ONLINE BENEFITS ENROLLMENT INSTRUCTIONS Use these instructions to help guide you through your 2019 online benefits enrollment process through e-Vantage. You can enroll for benefits online any time, any day and from any location during the enrollment period!

Note: As you go through the online enrollment on e-Vantage, click Edit to make changes and Continue to advance to the next screen. After you enroll in each of the plans, the system will display the Enrollment Summary screen to confirm your benefit selections and the biweekly premium.

LOG IN TO E-VANTAGE (IF YOU DO NOT KNOW YOUR LOG IN INFORMATION, CONTACT YOUR ACCESS COORDINATOR) From home:

- Go to emoryhealthcare.org/i-am/index.html and click the Go to e-Vantage link on the e-Vantage tile. - Log on using your network ID and password. Note: You must be enrolled in Duo Security Two-Factor

Authentication: ourehc.org/departments/is/Security/duo/

From work: - Go to the intranet, ourehc.org, and click on the e-Vantage quick link in the upper right corner.

ACCESS THE BENEFITS ENROLLMENT TOOL - Once logged in, select Self Service from the top dropdown menu.

- Select the Benefits tile. - Click Benefits Enrollment. - Click Select. The next screen displays the Benefits Certification question to be completed. - Click Submit, then your next page is the Benefits Enrollment page.

MEDICAL, DENTAL AND VISION PLANS - Click on the Edit button next to each plan name.

- Choose your coverage options. - Click on the Add/Review Dependent and enter the required information. - Continue to the Benefits Enrollment page and select the “Enroll” box next to their names at the

bottom of the page to enroll your dependents for the plan coverages you would like for them to receive (medical, dental and/or vision).

LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD & D) - For employee: Select your options, the amount and your beneficiaries. Use the instructions in the

previous step to update your beneficiary information. - For spouse and child: Choose your options, the amount and the dependent(s) to be covered. You

are automatically the beneficiary.

DISABILITY AND LEGAL PLANS – Choose your option only. Click Continue to advance. HEALTH CARE & DEPENDENT CARE FSA AND HEALTH SAVINGS ACCOUNT PLAN - REMEMBER: You must re-enroll in the health care or dependent care FSA each year.

- Choose your coverage options and the amount. Click Continue to advance.

FINISHING UP! - When you’re done with your elections, click Continue at the bottom of the Enrollment Summary page.

- Read the Authorization section, then click Submit to complete your 2019 elections. - Click on the link View/Print Elections to print a confirmation page of your elections for your records. - To view your elections after you have submitted them, log on to e-Vantage and select Self Service,

then the Benefits tile and then Benefits Summary. Please allow 24 -48 hours after enrollment for them to be displayed. You may modify your elections up to 31 days from your first day of employment.

- LOG OUT! Make sure you log out of e-Vantage, and, if at a shared computer, make sure to log out of the VDT to protect your personal information!

Call the Employee Resource Center at 404.686.6044 for assistance. Note: Benefits Enrollment will be temporarily unavailable during payroll and/or benefit processing.

403(b) – Retirement Savings/Matching Plan

Employees must complete two steps in order to enroll into the 403(b) Retirement Savings/Matching Plan.

Step 1: Enroll and Select your fund allocations online

Step 2: To select your contribution amounts log onto e-Vantage.

Note: Through e-Vantage employee’s can select contributions, making changes, stop contributions and change

vendor elections. You can update your 403(b) selections at any time through e-Vantage.

Follow these steps:

a. From Work: Go onto Emory Healthcare intranet home page (www.ourehc.org) and select e-

Vantage from the menu on the right.

b. From Home: Go onto Emory Healthcare website (www.emoryhealthcare.org) and click

“For Employees” at bottom of page. From the menu on the left select Virtual Desktop, install the

software & log in.

2. Log in using your Emory Healthcare network ID and password.

3. Click Self-Service on the left.

4. On the Self-Service page click Benefits; then click “403(b) Savings Plan Election.”

5. To make elections and vendor selections “Click here to enroll, change or stop 403(b) elections" and/or

To calculate your maximum contribution allowed "Click here to model 403(b) maximum contributions"

6. After making your elections, click the “Submit Elections” button. Click “Benefits Summary” to review

your elections. You may print a copy of this page for your records.

Note: Until you contact the vendors to select your funds investment, any contributions made to the plan

will be invested in Emory Healthcare's default fund – A Lifecycle investment.

Step by Step Instructions to Enroll in 403(b)

Go to e-Vantage Click on the following

o Self-Service o Benefits Tile o 403(b) Savings Plan Election

Once there, click on the button “Click here to enroll, change, or stop 403(b) elections.” Then click on the button that says “Begin Enrollment.” View the print screen below for the next set of instructions. Once you have done everything below,

scroll to the bottom of your screen and click on “Submit Elections.”

Enter your contribution percentage, then click on “APPLY.”

This explains that your contribution will be split between BASIC and SUPPLEMENTAL. This will allow our system to give you the correct MATCH percentage (if eligible).

This section is for your BASIC contribution. Check the box for “Pretax” or “Roth.” Then select your vendor. If eligible for the 2% CONTRIBUTION and/or MATCH, you will make elections choices in this section as well.

This section is for your SUPPLEMENTAL contribution. Whatever your supplemental percentage noted above should be entered down here. You can have it all with one vendor, or split your supplemental across more than one vendor. (ie: 1% Fidelity+ 1% TIAA-CREF+ 1% Vanguard = 3%)
