Wait times at Urgent Care Centers are typically shorter thanthose seen in hospital emergency
departments, and the cost to the patient is usually less.

Multidisciplinary Approach“There’saconsciousefforttomaketherightchoices,”Traceysays.Itwasn’teasy–oneweekshe gained five pounds and came to Dr. Russointears.Heexplainedshehadgainedweightduetoconsumingtoomuchfruit(sugars)that week.“Dr.Russohastheworld’sbestbedsidemanner.Hesaid,‘That’sok.Thisishowwelearn.’”
TraceyBoyle-DuFaultcanpinpointthemomentshedecidedtoloseweightandgethealthy.“Itwasmydaughter’ssixthgradegraduation,”saysthe47-year-oldLowerTownshipresident.“IlostmymotherafewyearsagotoALSandsadly,she’smissingalotofgreatmilestonemomentslikethis.IrealizedIwanttobetherewhenmygrandchildrengraduate.”Thenextday,TraceycontactedCapeRegional’sCenterforBariatrics.Tracey,whoweighed325pounds,hadalwaysbeenayo-yodieter.Theextraweightnegativelyaffectedherhealth:shewasprediabeticandtook

“My mood is improved,” she says.“I’m excited to do things and go places.”

“Wefollowupwithourpatientsclosely,”saysDr. Russo, who is Board-certified in general surgery.“Weliketohelpreinforcetheirbehaviorbecauseit’spossibletolapseintooldhabits.”Dr. Russo selected a laparoscopic sleeve gastrectomyfor Tracey which involves making small incisions in theabdomen and performing the procedure using a smallvideo camera and long instruments. A portion of thestomach (which produces Ghrelin, a hormone thatsignals hunger) is removed, leaving a narrow gastrictubeorsleeve.Theprocedurereducesthesizeofthestomach and makes the patient feel full faster. Sinceher August 31 surgery, Tracey has lost 82 pounds (shelost 25 pounds in preparation for surgery, too). But anumber on a scale is not her goal.
Living Life Again“The medicine I needed before surgery has all beenreduced,” she says. “I’m on half a blood pressure pill– my goal is to get off it entirely. I’m getting healthy for

JoshLorewasneartheendofanintense,two-and-a-halfmonthCoastGuardReservetrainingregimeninCharleston,SouthCarolinawhendisasterstruck.Withjustsevendaysleft,hewasinjuredwhilebench-pressing.“Itwaslikeapopandatearatthesametime,”Josh,a33-year-oldfatherofthree,saysofthefreakaccident.Heknewinstantlysomethingwaswrongashewasinexcruciatingpainandnoticedhisupperrightchestareawashangingdownslightly.“Itwasascarytime.IthoughtmycareerwiththeCoastGuardwasover.”Joshwasinbadshapeandrecallshowhecouldn’tdoanythingweightbearingbecauseitwastoopainful,includingapushup“ifhislifedependedonit.”Expert CareJoshreturnedhometoMillvillefortreatmentwherehewasreferredtoKevinMcHale,MD,afellowshiptrainedorthopaedicsportsmedicinesurgeonwithPennOrthopaedicsatCapeRegionalHealthSystem.AfterundergoinganMRI,JoshhadaconsultationwithDr.McHalewhodiagnosedhimwitharightpectoralistendonrupture–whenaportionofthechestmuscleispulledofftheupperarmbone.Dr.McHalepresentedJoshwithnon-surgicalandsurgicaltreatmentoptions.“Whilerehabilitationmaybeanoptioninsomecases,itwasunlikelytoprovideJoshwithhisoptimalleveloffunction.Withoutsurgery,hewouldlikelyexperiencesignificant persistent weakness,”saysDr.McHale.Successful SurgeryBasedonDr.McHale’srecommendation,Joshunderwentasurgicalrepairofthepectoralistendoninwhichsutureanchorswereinsertedintothesitewherethetendonrupturedfromthebone.Sutureswerethenusedtotietheretractedpectoralistendonbackdowntothebonefromwhereitpreviouslyruptured.“ThissurgicalapproachwasthemostreliablewaytooptimizeJosh’sstrengthandgethimbacktoworkassoonaspossible,”saysDr.McHale.

RobertMauger,anexperiencedumpireof41years,didnotseethesoftballcomingathimuntilitwastoolate − striking the inside of his leg.“Iwasinalotofpain,butit’soneofthosethingsyoudon’tlookat,”saysthe67-year-oldSouthDennisresident. “I could feel it start to swell up, but I figureditwouldjustbruiseandthatIwasn’tseriouslyhurt.”Despite being injured, Robert finished umping themen’ssoftballgameandthenumpedtwomoregames. Three days later when he woke up to findthewoundoozing,hewenttoCapeRegionalUrgentCarewherehewastreatedforamildinfection.
Expert CareTwoweekslater,withtheinfectiongone,butthewoundstilloozing,RobertreturnedtoUrgentCareandwasimmediatelyreferredtoCapeRegionalHealthSystem’sCenterforWoundHealing.OnhissecondvisittotheCenterforWoundHealing,StacyCobb,RN,SupervisorattheCenter,knewRobert’sinjurywassevere,soshescheduledanappointmentwithCharlesBirk,DPM,FACFAS,apodiatristaffiliated with the Center for Wound Healing.“Thetraumacausedbytheimpactofthesoftballwassosevereitkilledthetissueinmyleg,”saysRobert.“When they first opened the wound, it was so big,thatyoucould’vedroppedagolfballintoit.”
Wound Healing TreatmentUnderthedirectionofDr.Birk,Robertunderwentwoundhealingtreatmentthroughanegativepressurevacuum(VAC)device,whichappliesavacuumtoasealed dressing to increase blood flow to the area topromotetissuegrowth.HeworetheVACfor42dayswhilereceivingwounddressingtreatmentthreedaysaweek.UponcompletionoftheVACtreatment,Robertunderwentthreeskingrafttreatments.“RoberthadadifferentkindofwoundthatIdon’tnormallysee,”saysDr.Birk.“Ithoughthiswoundwasgoingtotakealongtimetoheal,buthewasaverycompliantpatientwhodidwhathewassupposedtodo,soheprogressedquickly.”
