policies
Program Purpose:
The purpose of this program is to recognize the most outstanding senior varsity-level student athletes
whose performance and scholastic accomplishments have brought credit to Bonita High School and its
athletic programs. The awarding of these Booster scholarships is also designed to enhance parental
participation in the Booster program, so additional consideration will be granted to scholarship
candidates whose parents have been active Booster members, although this will not be a prerequisite for
nomination or selection. Those student athletes that are selected for recognition under the program will
receive a scholarship grant.
Minimum Eligibility Requirements:
1. Must be a graduating diploma earning senior.
2. Must have earned at least one (1) varsity athletic letter during their senior year.
3. Must have earned at least three (3) varsity athletic letters during their high school career
(acceptance of letters for transfer students will be determined by the Athletic Director).
4. Must have completed participation in four (4) athletic seasons at any level among any
combination of sports during their high school career (acceptance of participation for transfer
students will be determined by the Athletic Director).
5. Must have a minimum cumulative grade point average (GPA) of 3.5 (on a 5 point scale), as
determined at the end of first semester of their senior year.
6. Must be nominated and performance rated by the coach from which they earned a varsity letter
in their senior year (only one coach per sport) in the following five areas:
Sportsmanship (i.e., citizenship during competition);
Conduct (i.e., non-competition citizenship);
Attitude;
Leadership; and
Playing ability.
(Each performance category will be rated on a five-point scale [maximum number of points is
25])
7. The Athletic Director must concur in the coach’s rating of the student athlete. Further, in the
event the student athlete’s coach is unavailable to file a nomination, then the Athletic Director
may submit the nomination on behalf of the coach.

Nomination and Selection Procedure:
1. The submitting coach/coaches should submit their nomination form to the Athletic Director as
soon as possible after the conclusion of their student athlete’s senior year season, BUT NO
LATER THAN THE FIRST THURSDAY IN APRIL. Spring season coaches must submit
their rating sheets by the first Thursday in April.
2. As soon as possible after the submission deadline, the Athletic Director will file all nominations
received with the Boosters Club Board of Directors.
3. The Boosters Club Board of Directors will calculate each nominated student’s total score
pursuant to the formula described herein.
4. The scholastic score will be determined by multiplying the cumulative GPA (through the first
semester of their senior year) by 10.
5. The performance score will be determined by averaging the coaches’ scores (if only one coach
provides a performance rating, then use the single rating score) and multiply the result by 2.
6. The total score will be determined by adding the scholastic and performance scores together (100
points are possible).
7. The highest total scores will determine the winners. If there is a tie, the candidate with the
higher scholastic score will determine the winner. If a tie remains, then the student athlete with
the higher class ranking will be the winner.
Number and Amount of Scholarships:
The Boosters Club’s Board of Directors will annually determine the number and dollar amount of the
scholarship grants, if any. The total shall always be an even number and the number of scholarship
grants shall always be equally divided among boys and girls. In the event that there is not an even
number of qualified nominee winners, the number of decided scholarships will go to the boys or girls
qualified. The scholarship grants shall be paid out during the year-end senior awards ceremony.

Annual Booster Allocation Process
At the end of each school year, the Boosters Board allocates available funds to cover requests submitted
by each sport. Typically requests include additional sports equipment (e.g., balls, pitching machines,
and goals), training fees, or facility equipment (e.g., wind screens). It is up to the coaches and the parent
representatives to complete a Boosters Allocation form and submit it on a timely basis (typically the
beginning of April). The amount available each year for allocation to the sports is subject to change and
will be approved by the Booster Board. The Booster Board will review all of the submitted Allocation
Requests and allocate the available funds based upon the following factors: attendance at Booster
Meetings and Booster fundraisers (e.g., football game snack bar) by coaches and parents associated with
the particular sport, financial need of the sport, overall level of requests submitted, and special
considerations raised by the Athletic Directors. The allocations will be announced at the last Boosters
meeting of the school year. Once approved, the funds are available for the sport to use for the specific
items approved. Funds allocated through this process can only be used for the approved purchases.
When submitting the Boosters Requisition form for items approved under the annual allocation process,
please note on the Requisition form that the items purchased are covered under the approved allocation.

BHS Activities Information Evening
Each school year, BHS hosts an Activities Information Evening for all incoming freshman. During the
Activities Information Evening, BHS administrators and Athletic Directors review the sports program
and discuss important information. Prior to the meeting and after the meeting, all sports can host tables

several members of the team present and at least their Parent Representative present to answer questions.

At the end of the event it is important to ensure that the Athletic Directors get copies of any signup lists.
BHS Mandatory Parent Nights & Team Meetings
Bonita High School has mandatory meetings for the parents of all athletes participating in each season's
sports programs. During the first part of the meeting the BHS Administration and Athletic Directors
discuss a variety of topics as highlighted including
1. Communications you should expect from your child’s coach
2. Communications coaches expect from parents
3. Appropriate concerns to discuss with the coaches
4. Issues not appropriate to discuss with the coach
5. Concern/complaint procedures
6. Physicals
7. Injury forms
Immediately following this meeting each sport currently in season will have a separate team meeting
with their coaches
1. Philosophy of the coach
2. Expectation the coach has for your child
3. Location and times of all practices and games
4. Team requirements, i.e. practices, special equipment, out of season training
5. Procedures to follow should your child be injured during participation
6. Discipline that may result in the denial of your child’s participation
7. Sport specific information

Athletic Packets/Clearance
Student-Athletes and their parent/guardian MUST register for EACH AND EVERY sport they plan on participating in separately. It’s better to register and change your mind not to participate than to miss a sports registration deadline date and not be able to participate. Please follow the steps below to register for a sport

Open each Legal Form one at a time, after the parent/guardian and student have both read
each Legal Form, be sure to click “PARENT/GUARDIAN” and click “PARTICIPANT”, then click

Step 7: Registration Summary- Add another Registration, Cancel or click “SUBMIT”. You will receive
an email confirming your registration.
REMINDER: After completion of the online registration bring in the hard copy of the Physical Examination
(signed and dated by the physician) and Health History Form, to the BHS Athletic Office. You will not be
cleared to practice until the hard copy of the physical form has been turned in and entered into the program
by the Athletic Office staff. Prior to turning in Physical and Health History Forms, make sure all signatures
required are completed.
Physicals are valid for 1 year.

FAQ
1. Can anyone attend Booster meetings?
Booster meetings are open to anyone who wants to attend. Booster meetings are a great way to
find out what is happening with Bonita Sports, receive first hand information from the Principal
and Staff, meet people, and attendance gets your sport recognition for funds. You must be a
Booster Member to Vote during meetings.
2. How long do Bonita Boosters’ meeting typically last?
The meetings average about an hour.
3. Are checks made out to Bonita Boosters tax deductible?
Donations to Bonita Boosters may be tax deductible, check with your tax preparer or accountant.
4. Does all of the money raised at the snack bar during football games go to football?
No, the money raised at the football game snack bar goes into the Boosters General Fund.
5. How much money does Boosters typically allocate to the various sports?
The amount of money allocated during the annual Booster allocation process varies from year-
to-year. Typically the amount is between $2,000 and $5,000.
6. How does a team get money allocated to it from Boosters?
In order to be considered for an allocation, the sport must submit an Allocation Request to
Boosters by the due date. (See, Booster Annual Allocation Process section above)
7. How does Boosters raise money?
Boosters raise money through its membership drive, the snack bar at football games, e-scrip,
house flag sales, personalized banner sales and other fundraisers or by direct donations.
8. What is e-scrip and how do I sign up?
There is no cost to sign up for the program. E-scrip is a program where participating merchants
make contributions to charitable organizations, such as Bonita Boosters, based on purchases you
make using your registered credit cards or debit cards. Cards that participate include Master
Card, Visa, American Express, Discover, and others. You can sign up by going to
www.escrip.com, click signup, and follow the simple instructions. The Bonita Boosters group
# is 147108959. This is a great way to raise money to support sports at BHS without costing
anything extra.

Allocations Program

The purpose of the Boosters Club Allocations Program is to financially assist athletic
teams at BHS with respect to the procurement of equipment, supplies, commodities,
facilities, training or services that cannot otherwise be obtained without Boosters Club
assistance. Please complete this form and submit it to your assigned Athletic Director by
April 14, 2018. If your coach is unavailable to submit this form, the Athletic Director
may submit on their behalf. The Boosters Club Board of Directors will determine the
