Job Title: Aftercare Coordinator

Salary: £11.81 p/h

Home from Hospital Care

The Charity, Home from Hospital Care works to improve discharge planning and aftercare for patients returning home from health treatment, in order to optimise their chances of recovery. Our voluntary Welcome Home Service learns of patients needing help from nurses, therapists, social workers and care workers, both in hospital and in the community, general practitioners, carers or patients themselves. Our volunteers have a range of different qualifications and undertake tasks according to their abilities and location.

Home from Hospital Care in partnership with Forward Carers have been commissioned to set up a Welcome Home Service in the Sandwell Area. The post holder will be required to implement and develop the service to meet the needs of discharged patients leaving Sandwell Hospitals.

OVERALL PURPOSE OF THE ROLE:

1. Main Duties

1.1 Liaise closely with and work under the direction of the manager of the Home from Hospital Care Service, attending team meetings and other meetings as required.

1.2 Work with the manager towards the future development of the charity, and to develop and expand our service within the Sandwell area

1.3 Liaise closely with admissions and discharge manager, delayed discharge manager, adults and communities teams, ward staff, occupational therapists and physiotherapists at the City Hospital. Work alongside any partner agencies to ensure organisational objectives are being achieved.

1.4 Attend Multi-Disciplinary Team meetings as required.1.5 Undertake visits to the wards, liaising with relevant ward staff and talking to

patients and their families to discuss any support needed at home on discharge (in addition to any support offered by Adults and Communities), working to targets set by the Funder and the Charity.

1.6 Promote the charity and its work at their main hospital base and give talks/presentations to agencies and community groups to raise awareness.

1.7 Accompany new volunteers on their first visits and undertake visits if necessary. Where necessary, complete follow up visits with the volunteer or patient.

1.8 Facilitate work required by the Handyman Service.1.9 Purchase and deliver food parcels to patients leaving the hospital when

required.1.10 Build up a database of resources such as luncheon clubs, mobile

hairdressers, chiropodists, cleaners, gardeners etc. Support volunteers with their requests for assistance and guidance.

1.11 Set up a robust recording system that captures detailed data for each referral1.12 Prepare quarterly reports for the manager using an appropriate system, e.g.

Excel.1.13 Ensure patient feedback questionnaires are completed and returned for

analysis.1.14 Assist and be involved in organising training for volunteers with the Volunteer

Coordinator and encouraging attendance.1.15 Organise and attend fundraising and promotional events. 1.16 Prepare for and attend quarterly Trustee Meetings.1.17 Undertake relevant mandatory and other training and be responsible for own

self-development for benefit of charity and personal development.

2. People2.1 Provide support, guidance and assistance to Aftercare Support and

Volunteer Coordinators.

3. Safeguarding3.1 The post holder is responsible for promoting and safeguarding the welfare of

vulnerable people for whom they are responsible, or come into contact with, in line with the Charity’s safeguarding policy.

4. Financial4.1 Ensure volunteers expenses claims are checked and processed accurately

and forwarded to the Finance manager in a timely manner.4.2 Ensure financial rules regarding use of petty cash and expense claims are

followed.

5. Policies and Procedures5.1 The post holder will be responsible for ensuring familiarisation with all

relevant policies and procedures and will ensure that tasks, duties and responsibilities are carried out in line with same.

6. Lone Working6.1 The post will require an element of lone working and the post holder will be

expected to comply with any relevant policies or procedures to ensure their own safety whilst undertaking their duties.

Skills, abilities, personal qualities, experience and knowledge

Essential Desirable Organisation skills Personable and able to get on with

people Caring and compassionate Experience in managing or

supporting a team Reliable and responsible Demonstrable IT skills including the

ability to use Microsoft Word, Excel and Outlook

Access to a car and a clean driving licence. Travelling expenses are paid when using the car during your working day, but not for travelling to and from work. Any parking charges related to your work will also be reimbursed

Presentation skills Ability to work as part of a team but

also to work on your own with minimal supervision

Networking skills and experience of partnership working

Ability to use own initiative to make decisions

Report writing skills Flexible approach to work Proven ability to communicate with a

diverse range of people with a number of different needs

Previous experience of working in health/social care setting

Experience of using Microsoft Publisher and PowerPoint

Experience of managing/working with volunteers

Knowledge or experience of one or more of the four specialisms (Substance misuse; Homelessness including advocacy/benefits, Dementia, Mental Health) and willingness to undertake additional training as required

Good knowledge of Birmingham area

DBSThe successful candidate will be required to undergo a DBS check.
