    About OMSAS

    The Ontario Medical School Application Service

    (OMSAS) is a not-for-profit, centralized applicationservice for applicants to the six Ontario medicaschools. OMSAS was developed by admissions

    personnel at these schools and is operated by theOntario Universities Application Centre (OUAC)

    which is a division of the Council of Ontario

    Universities. Its purpose is to facilitate the process oapplying to Ontario medical schools and to reduceduplication in application processing to save timeand resources for the applicants and the institutions

    Each medical school is completely autonomous inreaching its own admission decisions. OMSAS only

    provides the application processing service.

    The OMSAS online application is available atwww.ouac.on.ca/omsas/.

    You must submit only one set of applicationmaterials and academic documents to Ontario

    medical schools, regardless of the number ofschools you apply to. OMSAS does not accept

    collect (COD) courier deliveries of any documents

    Complete an online application to apply to Ontariomedical schools. There are no paper applications.

    Visit the OUAC website for information about themedical school application process and to view

    details about your application.

    Each of the six Ontario medical schools has its

    own admission requirements, and reserves theright to determine whether degrees granted bypostsecondary institutions are suitable for the

    purpose of admission. Please be aware of thesevariations and be sure you qualify for considerationbefore submitting your applications to OMSAS

    OMSAS will process and forward applicationsto all requested medical schools, regardless of

    your qualifications or the completeness of yourapplication. There are no refunds.

    You are advised to contact the medical schoodirectly for additional information about the

    academic program and admissions. Contact detailscan be found in the university information section

    of this booklet.

    Please be aware that the application process islengthy. The amount of time required depends on

    residency positions in Ontario after graduating fromsuch schools could be severely limited.

    Registration/Licensure

    Regulations about licensure in Ontario shouldbe obtained from the College of Physicians andSurgeons of Ontario.

    Note: If you can answer yes to either of the

    schools you are applying to, or consult with theCollege of Physicians and Surgeons at 416-967-2600

    1. Have you ever been convicted of an offence(not including traffic violations) for which a

    pardon has not been granted?2. Are there any criminal charges pending agains

    you?

    Medical school graduates with criminal records maynot be eligible to receive registration (licence) to

    practice medicine.

    Graduates from a medical school outside of Canadaand/or the United States should contact the Centrefor the Evaluation of Health Professionals Educated

    The OUAC and Privacy

    The OUAC takes extensive measures to ensure thesecurity of its website.

    The OUAC encrypts application data using Secure

    Sockets Layer (SSL) technology, the industrystandard for protecting web communications.

    EmailMessages that you send to the OUAC by email may

    The OUAC was founded in 1971 by the Committee of

    The OUAC is committed to protecting your privacyin relation to the personal information you providein support of your application. If, after providingyour application, you do not register in an Ontariomedical school, the OUAC will not retain any originadocumentation or paper records in respect toyour application. Transcripts, autobiographical andpersonal submissions, and supplementary material insupport of applications filed in a specific academic

    year will not be returned, forwarded or copied toapplicants or forwarded to third parties prior tobeing destroyed at the end of each application cycle

    The OUAC retains the current (and historical)electronic version(s) of your demographic, academicand choice data provided in your application in

    accordance with the OUACs records retentionpolicy and maintains administrative, technical and

    physical safeguards in an effort to protect againstunauthorized access, use, modification and disclosure

    of your personal information. The OUAC will maintainthe confidentiality of all personal information it

    collects in connection with the application and wildisclose such personal information only for thepurposes described in this Declaration and Notice

    The OUAC stores electronic records off-site as partof its disaster recovery procedures. You are solely

    responsible for keeping your OUAC application useridentification, login information and other registration

    information confidential and secure. Please notify theOUAC immediately if you suspect any unauthorizedaccess, use or disclosure.

    Collection of Personal Information

    The OUAC will collect the personal information

    you provide in your application, or in referenceto your application, to process your applicationto the university(ies) of your choice.

    The OUAC may collect information about yourstatus as a Canadian Aboriginal person. You arenot required to provide this information; howeverif you provide it, the OUAC will forward it tothe university(ies) of your choice to allow theuniversity(ies) to inform you of specific servicesavailable to Canadian Aboriginal students, inan effort to improve the availability of medicaeducation and care throughout Aborigina

    communities. The OUAC will collect transcripts from educationa

    institutions, as provided by you.

    The OUAC will collect your Medical CollegeAdmission Test (MCAT) scores directly from

    the Association of American Medical Colleges(AAMC) with the consent you provide when yousubmit this application.

    The OUAC will collect personal information fromindividuals you provide as references.

    The OUAC will collect payment informationfrom you (or from whoever is paying for this

    Declaration and Notice of Collection, Use,Disclosure and Treatment of Your Personal

    Information Provided as Part of Your OntarioMedical School Application Service (OMSAS)

    ApplicationThe Ontario Universities Application Centre

    (OUAC) was created by the universities of Ontariothrough the Council of Ontario Universities (COU).The OUAC has been processing applications on

    behalf of universities in Ontario since 1971. To applyfor admission to a medical school at a university

    in Ontario, your application must be processedthrough the OUAC. The OUAC forwards your

    application information to the university(ies) of yourchoice. By applying through the OUAC, you agreethat the university(ies) of your choice will obtain

    the personal information you have provided to theOUAC and the OUAC will collect, use, disclose and

    otherwise manage your personal information as setout in this Declaration and Notice.

    The personal information requested in this

    application is required by the OUAC and by themedical schools for the purpose of your application

    and must be provided together with your applicationfee. Incomplete applications will not be considered.

    Medical schools may require additional personalinformation from you to complete your application.Medical schools may use and disclose your personal

    information for other purposes in accordance withtheir own admission and personal information

    policies and practices, including requirements forgovernment enrollment reporting, which you must

    investigate yourself. For example, medical schools willrequire you to provide them with information about

    whether and to what extent you have a prior criminalrecord. Universities also disclose personal informationto regulatory authorities, law enforcement or other

    persons, when authorized or required to do so by law.

    It is your responsibility to ensure that your applicationinformation and all supporting documentation

    is truthful, complete and correct, and that yourautobiographic and personal submissions areauthored solely and entirely by you. The OUAC

    and the universities reserve the right to verify anyinformation provided as part of this application. If

    any information in your application is determinedto be false or misleading, concealed or withheld, or

    written by a third party, at the absolute discretion ofthe OUAC and/or a university, your application maybe invalidated. This could result in its immediate

    rejection or in the revocation of an offer of admissionor registration at a university. Any such information

    may be shared by the OUAC or by universities andcolleges with the Association of Faculties of Medicine

    of Canada (AFMC), and with other universities andcolleges across Canada.

    research purposes by the Ministry of Educationthe MTCU, and postsecondary institutions, as

    allowed within the MTCU Act. The OUAC maydisclose your OEN to your universities of choice

    in order to maintain this registry.

    Based on the MTCU Act, the OUAC may disclose

    personal information to the MTCU for policydevelopment and research purposes.

    The medical schools will disclose your application

    information to the AFMC, for research in medicaeducation.

    If any information connected with your applicationis determined to be false or misleading, concealed

    or withheld, or contains evidence of academicdishonesty, or inappropriate or unethical conduct

    the AFMC, and universities and colleges acrossCanada will be advised, at the absolute discretion

    of the OUAC and/or a university. Fraudulentuse of credit cards or demanding/obtainingunauthorized application fee reimbursement is

    considered unethical.

    The OUAC will disclose personal information

    from all medical school applications inaggregate, non-personally identifiable formto the public via the OUAC website, to Ontario

    government ministries (MTCU, Ministry of Healthand Long-Term Care); MAC-COFM; and academic

    researchers (at the discretion of the OUAC andthe medical schools) for admissions, enrollmentand other academic policy development and

    research purposes.

    If you have accepted an offer for admission to

    a medical school, the OUAC will disclose yourname, gender, and date of birth to other medica

    schools across Canada to which you have

    applied for enrollment management purposes. If you have accepted an offer for admission to

    an Ontario medical school, the OUAC and/or themedical school will disclose your name, gender

    date of birth, language of correspondenceOUAC/OMSAS Reference Number, and the

    name of the medical school whose offer youhave accepted to the Medical Council ofCanada (MCC) for validation purposes when

    assigning a Medical Identification Number forCanada (MINC).

    The OUAC may disclose your personal information

    to regulatory authorities, law enforcement orother persons, as authorized or required by law.

    If you have ordered electronic transcripts in

    support of your application, the OUAC maydisclose your personal information to theinstitution(s) from which you have ordered your

    transcripts to process your transcript request.

    For more information about the collection, use

    disclosure, and treatment of your personal information

    at the OUAC, review the OUACs Privacy Code at

    www.ouac.on.ca/privacy/ or contact the OUAC Privacy

    All credit card processing is done by a third partyand no cardholder data is collected, transmitted

    or stored on OUAC systems.

    Use of Personal Information

    The OUAC will compile and process yourapplication and payment.

    The OUAC will use personal information from all

    medical school applications to create aggregate,non-personally identifiable information for use

    by the public, via the OUAC website, by Ontariogovernment ministries (Ministry of Training,

    Colleges and Universities [MTCU], Ministryof Health and Long-Term Care); the MedicalAdmissions Committee (a sub-committee of

    the Council of Ontario Faculties of Medicine[MAC-COFM]); and academic researchers (at the

    discretion of the OUAC and the medical schools)for admissions, enrollment and other academic

    policy development and research purposes.

    The OUAC and/or the university(ies) will use

    your email address for communication purposes. If you have ordered electronic transcripts in

    support of your application, the OUAC may

    use your personal information to process yourtranscript request.

    The OUAC and/or the university(ies) willuse your date of birth for identification and

    document matching.

    Disclosure of Personal Information

    The OUAC will disclose your demographic,academic and university/program choice data to

    all universities for which you have submitted an

    OMSAS application. In the event that you have applied to a joint or

    collaborative university/university or university/college program, the OUAC and/or the universities

    will disclose your application information tothe relevant partner Ontario college(s) oruniversity(ies).

    The OUAC will send the individuals you list asreferences an acknowledgement letter advising

    them that they have been named as a reference. Ifyour references contact the OUAC with questions

    regarding your application, the OUAC will disclose

    personal information about you, as necessary andappropriate, to respond to the inquiry.

    If a third party who is paying for this application

    has questions regarding the payment for yourapplication, the OUAC will disclose personal

    information about you, as necessary andappropriate, to respond to the inquiry.

    The OUAC and/or the university(ies) may use

    your personal information to validate or assignan Ontario Education Number (OEN). The

    OEN Registry is maintained by the Ministryof Education, and is used for tracking and

    Submitting the Application

    If you successfully submit your application, you

    will receive an OUAC/OMSAS Reference Number(2016-80xxxx) after you complete your payment

    You will also receive an acknowledgement emaifrom OMSAS. If you successfully submit your

    application but do not receive an acknowledgementemail, contact OMSAS immediately.

    Verification of Application DataOnce your payment is processed, you can inquire

    online to determine what supporting documentsOMSAS has received.

    OMSAS will perform an item-by-item review to

    compare all courses on the academic record againstyour official university transcripts. Beginning inNovember, and after this review, you will have access

    to an online report that indicates the data OMSAShas on file. The report will identify any missing

    transcripts or Confidential Assessment formsAccess the report by logging in to your submitted

    application with your user ID and password, andselecting the appropriate link.

    Please be aware that OMSAS must follow certainconventions requested by the medical schools in

    the presentation of the data. This report representsonly one part of the academic information used in

    the adjudication process. In addition, each schooreviews all of the admission material you submit.

    Each medical school reserves the right to determinewhether degrees granted are suitable for admission

    to their medical program.

    If you have any questions about the verification of

    the academic records, contact OMSAS immediately

    using the SAM tool.You are responsible for reportingany required changes to your academic record by

    December 1, 2015. Note: Schools may use their own

    admission formula, which may or may not use the

    GPA values OMSAS calculates.

    Application for Advanced Standing and Transfer

    Applicants for advanced standing and transfer mustcontact the individual medical schools directly.

    Scholarships

    Some medical schools offer scholarships to certainapplicants from specified groups. For informationabout scholarship availability and criteria, consult

    the academic calendar of the particular universityor medical school.

    Applicants Declaration

    You are required to consent to the personal

    information practices as set out in the Declarationand Notice of Collection, Use, Disclosure andTreatment of Your Personal Information Provided

    as Part of Your Ontario Medical School ApplicationService (OMSAS) Application, and certify the

    following statement:

    I certify that the personal information and documentssubmitted in this application, or to be submitted

    (all of which constitutes the application), are true,complete and correct in all respects, including my

    declarations as to citizenship and immigration statusin Canada; that my autobiographic and personal

    submissions are true, accurate, and authored solelyand entirely by me; and that all information requestedin this application has been disclosed. I understand

    that it is my responsibility to keep OMSAS and themedical school(s), to which I have applied, or at which

    I register, informed of any changes to the informationin my application materials, and I agree to do so in

    writing immediately after any such change occurs.

    Applicant Responsibilities

    You are expected to become familiar with and

    observe the application procedures for eachmedical school/program you apply to. Read the

    entire instruction booklet and the program-specificinformation from each medical school beforecompleting the application.

    It is your responsibility to ensure that OMSAS

    receives the application and all required supportingdocumentation by the required deadlines. Normally,

    unsolicited application materials and informationwill not be considered for admission purposes.

    Faxed documentation will not be accepted.

    Failure to comply with the admission requirements

    and deadlines may result in the cancellation of yourapplication. Application fees are non-refundable.

    The application process may be delayed until all

    required transcripts and documentation are receivedas specified by the medical schools you apply to.

    Supporting documentation received after the

    deadline will be forwarded to the applicable medicalschools; however, OMSAS cannot guarantee thatthe medical schools will consider these documents.

    Offers of AdmissionFirst-round offers of admission are sent out on

    May 10, 2016. You may hold only one acceptance ofan offer of admission to an Ontario medical schoo

    at any time. Once you accept an offer of admissionyou will be removed from the wait lists of the other

    Ontario medical schools. Responses to offers ofadmission must be submitted using SAM.

    Personal Information

    Title, Gender and Date of Birth

    Title and gender are optional fields. Date of birth

    is used for identification and document matchingpurposes. Gender and date of birth are collected

    for statistical purposes only and do not form part ofthe universitys admission decision.

    Language of ApplicationApplications for the University of Ottawa can be

    made in English or French. For all other universitiesthe application must be completed in English

    You must be proficient in both oral and writtencommunication for your language of application.

    Official Legal Name

    Please ensure that the name you use to apply isthe complete name by which you are legally andcorrectly known. Record any former surname(s)

    so OMSAS and the universities can match yourdocuments to your application file.

    Citizenship

    Citizenship will be verified by all medical schools.

    Note: Normally, only the medical schools atMcMaster University and the University of Torontoconsider applications from qualified non-Canadian

    applicants. The University of Ottawa will considerapplications from the children of alumni who do not

    reside in Canada.

    Permanent Residents (Landed Immigrants)All Ontario medical schools consider applications

    from qualified Canadian citizens and permanent

    residents. Canadian permanent residents, for thepurpose of admission to an Ontario medical school

    are those who, by the application deadline, possessa Canadian Immigration Record of Landing or a

    Permanent Resident Card issued by Citizenshipand Immigration Canada. Permanent residents

    must submit a photocopy of the front and back oftheir Permanent Resident Card to OMSAS by theapplication deadline. The date on the back must be

    legible.

    Police Record CheckOnce you are accepted, some schools require you

    to undergo police record checks. You will not beregistered until this check is complete. The medical

    schools reserve the right to withdraw an offer ofadmission based on the results of the police record

    check. For specific details, refer to each individualschools requirements.

    If any criminal charges are laid against you after thepolice record check is completed, you are required

    to disclose this information to the medical schoolsthat have offered you admission. The medical

    schools reserve the right to revoke admissionoffers and/or registration at any time, as a resultof reviewing information pertaining to criminal

    charges.

    CPRYou must complete a CPR Basic Rescuer Course

    (Level C) and be able to produce valid certificatesbefore enrollment in medical school.

    For McMaster and the Northern Ontario School of

    Medicine (NOSM), incoming students must provideevidence of a valid certificate in Basic Life Supportfor Health Care Providers.

    OMSAS Secure Applicant Messaging (SAM) Tool

    The OMSAS SAM tool allows you to send personalinformation to OMSAS via a secure process, to

    ensure this information is protected. You can accessSAM by logging in to your submitted applicationusing your user ID and password, and selecting the

    appropriate link.

    The following change requests mustbe made usingSAM:

    Once you submit your application, you can access

    Verifiers

    The verifier list should not contain the descriptionof the activity. It should contain the verifiers titlefirst and last name, address, telephone number and

    any additional comments. You may use the sameverifier for multiple activities.

    To associate a verifier with a given activity, enter a

    number in the Verifier field. This number should

    correspond to the number on the verifiers list.

    Any activities that can be substantiated byacademic transcripts do not require verifiers (e.g.

    scholarships, special commendations). For theseitems, enter a zero in the Verifier field to save your

    information.

    The medical schools reserve the right to confirm theinformation provided in these sections by consulting

    the individuals you listed as verifiers.

    McMaster applicants are not required to provideverifiers for the Autobiographical Sketch.

    Referees

    Confidential Assessment FormsOMSAS is prepared to receive Confidentia

    Assessment (Referee) forms before receiving yourapplication, but not before August 1, 2015. The

    Confidential Assessment forms must be forwardedto the referees, who should, in turn, forward the

    completed forms directly to OMSAS. Refereesmust attach a separate letter, printed on their usuastationery, elaborating on the information provided

    on the assessment form. The medical schools wilnot be able to give you full consideration without

    this additional information.

    Assessments must be completed in confidence by

    the referee and should not be made available toyou.

    You should select referees who have extensive

    personal knowledge of you and are in a positionto make statements concerning your character

    personal qualities, academic capabilities and speciacircumstances, if applicable. Referees may not be

    in a position to evaluate all of the characteristicsrequested; however, in choosing your three refereesensure that all subject areas are covered. At least

    For instance, volunteer work is often perceived asonly those activities that are coordinated by an

    organization. However, there are many forms ofvolunteer work. For example, if you were raised in a

    farming community and helped to run a neighboursfarm (e.g., when the neighbour was sick), this wouldbe considered volunteer activity. However, ensure

    you have a contact for each of the activities listed.

    To ensure that full information is provided in the

    Autobiographical Sketch, OMSAS recommends you:

    1. consider and record (separate from the

    application) allactivities since age 16; and then2. complete the application by arranging these

    activities into the categories listed above.

    Please provide the information in point form.Do not forward supplementary pages, letters of

    recommendation, curriculum vitae, etc.

    You will need to provide more specific details for theEmployment, Volunteer Activities, Extracurricular

    Activities, Awards and Accomplishments, andResearch categories. These details are not requiredfor McMaster or Western. If you are applying to

    Ottawa, you will be required to identify the topthree (3) activities that are most important to your

    application in medicine, in each category, and thathave been completed during your postsecondary

    (undergraduate university) studies.

    OMSAS collects this information on behalf ofthe medical schools. OMSAS will arrange the

    September 2015, you may not have received yourAAMC ID number at the time of application. If this

    is the case, use SAM to provide OMSAS with thisnumber as soon as you receive it.

    Contact the AAMC each year you apply, but no

    earlier than July 15, 2015, to request that all of your

    MCAT scores be released to OMSAS. The Ontariomedical schools require full disclosure of all MCAT

    score results.

    If you wrote the July, August or September 2015MCAT, you must wait until your scores are made

    available before requesting their release to OMSASAll scores must be released to OMSAS before

    November 1, 2015. The medical schools reserve theright to disqualify you if they do not receive your

    MCAT scores by November 1, 2015.

    Note:The medical schools are aware that you may

    not have received the results of the Septemberwritings of the MCAT prior to the October 1, 2015

    OMSAS application deadline. Fees will not berefunded for choices dropped because the MCAT

    results did not meet eligibility requirements set bythe medical schools.

    Once your application is submitted, your AAMC IDnumber must be submitted using SAM.

    Transcripts

    Official transcripts are required for each university

    college, CEGEP, junior college, graduate school orother postsecondary institution at which you have

    registered. This includes transcripts for work takenon Letter of Permission, for transfer credit or onan exchange program. OMSAS does not require

    International Baccalaureate (IB) or AdvancedPlacement (AP) transcripts, unless the courses are

    prerequisites. To be official, transcripts must be sentdirectly from the Registrars Office to OMSAS and be

    printed on the institutions official transcript paperwith the school seal and/or bear the appropriatesignatures. Photocopies are not acceptable. OMSAS

    cannot accept undergraduate transcripts providedby graduate departments or those you send directly

    The submission of a University Premedical Advisory

    Report (provided by some schools in the UnitedStates) will be accepted but does not replace the

    use of the three Confidential Assessment forms.

    You must print Confidential Assessment forms fromwithin the online application before submittingthe application and receiving an OUAC/OMSAS

    Reference Number. The pre-printed number onthese forms will be your user ID (IT#) rather than

    your OUAC/OMSAS Reference Number. OMSAS willuse the user ID to link the assessment forms to your

    application.

    You cannot make changes to your referees onceyour forms are downloaded. Altered forms will notbe processed.

    Note: The Confidential Assessment forms should

    be received by October 1, 2015. Failure to meet thisdeadline means the application will initially be sent

    to the medical schools without these forms and willdelay progress of the first evaluation. Confidential

    Assessment forms from the three referees indicatedin your application will be sent to the medicalschools, regardless of when they are received.

    However, the medical schools reserve the rightto disqualify applications due to late references.

    Medical College Admission Test(MCAT)

    Final Transcripts

    At the end of the academic year, if you are stil

    being considered for admission and are currently

    attending university, you must arrange with yourRegistrars Office to forward a copy of your fina

    transcript directly to OMSAS. Please obtain a copy

    of the final transcript for your records and verify its

    accuracy. Applicants to Western University who fai

    to forward a transcript to OMSAS will be disqualified

    A transcript of the current years December results is

    not required unless it is your final term.

    Note:If you attend an Ontario university, you mustuse the TRF to order transcripts. You can access this

    OMSAS is prepared to receive transcripts beforereceiving your application, but not before

    Transfer credits, Letter of Permission credits and/or exchange program credits recorded on the

    transcript of another university cannot be acceptedin place of the transcript from the university wherethe course was taken.

    If the university/college you attend/attended does

    not issue transcripts, you must arrange with theRegistrars Office to provide OMSAS with an official

    statement of attendance and/or degree granted, bythe October 1 deadline.

    It is your responsibility to ensure that OMSASreceives all transcripts and/or attendance

    Transcripts from an Ontario University or College

    If you attended, or are currently enrolled at,

    an Ontario university, you must use the OUACTranscript Request Form (TRF) in the online

    application. If you attended, or are currently enrolledat, an Ontario college, you may use the TRF. Bycompleting the TRF and forwarding the appropriate

    fees, you authorize the OUAC to arrange for yourofficial transcripts to be sent to OMSAS; you do not

    need to contact the Registrars Office directly.

    Exceptions:

    1. You cannot use the TRF to order transcripts

    from the Royal Military College of Canada.

    Undergraduate Academic Record

    The medical schools receive applications fromapplicants who studied at universities with various

    grading systems. The admission committees at themedical schools attempt to give each applican

    equal consideration. The Postsecondary EducationUndergraduate Academic Record and the Grading

    System Conversion Table display applicants academicrecords in a uniform manner and equate the variousgrading systems.

    The following courses are not normally included in

    the OMSAS GPA:

    naturopathic and chiropractic medicine

    consecutive Bachelor of Education programs

    college courses (even if transfer credit is granted

    challenge for credit courses diploma or certificate programs

    graduate courses

    undergraduate courses taken as part of agraduate program

    JD or LLB programs

    MD programs

    The medical schools equate university work on

    the basis of the Ontario traditional academicyear system. If you are applying from universitiesoperating in the semester system, list your courses

    in multiples of two semesters (i.e., semesters oneand two are combined for first year; semesters

    three and four for second year). If you are applyingfrom the American schools quarter system, list your

    courses in multiples of three quarters.

    Each medical school may use a different GPA than is

    calculated in the Undergraduate Academic Recordbased on their own admission criteria. Each medica

    school reserves the right to determine whetherdegrees granted are suitable for admission to their

    medical program. Refer to the medical schoorequirements section of this document for more

    information.

    The Undergraduate Academic Record will include

    all information relating to undergraduate studiesyou undertake. This record must display all relevant

    data in academic and calendar year sequence.

    To record the required information, list your firstacademic year of study at each university, followed

    by your second year of study, etc. OMSAS wilcalculate a cumulative GPA.

    form by logging in to your submitted applicationusing your user ID and password, and then selecting

    the appropriate link.

    The deadline for receipt of final transcripts isJune 30, 2016. Receipt of the TRF by this date doesnot satisfy the deadline requirement.

    If you have completed the work for your degree,

    ask the university to indicate on the transcript thatyou are eligible for the degree or that it has been

    conferred.

    Assessment of International Academic Credentials

    Whether you are a Canadian or non-Canadian

    applicant, if you have not met minimum coursenumber criteria utilizing your Canadian or US dataand require inclusion of your international education

    data, you must have your foreign transcriptassessed by World Education Services (WES). WES

    assessment greatly assists in the consideration ofthe application. Credentialing assessment means

    converting foreign academic credentials intotheir Canadian educational equivalents. The WESassessment should be released to OMSAS directlyby WES. If the WES assessment includes a copyof the official transcript, you are not required torequest this transcript from your registrar. Pleaseconsult the individual school sections of this bookletfor full details.

    Please request a course-by-course evaluationfor your foreign grades. The assessment will notbe valid without an overall GPA. However, the

    admissions committees of the medical schoolsreserve the right to apply their own evaluation.

    WES evaluations must be sent directly to OMSASby WES, and must be received by the application

    deadline, October 1, 2015.

    Note: OMSAS will convert grades of courses takenat accredited universities in the US. Applicants donot require a WES assessment for these courses.

    Foreign transcripts are processed as non-convertible,

    therefore a GPA is not calculated for universities

    outside Canada and the US. OMSAS will convert

    grades for Canadian university colleges and US

    schools.

    OMSAS uses a 4.0 scale. Your grades will be

    converted using the Undergraduate Grading System

    Conversion Table and the scale noted for your

    university in the table. Do not attempt to reconcilealpha grades to any of the percentage scales.

    Courses for which a Pass grade is assigned are

    counted for credit, but will not be included in the

    GPA calculations.

    Please record all courses and marks, including failed,

    incomplete, repeated and supplementary courses.

    The number and name of each course taken should

    be listed in the same order as it appears on the

    transcript.

    Where both alpha and numeric grades are reportedon the transcript, the numeric grade will be used.

    Do not use a GPA to report grades for individual

    courses.

    Course Weight

    Courses are weighted according to the length of

    study.

    Full-year course: weight of 2

    Half-year course: weight of 1

    Semester course: weight of 1Three-quarter course: weight of 1.5

    Cumulative Average

    OMSAS calculates the Cumulative Average by adding

    all of the course weights and all of the converted

    grades, then dividing the total course weights into

    the total converted grades. Note: The Cumulative

    Average is not an average of the averages.

    Graduate Study Record

    Transcripts are required for all graduate study work,

    although individual courses are not recorded on

    the academic record. The deadline for receipt oftranscripts is October 1, 2015. If you have recently

    started graduate studies for which a transcript is

    not available, send a statement of registration from

    Selection ProcedureThe following requirements are for admission in thelast week of August 2016.

    Our intention is to prepare you to become a physician

    with the capacity and flexibility to select any area inthe broad field of medicine. You are selected with

    this goal in mind. Faculty members, medical studentsand members of the community are usually involved

    with assessing applications.

    Admission and Registration

    Applying to the medical program implies that youaccept the admissions policies and procedures

    and the methods by which you are chosen for theprogram.

    To be considered for an interview, you must meet

    all requirements described in this policy statementunder the following sections:

    Academic Eligibility;

    Application Procedures;

    Transcript Requirements; and

    Computer-based Assessment for SamplingPersonal Characteristics (CASPer).

    Several hundred applicants are invited for aninterview in Hamilton in March or April 2016

    Invitations for interviews are determined byyour academic performance, assessment of your

    preparedness for a career in medicine, suitability

    The following requirements are for admission infall 2016. The admission policy is reviewed annuallyand the requirements from previous years may notapply. The university reserves the right to reviewand change the admission requirements at any timewithout notice.

    Because of the nature of selection procedures,deadlines are strictly enforced. All relevant

    documentation must be provided by the specifieddeadlines. You must follow the instructions precisely.

    Failure to comply with the instructions or meet

    the deadlines will result in your application beingdisqualified.

    False Admission InformationIf any application information is found to be false or

    misleading, concealed or withheld, the applicationwill be deemed invalid and immediately rejected. If

    you are already admitted and registered, you maybe required to withdraw from the university. The MD

    Admissions Committee will usually not allow you tore-apply to the MD program for seven years.

    MD/PhD Program

    McMaster offers an integrated MD/PhD programfor up to three students each year. These students

    Requirements

    Academic Eligibility

    On the Academic Record, you must record algrades received in all credit courses from yourundergraduate degree. Failure to report courses

    programs or grades on the Academic Record wilresult in your application being disqualified.

    You must fulfill the following requirements.

    1. By May 2016, you must complete a minimum

    of 15 full courses, or 30 half courses (or acombination) of undergraduate university workThere is no requirement to carry a full course

    load. To satisfy this requirement, academiccredentials obtained from a Canadian university

    must be from an institution with academicstandards and performance consistent with

    those of member institutions of the Counciof Ontario Universities (COU). You must beable to demonstrate a high level of academic

    achievement consistently throughout yourundergraduate career. If you completed a

    diploma at a CEGEP, you must complete at leasttwo additional years (10 full courses or 20 half

    courses) of degree credit work at an accrediteduniversity by May 2016.

    You are also eligible if you confer a baccalaureatedegree by a university in less than three years

    by the time you apply on October 1, 2015, andyou meet the overall GPA requirement of 3.0 on

    the OMSAS 4.0 scale.

    2. You must achieve an overall simple average ofat least 3.0 on the OMSAS 4.0 scale in yourundergraduate university work to date, by

    October 1, 2015. While this meets the minimumcriterion for admission consideration, be aware

    that, given the rising level of competition fora limited number of positions, a significantly

    higher GPA would provide you with a morereasonable chance of admission.

    Marks for supplementary and summer courseswill be included in the GPA calculation. Courses

    for which a pass grade is assigned are countedfor credit but are not included in the GPA

    calculation. If you are attending a program wherepass or credit is given in some courses in

    lieu of a letter or numerical grade, independentgrades from a minimum of five half-year or fivefull-year courses are required or your application

    will not be considered.

    Application Procedures

    Completed online OMSAS applications are due by4:30 p.m. (ET), October 1, 2015. Fees paid by credit

    card are also due at this time. If you pay by onlinebanking or Western Union Business Solutions GlobalPay for Students (bank-to-bank transfer for

    international payments only), OMSAS must receiveyour payment by October 5, 2015.

    OMSAS must receive transcripts from all

    postsecondary institutions you attended, byOctober 1, 2015. See Transcript/RegistrarStatements Requirements on page 18.

    OMSAS expects to receive your three Confidential

    Assessment forms and reference letters byOctober 1, 2015. However, McMaster will accept

    Confidential Assessment forms and reference lettersuntil December 1, 2015, at which time incomplete

    complete CASPer to maintain admissioneligibility.

    CASPer assesses the non-cognitive skills andinterpersonal characteristics we believe are

    important for successful students and graduatesof our program, and complements our otherapplicant screening tools. By implementing

    CASPer, we are trying to further enhancefairness and objectivity in our selection process.

    To take CASPer, you must have access to a

    computer with audio capabilities and a reliableinternet connection on your selected testdate. CASPer can be taken anywhere that you

    can satisfy the aforementioned requirementsExceptions will not be made if you are unable to

    take CASPer online due to an unreliable internetconnection, as a result of technical or politica

    factors.

    The CASPer test is comprised of 10-12 sectionsof video and written scenarios. Following eachscenario, you must answer a set of probing

    questions under a time contract. Each responseis graded by a different rater, which offers a

    robust and reliable view of the personal andprofessional characteristics that are important

    to our program. You do not need to study forCASPer, although you may want to familiarizeyourself with the test structure and ensure you

    have a quiet environment to take the test.

    Aboriginal Applicants

    If you wish to be considered as an Aboriginaapplicant (First Nations, Inuit or Mtis), you mustprovide the following when you apply:

    1. A letter that declares Aboriginal ancestry

    and provides specific information about FirstNations, treaty, community, or organizationa

    affiliation. The letter should requestconsideration under the alternative process

    expand on your academic and personabackground, and describe your reasons andmotivation for wishing to become a physician.

    2. A letter of recommendation from your FirstNations, Band Council, Tribal Council, Treaty

    community or organizational affiliation.3. Proof of Aboriginal ancestry. Acceptable proofs

    of ancestry include: Status or Treaty card, MtisMembership card, Nunavut Trust Service cardor Inuit Roll Number. McMaster University wil

    An overall simple average will consist of allundergraduate university degree credit courses

    taken, except those taken on foreign exchange,in which the work of different years will be

    treated equally. You calculate this average in theAcademic Record section, then verify it on the

    OMSAS Verification Report. We may also reviewthis average.

    Your graduate experience will be consideredin the admissions process if it is complete

    and the degree has been conferred by theOctober 1, 2015, application deadline. Individual

    grades received for course work taken as partof a graduate degree will not be included in theGPA calculation. Applicants with a completed

    and conferred graduate degree at the time ofapplication will receive additional consideration,

    to be included in the formula that determines thelikelihood of an invitation to interview (masters

    degree holder +0.01; PhD degree holder +0.04),providing a significant, albeit not decisive,

    advantage for graduate degree holders.

    If you are enrolled in a graduate program atthe time you apply, you must arrange for yoursupervisor, a member of your supervisorycommittee or the Chair of the department toprovide a letter indicating that they are awareyou intend to apply to medical school. OMSASshould receive this letter by October 1, 2015.If your graduate supervisor, member ofyour supervisory committee or Chair of thedepartment is one of your referees, a secondletter is not required.

    3. You must write the Medical College AdmissionTest (MCAT) prior to the October 1, 2015,

    deadline. The score from the Verbal Reasoningsection of the MCAT will be used for those whowrote the MCAT before April 2015. If you write

    the new MCAT 2015, the Critical Analysis andReasoning score will be used in both formulae

    (offer of interview and advancement to collation[full file review]). A minimum score of 6 on the

    Verbal Reasoning component or 123 on theCritical Analysis and Reasoning component is

    required. The scores on the other sections ofthe MCAT are not considered in the selectionprocess. MCAT test results are accepted

    provided the test was written no more than fiveyears prior to the application deadline. The most

    recent test result will be used if you attempt theMCAT more than once.

    If you do not use the TRF, transcripts shouldbe ordered before September 15, 2015, to allow

    adequate processing time and for OMSAS toreceive them by the deadline. Be prepared to send

    OMSAS a copy of all dated postage receipts andcorrespondence related to your transcript requests

    by the application deadline, if required. In the eventof a missing or late transcript, you will be requiredto provide evidence to McMaster University that you

    requested transcripts in a timely fashion.

    Foreign Transcripts/Registrar StatementsIf you have foreign transcripts (outside Canada or

    the United States) and require your foreign gradesto be included (if you have not met the minimumcourse number criteria utilizing your Canadian or US

    data), you must have your transcript sent to WorldEducation Services (WES) (see the Credentialing

    Foreign Grades section). Your WES report anda copy of your verified transcript must be sent

    directly from WES to OMSAS by October 1, 2015.

    Note: It is not possible for OMSAS to notify you ofany outstanding transcripts before October 1, 2015Therefore, it is your responsibility to ensure that

    OMSAS receives all transcripts and registrarstatements by October 1, 2015. Your application wil

    be disqualified if you fail to meet this requirement.

    Other Transcript Requirements to AvoidDisqualification

    1. McMaster requires that you provide transcriptsfor all postsecondary courses/programsattended. This includes community colleges

    CEGEPs, junior colleges, pre-universityprograms, etc.

    2. OMSAS must receive transcripts and grade(s)for courses taken on a Letter of Permission

    at another institution, or for which transfercredit/advanced standing was granted, byOctober 1, 2015. This applies to courses and

    terms/semesters of study taken abroad as partof a regular program.

    3. Evaluations of work terms done as part of aco-operative program are not required.

    To qualify for (1) above, you must:a. be a Canadian citizen or permanent resident by

    October 1, 2015; andb. have resided in Ontario for at least three years,

    since the age of 14, by the date of possible entryto the program.

    Transcript/Registrar Statements RequirementsTranscripts from Ontario universities ordered

    through OMSAS via the Transcript Request Form(TRF) will be accepted if OMSAS receives the

    TRF by October 1, 2015. If you attend/attended anOntario university, you must use the TRF. If youattend/attended an Ontario college, you may use

    the TRF. All other required transcripts and registrarstatements must be submitted directly to OMSAS

    by the postsecondary institutions attended.

    Police Information Check

    Throughout your medical school program, you wil

    serve vulnerable populations. In an effort to protectthese vulnerable people against potential risk of harm

    the Ontario Faculties of Medicine and many clinicaagencies require that you provide confirmation ofthe absence of a criminal conviction or outstanding

    criminal charges. At the beginning of each subsequentacademic year in the MD program, you will be

    required to sign a criminal record and disclosure

    form to confirm that there has been no change in theinformation contained in the police information check

    The police information check, including a VulnerableSector Screening, is required.

    Offer of admission is contingent upon provision of

    the above checks, at your expense. The Michael GDeGroote School of Medicine will review any files

    presented with a Not Clear check to determinewhat action, if any, will be taken. For moreinformation about obtaining a police information

    check with Vulnerable Sector Screening, pleasecontact your local police agency.

    If you receive an offer of admission to the MD

    program, you must provide your police check to theMD Admissions Office by August 1, 2016.

    Immunization

    You will receive details about health regulationswhen you are offered admission to the program.

    Advanced Standing/TransferYou are required to begin with Medical Foundation 1

    Therefore, there is no provision for advancedstanding or transfer into the program.

    Basic Life Support Training for

    Health Care ProvidersIf you are offered admission, you must provideevidence of a current Basic Life Support (BLS) fo

    Health Care Providers (HCP). Courses are readilyavailable in most communities, from both the Red

    Cross and the St. Johns Ambulance associations. Youwill be sent specific information before registration.

    Credentialing Foreign GradesWhether you are a Canadian or non-Canadian

    applicant, if you do not meet minimum coursenumber criteria by utilizing your Canadian or US

    data and require your international educationdata to be included, you must have your foreign

    transcript assessed by WES at: www.wes.org.Credentialing assessment means converting foreignacademic credentials into their Canadian educational

    equivalents. See the Transcripts section of thisbooklet for further details.

    If you have a foreign university education and

    meet the minimum course number requirementwith Canadian or US undergraduate universitydegree-level work, the foreign grades will not be

    used in calculating the GPA. In this case, a WESassessment is not required; however, the transcript

    must be sent directly from the foreign university toOMSAS by the deadline.

    English-Language Proficiency

    To be granted admission, you must be proficient inspoken and written English. You are expected towrite clearly and correctly in English. All application

    materials must be submitted in English, otherwisethe application will not proceed further in the

    admissions process.

    If English is not your first language, you mustsatisfy at least one of the following conditions by

    October 1, 2015:

    1. Provide evidence that you achieved a score of

    at least 86 on the internet-based TOEFL (iBT)with a minimum score of 20 in each of the

    four components, or the equivalent on otherrecognized tests (McMaster Universitys TOEFL

    code is #0936); or2. have attended an educational institution for

    General Information

    The Northern Ontario School of Medicine (NOSM)

    is the first medical school in Canada for the 21 stcentury. Our mission is to contribute to improving

    the health of the people and communities ofNorthern Ontario by advancing the highest qualityof medical practice, learning, teaching, research

    and professionalism. Our School of Medicine isthe Faculty of Medicine of Laurentian University,

    Sudbury, and of Lakehead University, Thunder Bay.With main campuses in Thunder Bay and Sudbury,

    Patient-Centred Medicine (PCM) is the major

    underlying concept of health and medicine inour MD program. PCM is a comprehensive clinical

    method with six interactive components supported

    by substantial and growing research evidence. PCMlinks well to Learner Centred Education (LCE),which is the underlying concept of NOSMs overalleducation.

    The curriculum is organized around five themes

    throughout the four-year program:

    1. Northern and Rural Health;2. Personal and Professional Aspects of Medical

    Practice;

    3. Social and Population Health;4. Foundations of Medicine; and5. Clinical Skills in Health Care.

    The focus of NOSMs MD program is on graduatingskilled physicians who are ready and able to

    pursue further training and clinical practiceanywhere, but who have a special affinity for, and

    comfort in, Northern Ontario. The curriculum ishighly integrated, where much of your learning is

    small-group, patient-centred, case-based learningThe cases present complex, real-life scenarios thatpresent people in their home, family and community

    contexts.

    You will also participate in hands-on practicaclasses, self-directed learning, and clinical education

    in a range of different health service and communitysettings. Through the mix of themes and different

    learning modalities, the program covers corecurricula to ensure a strong grounding in basicmedical sciences, humanities, social and behavioura

    sciences, and clinical medicine.

    Clinical education starts at the beginning of theprogram and occurs in a range of different settings

    Community-based medical education is a keycomponent of the program where you will learn inother and larger hospitals, health services, family

    practices, and various community settings. Thisapproach ensures you gain a diversity of clinica

    knowledge and skills, and experience the speciafeatures of Northern Ontario.
