CONNECTIONS is a free program supporting patients and families facing the last 18 months of life. Patients and families might not be ready for hospice or palliative care, and need a strong partner who understands both health concerns and goals. In Connections, patients and families are supported by social workers and VNA-trained volunteers. Change can be di�cult and confusing. Our Connections Program serves patients and families coping with serious advanced illnesses with dignity and compassion. There is no cost to patients and their families.
