CLINICAL ACTIVITIES SUMMARY As a bilingual board certified pediatric nurse practitioner of Filipino descent, I have extensive pediatric primary care experience in various clinical settings (e.g, private practice, a Health Maintenance Organization (HMO), and federally funded community clinics). I provided pediatric healthcare services for children ranging form newborns, toddlers, pre-schoolers, school-age, and adolescents. I have also served in an expanded role as a home-health nurse practitioner providing early postpartum and newborn healthcare to Spanish speaking mothers who delivered within 1 week receiving services in a San Diego community clinic. Furthermore, I honed my administrative skills as co-director of a the pediatric primary care clinic at The Birth Place - a San Diego multi-disciplinary OB/GYN, women’s and pediatric practice with a free-standing birthing center for underserved women and families from diverse racial/ethnic populations. Other clinical responsibilities included conducting English and Spanish group parenting classes focused on child development from newborn to 12 months with education on safety, immunization, health maintenance, common pediatric illnesses, parenting, discipline, sleep, nutrition, physical activity, and family time. Adolescent healthcare services included GYN and STD clinics and counseling for: substance abuse, pregnancy, contraception, mental health evaluation and specialty referrals.

SERVICE ACTIVITIES SUMMARY As a clinician and pediatric nurse practitioner, I participate with outreach programs serving local homeless shelters, community centers, Thanksgiving events, and with medical outreach programs locally and internationally in Mexico and Vietnam. During 2013-2014, I was the committee chair as the UCSF SON representative with the Pilipino UCSF Student Organization (PUSO) – a Filipino student organization promoting cultural diversity in the student body for all UCSF health care disciplines. I was involved with the 2014 undergraduate recruitment day open to all UC and California State University undergraduate students interested in a career in health care and applying to the various UCSF schools (Nursing, Medicine, Dental, Pharmacy, etc.). PUSO is also involved in organizing and participating in UCSF cultural diversity events throughout the year. During 2015 and 2016 I represented the UCSF SON at the annual PUSO Undergraduate recruitment day for undergraduate students for underrepresented minority students interested in healthcare careers. I also represented the UCSF SON at the Undergraduate Recruitment Day sponsored by UC Davis for marginalized minority students interested in health care careers where I presented our SON program information to potential students from underrepresented minority populations interested in graduate degrees in Nursing. Since 2015 - 2016 I have participated as a preceptor with UCSF Flu Crew Program lead by UCSF students from the schools of pharmacy, medicine, and nursing to provide free flu shots at local community centers throughout the Bay Area serving low-income and diverse populations who may or may not have access to healthcare. From 2015 through the present, I have served on the Board of the Philippine Nurses Association of Northern California (PNANC) as Chairman of Profession Development responsible for organizing three continuing education event for the members and community every year. Recently, I organized and coordinated the PNANC 2016 Spring Conference - Promoting Health Awareness in Filipino Americans to educate health professional and the general public on the Filipino health disparities and research being done to improve health outcomes among Filipinos. I was reelected and will continue to serve another year in this role with the PNANC. Currently, I am actively involved with the San Francisco Host Lions Club - Community Health Fairs rotating throughout various churches in the SF Bay Area providing free healthcare services to low-income underserved populations who have limited access to health care. These health fairs offer free flu shots, BP checks, random blood glucose checks, Hepatitis screening and hepatitis vaccines along with associated health counseling including food bank services. I also volunteer with Cornerstone Church's outreach programs participating at the San Francisco Food Bank and the San Francisco City Teams serving the inner city communities and homeless.

CONTRIBUTIONS TO DIVERSITY Health equity to improve health disparities is the core value that has defined my overall research, clinical, teaching and mentoring philosophy. My research focuses on behavior modification to prevent obesity related chronic disease such as type 2 diabetes and cardio-metabolic disease. To reduce health disparities my research emphasizes community based participatory research to design culturally relevant weight loss lifestyle interventions promoting physical activity and healthy eting that include intervention delivery using mobile technologies. The objective of my current clinical trial research studies is to identify effective strategies to mitigate the prevalence of obesity related chronic diseases in the high-risk underrepresented Filipino population. As a bilingual board certified pediatric nurse practitioners of Filipino descent, I have extensive primary care experience in various clinical settings, (e.g. Health Maintenance Organizations, private practices, and hospitals). However, my passion and most of my clinical work has been with community clinics located in low-income communities providing health care services to marginalized racial/ethnic minority groups. I currently advise Master PNP nursing students from diverse backgrounds, and intend to mentor students from diverse underrepresented backgrounds recruited through the planned UCSF SON recruitment and retention program. This program is designed to improve the diversity among students in our UCSF Master and Doctoral nursing programs. In an effort to promote diversity and inclusion in teaching, I lecture on the importance, benefits, and strategies needed to culturally adapt interventions to improve intervention relevancy and health education effectiveness for diverse populations. Moreover, I recently completed the workshop for DIVA facilitator training, honing my skills and knowledge to more effectively promote diversity and inclusion through teaching and mentoring. This coming academic year, I plan to help facilitate one of the DIVA training modules for UCSF students, faculty and staff. Finally, as part of my University and public service, I have participated in multiple student recruitment events to promote student diversity in all UCSF Schools and other UC campuses. My overall public service has centered around both domestic and international outreach programs designed to provide social and health care services for poor and underserved vulnerable populations. I plan to continue my participation with professional and community

TEACHING SUMMARY As a board certified pediatric nurse practitioner, I have experience in both academic and clinical teaching. Most of my academic teaching has been in the Advanced Practice Nurse Practitioner Programs at the University of San Diego and University of California San Francisco, Schools of Nursing. My clinical teaching has been as a preceptor/mentor in San Diego primary care clinics and at the University of California, San Diego Medical Center and the various pediatric primary care practices where I worked. At the University of San Diego School of Nursing, I team-taught a pediatric chronic disease course for pediatric nurse practitioner students. I was involved with preparing the curriculum, syllabus, and assigned lecture for this course. I also conducted topical discussion sessions with the students. At the University of California, San Diego Medical Center, I was a clinical instructor/preceptor in the Department of Family Practice for nurse practitioner students and medical students in the Newborn Nursery. Mentoring included daily newborn nursery rounds, the newborn admission and discharge physical examination, support for maternal-infant bonding, identification of common newborn illnesses and diseases, identification of normal versus abnormal newborn characteristics, and support for maternal breast-feeding. In my primary care clinical practice, I mentored pediatric nurse practitioner students on the skills and roles of the pediatric nurse practitioner. At the UCSF School of Nursing, I have lectured on child development and behaviors, and completed a teaching internship as a clinical instructor in the pediatric assessment skills lab. I have also been an invited guest lecturer on child development and behaviors. As a clinical instructor I have also conducted site evaluation visits for first year PNP/ACPNP students.

For 2016 I was co-FOR for the Winter N231A Symptom Research Proseminar. This Symptom Management Theory seminar for pre-doctoral and post-doctoral nursing students focused on discussions about individual's symptom experience regarding their illness and/or disease process. Issues regarding the individual experiences and responses were presented in lectures by the FOR and presentations by students in regards to their particular research interests. Feedback and evaluations were provided for students papers in relation to their ongoing research projects. In the up coming Fall 2016, plans are teach as co-FOR for the N301.11 Pediatric Health Assessment Skills Lab for the 1st year Masters students in the Pediatric Nurse Practitioner Program. This course includes an additional 4-day summer "boot camp" pediatric skills lab to introduce these PNP students with the pediatric examination procedures and familiarize them with the techniques handling medical equipment typically used during a pediatric physical exam across various age groups. In the Spring 2017, I am scheduled as the FOR for the N241.04 Professional Issues in Advanced Practice Pediatric Nursing. Students provided input that this course was very helpful in supporting their transition from academia to their clinical career as a professional PNP in the community. I plan to incorporate student feedback suggestions to improve this course.

MENTORING SUMMARY “Lead, Empower, Transform” is the motto and framework I would use to guide my efforts in teaching and mentoring nursing students to successfully achieve their advance degrees in nursing. I plan to incorporate skills including observing, careful listening, and culturally sensitivity to enhance the skills each of my mentees will need to successfully navigate the academic system to excel in their clinical, academic and research endeavors. As a mentor, my efforts are to role model my leadership skills, research acumen, clinical nursing knowledge and cultural sensitivity to inspire graduate nursing students to pursue their doctoral degrees, particularly nurses from diverse backgrounds and cultures. As a Filipino nurse faculty in the UCSF School of Nursing, one of my goals as a mentor is to improve the diversity among nursing faculty in academia and research that includes Filipinos. Filipinos represent 20% of all registered nurses in California. Yet, Filipinos are an underrepresented minority among nursing faculty in academia. To improve diversity, I am currently mentoring two Filipina students in the Masters Degree - Advance Practice Nurse Practitioner Program as future leaders in nursing. I am a preceptor for a multidisciplinary team of medical, nursing, and pharmacy students with the UCSF Flu Crew providing free flu shots to walk-in patients at the Bayanihan Community Center. As a preceptor, I perform the following activities: 1) supervise patient assessments (PMHx, vital signs: Temp, pulse, respirations, BP, and general assessment of skin, MS, color, and demeanor), consent for vaccine, risks and benefits of flu shot, previous vaccination hx, and s/s of adverse reactions to flu shot and f/u as needed, 2) review dosage, technique and site administration, and how to maintain sterile technique and good hygiene, 3) discuss types / appropriateness / contraindications of flu vaccines, health education, and follow/up. Currently I am a faculty advisor for two Masters nursing students in the Pediatric Nurse Practitioner Program and unofficial mentor for two pre-doctoral nursing students. All these four students are from racial/ethnic diverse minority groups. I provide academic guidance, feedback their papers and projects, and support during quarterly meetings. Plans for the next 2016/17 academic yeare are to: 1) increase my advisory role with two - three additional Master PNP students and 2) become an official mentor for pre-doctoral students as requested.

RESEARCH AND CREATIVE ACTIVITIES SUMMARY In an effort to reduce health disparities in obesity and related chronic disease, my overall goal is to build a program of research focused on improving lifestyle behaviors (e.g., healthy eating and physical activity) in diverse racial/ethnic populations at high-risk for obesity. My area of research emphasizes the identification and implementation of effective lifestyle intervention strategies to help mitigate or prevent obesity and related chronic diseases such as type 2 diabetes and cardiovascular disease in at-risk underrepresented Asian Americans, particularly Filipinos. I primarily focus on clinical trials of culturally relevant community-based participatory lifestyle interventions incorporating mobile digital technologies (e.g., smartphones, tablets, accelerometers, mHealth apps) and virtual social support (Facebook groups). Mobile technologies serve as an innovative means to deliver scalable, cost effective, and sustainable strategies for lifestyle intervention programs among at-risk populations. Evidence indicates that Asian Americans particularly Filipinos are prolific users of digital technologies. As an experienced bilingual, Filipina nurse researcher and nurse practitioner, I have accumulated extensive knowledge and skills in the areas of leadership, community engagement and project management that are necessary to conduct culturally adapted community-based participatory research incorporating mobile technologies and social media. My Philippine heritage, multi-cultural background, language skills (Spanish and Tagalog), and extensive clinical experience have enabled me to serve racial/ethnic diverse populations. One of my prior community-based participatory research studies was the Vida Saludable, “Living Healthy” feasibility study - a community engaged culturally relevant lifestyle intervention for underserved Latino mothers/child dyads to reduce obesity and related chronic disease risks. Qualitative focus groups and in-depth interviews with community stakeholders informed the development of a culturally tailored intervention. Through my research with diverse racial/ethnic populations, I developed an expertise in culturally adapting interventions, including guidelines and a novel method for scoring and ranking the relative rigor and extent of culturally adapted intervention studies (see publications). Currently, I am Principle Investigator for two research projects: 1) NIH/NIDDK R34 funded pilot study the "Filipinos Fit and Trim Weight Loss for Diabetes Prevention" study. This is a 3-month mobile phone-based pilot RCT weight loss lifestyle intervention followed by a 3-month maintenance phase with an active wait list control for N=67 overweight Filipino Americans at risk for developing type 2 diabetes; and 2) American Heart Association funded pilot study for the "Pilipino Americans Go4Health Weight Loss Program to Prevent Heart Disease" – also a 3-month mobile phone-based pilot RCT weight loss lifestyle intervention followed by a 3-month maintenance phase with an active wait list control for N=45 overweight Filipino Americans with type 2 diabetes on only oral medication (non-insulin dependent). Both studies are culturally adapted lifestyle interventions incorporating digital technology (smartphone, accelerometer, mHealth app) with virtual social networking (Facebook). Filipinos have the highest prevalence of obesity and type 2 diabetes among all Asian subgroups. Beside a genetic predisposition, Filipino culture is characterized by sedentary behaviors and a diet high in fats and sugars. Nonetheless, Filipinos are seriously underrepresented in preventive health research. Based on these health disparities, it is imperative to identify effective intervention strategies to reduce these disease risks and improve outcomes for Filipinos. As one of the few Filipino investigators, my plans are to continue my research with underrepresented vulnerable racial/ethnic populations, specifically with Filipinos who suffer major health disparities exacerbated by a dearth of effective intervention strategies to improve their health outcomes. My two current pilot mobile phone-based RCT weight loss lifestyle

intervention studies lay the foundation for a full scale RO1 RCT to identify effective intervention strategies to prevent or delay the onset of type 2 diabetes and related comorbidities in Filipino Americans. I recently submitted an R01 grant proposal to the NIH, NIDDK for a larger follow-on RCT to test the effectiveness of the Filipinos Fit and Trim mobile phone based lifestyle intervention for overweight Filipinos to reduce their risks for type 2 diabetes. When funded our research team should begin the study start up phase in early 2017. Recent research reported that Filipino children have the highest prevalence of obesity among all major Asian American sub-groups. If children are obese by 3 years of age they are likely to be obese by 5 years of age with an 80% likelilhood of being obese as adolescents and adults. Therefore, early childhood obesity prevention is needed in Filipino children to improved their long term health outcomes given their obesity related chronic disease risks. My plans are to expand my program of research in behavioral modification lifestyle intervention research to include Filipino children and their parents. Plans are to submit two grant proposals in the near future for pilot funding from the UCSF Research Allocation Program and/or the Gordon and Betty Moore Foundation, Patient and Family Engagement Early-Career-Investigator Award- 2016.

Lifestyle and Sleep Intervention for Prediabetes and Metabolic Syndrome

Objective is to increase the efficacy of an existing lifestyle improvement program by adding a sleep extension and sleep hygiene intervention. Improved sleed can improve weight loss through two possible routes, lessening fatigue my potential physical activity (PA), and decreasing appetite may potentiate diet improvements. This study combines a simple, potent sleep intervention with a nationally recognized Group Lifestyle Balance Program (CLB), a comprehensive lifestyle behavior change program focused on diet and PA. The combined program has the potential to significantly improve weight loss in those at risk for diabetes and cardiovascular disease (CVD). This intervention incorporates a mobile health (mHealth) component including a Fitbit accelerometer to track PA steps/day and sleep activity, and the Fitbit app/diary to track calorie intake and weight inputted as self-reported daily foods consumed and weekly weights.

My role as co-investigator will be to oversee the set up of the 1)Fitbit study account with subject’s Fitbit device and associated Fitbit app/diary downloaded to subject’s smartphone, tablet or laptop computer. I will also oversee the Fitbit training of research staff. 2) I will also oversee the set up of the Fitabase account - a software program that virtually uploads and stores the Fitbit data (steps, calories, weight, and sleep) in real time to secure Fitabase database servers. I will oversee the Fitabase training of research staff on setting up the account tied to each subject’s Fitbit account. 2) Finally, I will help develop a subject recruitment strategy and oversee the recruitment process along with train research staff on the recruitment protocol to ensure fidelity.

The Filipinos Fit&Trim (Fit&Trim) is a culturally adapted 8-month mobile phone-based RCT weight loss lifestyle intervention including virtual social networking (Facebook) with an active wait list control design for 180 overweight Filipino Americans. The objective is to test the effectiveness of the community-based Fit&Trim program specifically targeting Filipinos to reduce their obesity and diabetes risks. The aims are to: 1. Evaluate the effectiveness of the Fit&Trim intervention versus an active control on: a) primary outcomes: change in body weight (%) and b) secondary outcomes: change in BMI, waist circumference, fasting plasma glucose, and HgbA1c. 2. Assess if greater digital technology use is associated with greater weight loss. 3. Assess change in adiponectin levels in relation to physical activity engagement.

I am the Principal Investigator for the project, responsible for overseeing the management and administration, recruitment, and implementation of the RCT, as well as monitoring for compliance and safety. Applying Virtual Social Networking to Diabetes Prevention for Filipino Americans: a pilot study for a 3-month RCT with a wait list active control design for 40 overweight Filipino Americans to assess the potential efficacy of the MeetUp-DPP lifestyle and social networking intervention. The primary aim is to obtain preliminary estimates of the effect of MeetUp-DPP on outcomes (including body weight, waist circumference, fasting plasma glucose, and HgbA1c), as well as targeted mediators (physical activity and diet).

My role as PI along with Dr. Fukuoka, Co-PI of this research study will oversee the scientific integrity and fidelity of the project, provide leadership for the research team, conduct the training and orientation for the research staff, monitor and report adverse events, and meet with the staff on a weekly basis to review subject enrollment, recruitment, study implementation, concerns, and follow-up status. I submitted the UCSF CHR application for approval to conduct this study and the study Clinical Trials registration. I also oversee the data collection, management, cleaning, and storage of data on secure encrypted data servers. As co-investigator on the Vida Saludable pilot feasibility lifestyle intervention for Hispanic mother-child dyads, qualitative focus groups and in depth interviews with Latino community stakeholders were conducted to inform the development of a culturally tailored intervention. As the primary author, I wrote all the sections for this article regarding how Vida Saludable was culturally adapted to be relevant for low-income Hispanic mother with 3-to 5- year-old preschool children.
