The BlueCare Freedom Dental AdvantageYou’ve chosen a dental plan from Blue Cross and Blue Shield of Texas (BCBSTX), aDivision of Health Care Service Corporation, a company that has provided dental coveragesince 1973 and currently serves more than 800,000 dental members. We’ve beenproviding health care benefits longer than most companies have been around.

Freedom to ChooseAs a BlueCare Freedom Dental plan member, you have lower out-of-pocket costs whenyou choose a dentist in the BlueCare dental network. You may also choose to use aDentaBlue dentist, however, your out-of-pocket costs may be greater. You and yourdependents will receive benefits based on the BCBSTX allowable amount and willnot be billed for any remaining balance. In addition, you will not be responsible forcharges over the allowable amount and you will not have to file claims. You willonly be responsible for any coinsurance or deductible as well as limited and non-covered services.

With BlueCare Freedom Dental, you always have the option to receive care from dentists who are not in the network. However, your out-of-pocket expenses may behigher. Non-contracting dentists will charge their regular fee and may bill you forthe balance not covered by your benefit plan. Additionally, you may have to submityour own claims for reimbursement.

Finding a Dentist Is EasyWith more than 139,000 dental access points throughout the U.S. and 12,700 in Texas, BCBSTXoffers you flexibility in choosing a dentist. A complete list of contracting dentists* can be foundonline. Visit www.bcbstx.com and use the Provider Finder. Under Find a Doctor, select BlueCare

Dental, or DentaBlue, in the dropdown menu, and click Find. You can search for a contracting dentistnear your home, school or office, and conveniently download a map with driving directions. In

addition, you can create and print a custom provider directory based on your preferences. You also maycall our Customer Service Center toll-free at (800) 521-2227 for help finding a dentist.

BlueCare Dental ConnectionSM

BlueCare Dental Connection is an integrated medical-dental and preventive outreach program that compares

individuals’ medical and dental data. This allows us to identify and reach out to members at risk for health

conditions complicated by periodontal disease, such as:

• Diabetes

• Cardiovascular disease

• Pregnancy

BCBSTX’s preventive program will reach out to select members identified as at risk for cavities and periodontal

disease, in an effort to minimize these dental conditions early on, or prevent them altogether.

To help educate at-risk members about the importance of maintaining good oral/periodontal health,

we will provide:

• Educational member mailings

• 24-hour online access to the Dental Wellness CenterTM, which offers current

educational articles and special tools to assist members with their oral care.
