Since the introduction of HTM 01-05 more and more dental practices are ceasing to use ultrasonic baths, some because practices have moved onto fully automated washer disinfectors, but others because they have been advised that they require expensive validation and testing requirements.
There are many benefi ts to using an ultrasonic bath as part of the decontamination process: Although manual cleaning is an accepted method of cleaning instruments as part of HTM 01-05 essential quality requirements, this method cannot be validated.
Historically the majority of ultrasonic baths in dental practices are relatively small pieces of kit, usually consisting of a small bath, basket, lid and either a manual time clock or digital display. Misinterpretation of HTM 01-05 has led some people to believe that these baths do not meet HTM 01-05 regulations and should therefore be decommissioned leaving practices no choice but to revert back to the un-validated method of manual cleaning.
Where the confusion lies
Many practices, as said, have put their ultrasonic cleaners in the back of the cupboard because they believe them to be non-compliant, reasons given are:
• It doesn’t have a lockable lid
• It doesn’t have a printer or data logger
• It doesn’t have a built in temperature sensor
Many Ultrasonic baths cannot meet these specifi cations; however it is possible to still use the bath within your practice by creating standard operating procedures (SOP) that all staff members follow.
This can be a document, one of which all staff members have signed to say they understand the SOP and it should be on display within the decontamination area.
This document would explain:
• Correct detergent and what the dosage is; how to make up the detergent. If you require a measuring jug for example
• Purchase a thermometer: Document the need for the continuous monitoring of the ultrasonic bath throughout cycles; ensuring that it doesn’t ever exceed 45°C
Ensure that it is documented that staff do not remove the lid of the ultrasonic bath part way through a cycle.
Testing and Validation
The periodic testing required by HTM 01-05 are all tests which can be performed by a suitably competent member of staff. Information for these tests can be found on the ISL website www.HTM01-05.com
Isopharm are of the opinion that if you have a ‘standard’ ultrasonic bath within your practice then you do not require an engineer to visit quarterly or annually to validate this type of ultrasonic as there are no tests that can be performed by an engineer that cannot be performed by a competent person within the practice.

If you have a fully ‘compliant’ ultrasonic bath such as the Ultrawave Hygea 2 or Walker Electronics Q105 then it would be advisable to have the engineer validate this as these are far more intricate machines.
It is extremely important to ensure that when you are purchasing a new ultrasonic bath, you are sold/purchase a bath that is fit for purpose.
Ensure that you know the manufacturers recommended testing routine and again that it is fit for purpose
For example there are ultrasonic baths which have built in heaters which automatically take the temperature above 45°C which is not acceptable for compliance with HTM 01-05.

Essential Quality requirements
2.4 h) Cleaning and inspection are key parts of satisfactory dental instrument reprocessing. Instruments may be cleaned using an ultrasonic bath, but this should be covered during use to restrict the release of aerosols. Manual cleaning may also be used. Practices should plan for the introduction of washer-disinfectors.
Inspection processes should ensure that the standards of cleaning achieved are visually satisfactory – that is that instruments are free from particulate contamination, salt deposits or marked discoloration. The use of a simple magnifying device with task lighting will improve the value of this part of the process
The inspection should identify:
• Instruments that are blunt, bent or damaged or show any signs of pitting or other corrosion should be discarded or, if appropriate, sent for repair.
• There is free movement of all parts and that joints do not stick.
• The edges of clamping instruments meet with no overlap and that teeth mesh together.
• Scissor edges meet to the tip and move freely across each other with no overlap or rough edges.
• All screws on jointed instruments are tight and have not become loose during use.
• Instruments should be inspected for any visible soiling such as blood or dental materials, especially joints, hinges or the serrated surfaces of jaws, which are diffi cult to clean.
• If there is any residual contamination, the instrument should be rejected and should undergo another cycle of the cleaning process.
At Isopharm Sentry, when we visit dental practices to carry out our CPD onsite training, what we tend to fi nd is that whilst practices have an inspection magnifi cation lamp the majority of practice staff do not use this device and go straight from the cleaning process to the steriliser, omitting the inspection stage. The biggest reason for this being that staff members feel they do not have adequate time to carry out this part of the decontamination process.
However as stated, this part of the decontamination process is critical to ensure adequate sterilisation of instruments.

TST Strips, or Emulating Indicators to give them their offi cial title, are a great method of simple cycle validation, being used in the majority of dental practices for many years. Place one on a tray, run a cycle and the colour change tells you a pass or a fail, there is nothing easier.
With HTM01-05 comes the need to store a vast amount of data for every machine used in the decontamination process. Cycle receipts, test records, manual records, it is endless. If the practice makes use of TST strips to verify the performance of the cycle then these also need to be kept.
It is worth noting that there is absolutely nothing in HTM01-05 to say that TST strips or Emulating Indicators need to be used at all. However, they do provide a highly recognisable, not to mention economical, method of cycle validation so their use is generally smiled upon by those auditing a decontamination process.
If the practice decides to make TST Strips part of the validation process, how often should they be used?
Putting a TST Strip in every cycle of the day obviously provides a good level of basic performance validation, but will signifi cantly add to the already mounting stack of hard evidence.
What to look at is what HTM01-05 actually requires. With regards to monitoring each and every cycle HTM01-05 states that if the machine has either a printer or a data logger then these records should be kept.

Therefore, if the machine has either a printer or a data logger attached then it is our opinion that a single Class 6 TST Strip should be used in the fi rst cycle of the day only. This will provide a single TST Strip as evidence to accompany the printout/data stating that steam was present in the chamber (something that printers and data loggers do not tell you), that the cycle was at least 134°C and that this was for over 3 minutes.
If the machine does not have a printer or data logger attached it is our opinion that this is the time to use a Class 6 TST Strip in every cycle as no other validation is taking place on a cycle to cycle basis.
Make sure that only Class 6 TST Strips are used. Class 6 TST (Time/Steam/Temperature) provides the best level of cycle verifi cation compared to other classes of test strip. Class 4 for example will only indicate the presence of steam in the chamber and not the temperature or that the cycle was for over 3 minutes.
Do You Need a TST Strip in Every Sterilisation Cycle?
Since the introduction of HTM 01-05, many dental practices have been given the impression that the use of alcohol-based wipes is prohibited: this is not the case.
HTM 01-05 States:6.57 ‘….Care should be taken in the use of alcohol wipes, which though effective against viruses on clean surfaces- may fi x protein and biofi lm….’
‘NOTE’ Alcohol has been shown to bind blood and protein to stainless steel. The use of alcohol with dental instruments should therefore be avoided.
Providing that a dental practice has a risk assessment and procedure on the use of alcohol based products and their use does not confl ict with the above exert from HTM 01-05 then alcohol wipes can have a place within the dental practice.

Yes alcohol can still be used to disinfect and clean surgeries. It is a common misconception that alcohol wipes are not allowed, However what HTM 01-05 does say is that ‘Care should be taken in the use of alcohol wipes. Alcohol has been shown to bind blood and protein to stainless steel. The use of alcohol with dental instruments should therefore be avoided.’
Basically this means that if there is a large amount of blood present on instruments or on any stainless steel then alcohol should not be used.
Alcohol wipes are quite often preferred to non-alcohol wipes as they have a faster drying time and tend not to smear. Providing you follow the correct application then there is no reason that you cannot use them.
In usual circumstances Masks are effective for only around 20 minutes so therefore the correct process would be to change your mask every 20 minutes.
Now this obviously poses an issue if you are treating a patient for longer than that time period so our opinion is that you should change your mask every 20-30 minutes where possible (in between patients) and when you are treating patients for longer than that ensure you change the mask after the treatment has finished.
TST strips need to be kept for 2 years, or the lifetime of the machine, whichever comes first. HTM 0105 states: 3.19 It is recommended that records be maintained for not less than two years.
HTM 01-05 asks that when you BUY a new ultrasonic that it has a locking lid, however historically ultrasonic baths did not have locking lids. Therefore a practice policy stating that all members of staff understand that the ultrasonic lid must NOT be removed during a cycle, and they must NOT put their hands in the bath when it is mid-cycle should be created.
The reason for this is so that all instruments are ensured a full cycle and that instruments cannot be added half way through and not benefit from the whole ultrasonic cleaning process.
So in short, if you write the policy and explain it to all of your nurses then you can continue to use the baths that you have in the practice without worrying.
Protein residue tests must be performed weekly and it is an essential quality requirement. Protein residue tests should not be confused with the manufacturers soil test.
It is also important to know that the protein residue test is a way of validating the cleaning process prior to sterilisation, therefore we would recommend that protein residue tests are carried out where manual cleaning is the only method of pre cleaning prior to sterilisation.
Can alcohol still be used to disinfect in surgery?

Chapter 2, Essential quality requirements and best practice
Essential Quality Requirements
2.4 (m) Develop a quality system approach so that the storage of wrapped instruments does not exceed 21 days for instruments sterilized in a non-vacuum (type N) sterilizer or 30 days if sterilized in a vacuum (type B or S) sterilizer (see paragraph 4.31):
(i) Where non-vacuum sterilizers (type N – see paragraph 4.5) are used, post-sterilization drying using disposable non-linting cloths should be supplemented by the packaging of instruments – this will improve resistance to contamination and re-colonisation in storage.
(ii) For vacuum sterilizers (type B or S – see paragraph 4.24), pre-wrapping will extend storage life to 30 days.
(iii) Subject to local policy choice, these measures will help to ensure stock rotation and will tend to limit recontamination of stored instruments. Simple but clear record keeping will be required to make these measures effective.
Chapter 3, Cleaning instruments

3.32 Instruments should be sterilized as soon as possible after cleaning to avoid air-drying (which can result in corrosion and/or microbial growth). For instruments processed in a vacuum sterilizer, before being wrapped, instruments should be dried using a disposable non-linting cloth.
Chapter 4, Sterilization:

4.22 Before carrying out the daily tests, the user should:
• Clean the rubber door seal with a clean, damp, non-linting cloth;
• Check the chamber and shelves for cleanliness and debris;
• Fill the reservoir with freshly distilled or RO water;
• Turn the power source on.
Packaging and related decontamination strategy
4.24 There are three combinations of steam-sterilization and instrument-wrapping strategies that can be used within dental practices:
With a type B vacuum steam sterilizer, instruments will be pre-wrapped using purpose- designed materials that are compatible with the sterilizer.
With a type S sterilizer, instruments may be placed in purpose-designed cassettes. In both cases, instruments should be dry before they are placed in the purpose-designed packaging. Wrapping should take place with a dry product shortly after washing and disinfection. Once the wrapped instruments have been sterilized satisfactorily, the product may be stored for up to 30 days (see paragraph 1.24). Note: instruments need to be dry before they are packaged.
With a displacement steam sterilizer (type N), the instruments will not be wrapped prior to sterilization. Immediately after removal from the sterilizer, instruments may be wrapped using suitable sealed view-packs. Instruments should be dry before being packed.
In addition, the entire tray may be placed within a sealed pack for storage purposes. In both of these instances, storage for up to 21 days is recommended.

Do you find that your decontamination room has sheets of paper everywhere?
One for recording the temperature of the autoclave, one to tell you that you have changed the water in the reservoir, and then there are all those test strips, curled up at the edge, stapled to yet another sheet of paper.
Why not consider getting a dedicated logbook for each piece of equipment that you use in your practice to keep all that important evidence in one place.
Evidence is a key part of what CQC inspectors are looking for to gauge a practice’s compliance.
What better way to prove that you run a tight ship and are in control of all your evidence than to be able to hand them a book where all staff roles and responsibilities are clearly defined, each machine’s varying test requirements are set out, every tick box required is listed, and of course is all complete with the evidence to back it up in the same book.
HTM 01-05 requires that:
11.4 All pieces of decontamination equipment will need a protocol for validation at installation.
11.9 These protocols will require full implementation and all results need recording in a logbook dedicated to individual equipment. Standard logbooks summarising all required tests are available for most types of decontamination equipment. Manufacturers should be consulted on the contents of the logbook.
Products from a type N sterilizer may also be transferred for use within the current session. In this instance, while covering the instruments is essential to protect against dust and aerosols, wrapping is not required.
However, the instruments are not regarded as “stored” and should therefore be used or streamed for a further decontamination process within one session. Instruments should be dry before being transferred for clinical use.
4.25 In all three cases, the instruments should be dried using disposable non-linting cloths and be appropriately handled. It is essential to ensure that the cloth is adequately dry and free from contamination. Accordingly, the cloth should be disposed of after each sterilizer load.

10.29 If recommendations include wiping with a detergent solution, then a clean non-linting cloth plus the recommended detergent solution (to wipe the instrument) should be used.
This should be followed by wiping with a clean damp non-linting cloth to remove residues. The instrument should be dried thoroughly using a clean non-linting cloth.
In summary:
• New reusable instruments requiring cleaning with detergent should be done so using a clean non-linting cloth
• Instruments regardless of how they are sterilised should be dried prior to pouching or be dried prior to clinical use if being used within the same session.

6.54 The dental practice should have a local protocol clearly outlining surface- and room-cleaning schedules.
The cleaning process will be most effective if the more contaminated areas are cleaned fi rst. Materials and equipment used to clean clinical areas and other higher-risk areas should be stored separately from those used for general and non- clinical areas.
Simple records should be maintained in accordance with the HCAI Code of Practice.

6.61 The patient treatment area should be cleaned after every session using disposable cloths or clean microfi bre materials – even if the area appears uncontaminated.

• local work surfaces; • dental chairs; • curing lamps; • inspection lights and handles; • hand controls including replacement of covers; • trolleys/delivery units; • spittoons; • aspirators; • X-ray units.
6.63 Areas and items of equipment that need to be cleaned after each session include:
• Taps• Drainage points• Splashbacks• Sinks
In addition, cupboard doors, other exposed surfaces (such as dental inspection light fi ttings) and fl oor surfaces, including those distant from the dental chair, should be cleaned daily.

7.1 b. All devices should receive disinfection according to the manufacturer’s instructions. This will involve the use of specifi c cleaning materials noted in the CE-marking instructions. After disinfection, the device should again be thoroughly washed. This process should occur before and after any device is placed in a patient’s mouth.

A type ‘N’ steriliser is also known as a downwards
displacement steriliser. Steam enters the chamber, pushing
the air out prior to sterilisation. This type of
machine is not capable of removing air from difficult
loads so should only be used for unwrapped solid
instruments.
A type ‘B’ steriliser is able toprocess more difficult loads such as wrapped items and holloware. It is able to do
this as the machine removes air from the chamber by creating a vacuum before
applying steam to the load.
A type ‘S’ steriliser can onlysterilise loads that the
manufacturer states themachine can process. Themachine manual should
indicate what can and cannotbe processed, or the
manufacturer themselves willhave this information. If youare unable to clarify this thenyou must treat the machine
as a type ‘N’ steriliser.
