Invitation for Bids

1. The Govt. Eye Cum General Hospital intends for the purchase of Iron

Racks. It is intended that part of the proceeds of this budget will be applied

to eligible payments under the contract for supply of following items:

2. The Govt. Eye Cum General Hospital now invites sealed bids from eligible bidders,

Manufacturers, authorized Sales & Service Dealers for the supply of above mentioned items.

3. Bidding shall be conducted through Open Competitive Bidding (Single Stage-Two Envelope)

procedures specified in the Punjab Procure Rules (PPRA) 2014 (amended Jan 2016), and is open

to all eligible bidders as defined in the bidding document.

4. Interested eligible bidders may obtain bidding documents and further information from Govt.

Eye Cum General Hospital

5. Sealed Bids must be delivered to the above office on or before 10:30 a.m. on 07 June, 2017

and must be accompanied by a Bid Security as mentioned above in the form of CDR/Pay

Order/Demand Draft/Bank Guarantee.

6. Bids will be opened in the presence of bidders’ representatives who choose to attend at

11:00 a.m.in the Office of Medical Superintendent, Govt. Eye Cum General Hospital, on the

same date.

7. The bidders are requested to give their best and final prices as no negotiations are expected.

8. Taxes will be deducted as per applicable government rules. NTN and Sales Tax registration

certificate must be provided.

Sealed offers are invited from all importers/Suppliers/manufacturers/whole sale dealers and their

distributors having good repute for the Purchase of Lab Stuff for Govt. Eye Cum General Hospital, Gojra. Tender should be addressed in the name of the Medical Superintendent, and with following documents :-

SPECIAL INSTUCTION

1. Sealed Tenders are invited from the General Sales Tax registered Firms for the supply of Iron Racks For Medicines for the financial year 2016 - 2017. Tender form is obtainable on cash payment (non-refundable) from the MS Office of this hospital on submission of written request.

2. Any offer received not as per terms & condition of the tender enquiry/bidding document of this Hospital, framed under Punjab Procurement Rules, Government of the Punjab is liable to be ignored.

3. No offer shall be considered i. If received after the last date and time.

ii. The tender is ambiguous. iii. The tender is unsigned. iv. The tender is conditional v. Tender is sent by telegram.

vi. Offer received without requisite earnest money vii. Offer received from the firm blacklisted or suspended by the

Health Department, Government of the Punjab.

viii. Without the original tender receipt.

4. In case the contractor fails to execute the contract strictly in accordance with the terms & conditions laid down in the contract, the security deposited shall be forfeited.

5. Duly attested photo copy of C.N.I.C of Executive / Authorized Representative must be attached with the tender along with copies of National Tax Number (NTN), General Sales Tax registration certificate .

6. The sealed bids (separate technical and financial bids further sealed in one envelope) will be dropped in the tender Box placed in the Committee Room of this hospital on the scheduled date & time.

TERMS & CONDITIONS

1. The vender must be financially sound, well reputed .

2. The Tendered shall be bound to accept all terms & conditions of the Government / PPRA and any further condition introduced by Government during the period of contract in addition to the terms, conditions, Rules & regulations of PPRA.

3. Any erasing / cutting, etc. Appearing on the offer, must be properly signed by

the person signing the tender.

4. The bidders are required to give their best and most competitive prices for the items.

5. Deduction of income tax will be made as per prescribed rules

6. The Medical Superintendent reserves the right to accept or reject any / all tender(s) without assigning any reason.

THE BIDDING PROCEDURE

1. Single Stage – Two Envelopes Bidding Procedure as per of PPRA Rules 2014 shall be applied: The bid shall comprise a single sealed package containing two separate sealed envelopes. Each envelope shall contain separately the Financial Proposal and the Technical Proposal; The envelopes shall be sealed & marked as “FINANCIAL PROPOSAL” and “TECHNICAL PROPOSAL” in bold and legible letters to avoid confusion; Initially, only the envelope marked “TECHNICAL PROPOSAL” shall be opened; The envelope marked as “FINANCIAL PROPOSAL” shall be retained in the custody of Procuring Agency without being opened; The Procuring Agency shall evaluate the Technical Proposal, without reference to the price and reject any proposal which do not conform to the specified requirements; During the technical evaluation, no amendments in the technical proposal shall be permitted; The Financial Proposals of bids shall be opened publicly at a time, date and venue to be announced and communicated to the bidders in advance;

After the evaluation and approval of the technical proposal, the Procuring Agency shall at a time within the bid validity period, publicly open the financial proposals of the technically accepted bids only. The financial proposal of bids found technically non-responsive or non- compliant shall be returned unopened to the respective bidders. The bid with maximum discount rate on given specifications shall be accepted.
