This manual contains information to write Homeowners Forms HO 00 03 10 00, HO 00 04 10 00 and HO 00 06 10 00 using the forms and endorsements specified in this manual. Covered Perils The following is a general description of coverages provided by the Homeowners policy form. The policy should be consulted for exact conditions.

LIABILITY
Coverage E Personal Liability
Covers payment on behalf of an insured for damages that the insured shall become legally obligated to pay because of bodily injury or property damage arising out of an insured’s premises or personal activities. Animal, Day Care, Trampoline, ATV, Diving Board and Pool Slide Liability Exclusions are standard endorsements to all policies. Applies on an occurrence basis.
Coverage F Medical Payments
To Others
Covers medical expenses incurred by persons, other than the insured, who sustain bodily injury caused by an accident arising out of an insured’s premises or personal activities. Applies on an each person basis.

●ELIGIBILITY / UNDERWRITING GUIDELINES● The following guidelines will assist you in identifying risks which are eligible and those properties we are unable to write in this program. Please note that underwriting reserves the right to determine the acceptability of each individual risk based on the information provided or obtained. These guidelines apply to both new business and renewal business.
• Located on five acres or less. Homes on more than five acres must be submitted for approval prior to
binding.
• Claims: o Prior Weather Related & Catastrophe claims are acceptable. o Prior Fire or Liability claims are ineligible for the programs.
• All Credit Scores are eligible.
• Protection Class 1 – 9 are eligible: o Homes located in Protection Class 9 must be in an approved subdivision with a credible water
supply. They must have continual occupancy (12 month).
General Eligibility
• Dwellings insured on the HO 00 03 form must be insured to at least 100% Replacement Cost Value. Documentation of replacement cost valuation is required on all new business. If replacement cost exceeds market value, refer to underwriting for approval. If a routine home inspection or information from Property Appraiser records reveals a discrepancy in the Coverage A – Dwelling limit of liability, we will change the limit of liability and notify the agent and the insured.
• Dwellings must be built by a licensed contractor.
• Home must be equipped with circuit breakers with minimum 150-amp service.
• Dwellings with stairways of three or more steps or porches or decks more than two feet off the ground or with three or more steps leading to them must be protected with properly installed handrails.
• Dwellings must be protected with smoke detectors and fire extinguishers in close proximity of kitchen, laundry, circuit breaker and sleeping areas.
• Dwellings located in a Special Flood Hazard Area (Zone A or V as designated by the Federal Flood Insurance Plan) must be covered by a federal or private flood insurance policy with matching or greater limits or the maximum allowed. FCIC urges all policyholders to purchase Flood Insurance,
• Dwellings must be located on established, paved roads and must be clearly visible from the road and to at least four neighboring dwellings.
• Risks located on more than one acre: o Must be accessible year round for fire service. o Must have a hydrant or credible water source within 1,000 feet of the dwelling, or responding
fire department must be able to transport at least 5,000 gallons of water. o Must have smoke detectors in close proximity of all cooking, laundry, circuit breakers and
sleeping areas. o Must have individual fire extinguishers for each cooking area and each room containing a
fireplace. o Must have an individual fire extinguisher for each additional floor if the home is more than one
story. o Must have a driveway at least 12 feet wide. o Must have inward opening gates and have an entrance at least two feet wider than the
driveway and be at least 30 feet from the road, (if property is gated). o Must have a key box of a type approved by local fire department if the gate is secured.
• FCIC reserves the right to inspect any risk and will be inspecting a large majority of their insured’s properties. If a discrepancy from information provided in the application is found during the inspection, policy information will be updated and/or corrected which may cause a change in premium. The agent and the insured will be notified of the change.

• Inquiries may be made on applicants as to their individual claims histories utilizing third party information. Undisclosed prior losses may result in cancellation of the policy.
Ineligible Risks
• Properties in a state of disrepair, reflecting a lack of maintenance, or with any prior unrepaired damage, including any unrepaired hurricane or flood damage or unrepaired fire code or building code violations.
• Properties with more than two mortgages or lienholders (does not include post catastrophe loans for repair).
• Dwellings with wood shakes, asbestos siding, EIFS, or Dryvit.
• Vacant or unoccupied dwellings: o Vacant means dwelling lacks the necessary amenities, adequate furnishings or utilities and
services to permit occupancy of the dwelling as a residence. o Unoccupied means the dwelling is not being inhabited as a residence. o A qualified seasonal dwelling is not considered unoccupied.
• Dwellings owned by Limited Liability Companies (LLC), Partnerships, Estates or Associations.
• Homes with aluminum wiring; knob and tube; fuses; Zinsco, Zinsco-Sylvania, Stab-Lok, Challenger or Federal Pacific breaker boxes or an electrical system that does not meet current building code.
• Dwellings with PEX (older than 2006) or polybutylene plumbing.
• Dwellings built partially or entirely over water and properties accessible by boat only.
• Mobile Homes, Modular Homes, Pre-fabricated Homes, Trailer Homes, Kit Homes or Metal Buildings.
• Homes of historical significance, located in historic districts, obsolete, unconventional, do-it-yourself or irreplaceable construction or construction materials, including but not limited to log homes, domes, earth homes, etc.
• Risks located in Protection Class 10.
• Townhouses, rowhouses and duplexes are ineligible unless they meet the townhouse, rowhouse and duplex definition as described on page 19 of this manual.
• Dwellings with excessive or unusual liability exposure (examples include but are not limited to skateboard and bicycle ramps or other attractive nuisances).
• Dwellings located on a farm, ranch, orchard or grove; or where farming activities or ranching operations take place.
• Dwellings located in a recognized brush area or within 1,500 feet of a recognized brush area.
• Dwellings located on a slope of 30 degrees or greater.
• Risks with buried oil tanks or located on landfills, in areas condemned due to condition or in an area scheduled to be condemned for urban renewal or highway construction.
• Dwellings located on or adjacent to a commercial zone or where exposure from an adjacent property significantly increases the risk of loss.
• Homes with burglar bars.
• Risks used primarily for non-habitational purposes or dwellings originally designed or constructed for non-habitational purposes.
• Properties with a second dwelling or residence on the insured premises.
• Any incidental business occupancy other than an office with no client or employee traffic.
• Properties with home day care exposure.
Age of Home FCIC will not require the insured to provide a 4-point inspection prior to binding a policy. However, we will be conducting 4-point inspections on all homes greater than 25 years old. The inspection will be paid for by FCIC. Please refer to the Florida Older Home Guidelines for requirements on roofs, air conditioning and heating systems, electrical systems and plumbing.

A four-point inspection may be required on any dwelling that has not been updated. Dwellings Over 40 years of age with a reduction in premium will be written with a Limited Water Damage Coverage endorsement. Multiply the All Other Perils Premium by a factor of 0.75
Applicant
• An applicant must hold title to the property with a maximum of two mortgages (does not include post catastrophe repair loans).
• Prior insurance, claims history, condition of property, age of property and other risk factors will be evaluated for all applicants.
• Applicants who have ever been canceled or non-renewed for material misrepresentation, insurance fraud or convicted of arson are not eligible.
• Applicants that have previously been rejected, canceled or non-renewed by any company for underwriting reasons must be submitted to underwriting.
Coverage Eligibility Form HO 00 03 – a Homeowners Policy may be issued to provide insurance under:
• Coverage A Dwelling: o Used solely for residential purposes except for permitted incidental office occupancies. o Containing not more than two families and with not more than two boarders or roomers per
family.
• Coverage B Other Structures: o At the same location as the dwelling eligible for insurance under Coverage A. o Not used for business purposes except for permitted incidental office occupancies. o All fences.
• Additional Insureds at Described Location: o When coverage is provided in accordance with Coverage A and B above, the policy may be
endorsed to include the name of an additional insured. This endorsement provides the additional insured with written notification should we cancel or non-renew the policy. There is no charge for this endorsement.
Use Endorsement HO 04 41 10 00 Additional Insured Residence Premises
• Coverage C Personal Property of: o The owner-occupant of a dwelling.
• Coverage D Loss of Use of: o A building eligible for insurance under Coverage A.
Form HO 00 04 – a Tenant Policy may be issued to provide insurance under:
• Coverage C Personal Property of: o The non-owner of a dwelling or an apartment situated in any building.
• Coverage D Loss of Use of: o A building eligible for insurance under Coverage C.
Form HO 00 06 – a Unit-Owners Policy may be issued to provide insurance under:
• Coverage A Dwelling: o The alterations, appliances, fixtures and improvements which are part of the building contained
within the “residence premises”. o Items of real property which pertain exclusively to the “residence premises”. o Property which is your insurance responsibility under a corporation or association of property
owners agreement. o Structures owned solely by you, other than the “residence premises”, at the location of the
“residence premises”.
• Coverage C Personal Property of: o The owner-occupant of a condominium.
• Coverage D Loss of Use of: o A building eligible for insurance under Coverage A or C.

• Coverage E Personal Liability: o Covers payment on behalf of any insured of all sums which the insured shall become legally
obligated to pay as damages because of bodily injury or property damage arising out of an insured’s premises or personal activities.
• Coverage F Medical Payments: o Covers medical expenses incurred by persons, other than the insured, who sustain bodily injury
caused by an accident arising out of an insured’s premises or personal activities.
Elevators
• Risks with elevators are required to provide proof of annual inspection or service agreement contract.
• Documentation should be maintained in agent’s file for future audit.
Foundation
You can bind a policy for the following foundation types:
• Concrete Slab
• Open foundation with or without crawl space less than six feet – Must be completely enclosed
with a material, such as block, brick, siding, or lattice.
• Open foundation with or without crawl space six feet or greater – Must meet the following: o Homes built prior to 1995 are acceptable if the Coverage A is greater than $175,000. o Homes built 1995 or later are acceptable.
• Pilings with 8’-10’ elevations – Homes built 2006 and newer are acceptable. If older than 2006,
please submit to underwriting for approval.
All other types of foundation must be submitted to Underwriting for review.
Heating and Cooling Systems
• Heating and cooling system must be centrally installed.
• Homes utilizing any type of space heaters or portable heaters are not eligible.
• Dwellings with fireplaces or wood burning stoves as the primary source of heat are not eligible. Wood burning stoves must be factory installed.
• Heating must be electric, natural gas or fuel oil only.
• Evaporative cooling systems are not eligible.
Occupancy and Use
• Any dwelling that is occupied by more than four families is ineligible.
• Owner Occupied Seasonal / Secondary Residences must be occupied at least three (3) months but less than nine (9) months in a one-year period.
• Owner Occupied Seasonal / Secondary Residences are not eligible unless: o The dwelling is located in a Secured Community and overseen on a regular basis by either the
insured or a professional management firm; or a reputable neighbor, friend or relative living within 50 miles of the residence, or

o The dwelling is not located in a Secured Community, but is located in a subdivision and is overseen on a regular basis by either the insured or a professional management firm; or a reputable neighbor, friend, or relative living within 50 miles of the residence.
• Dwelling must be used for residential purposes only. Properties where a business is conducted or dwellings located on the property are used for any commercial purpose are not eligible.
• A non-occupant joint owner of the dwelling may be listed as an Additional Insured. Use Endorsement HO 04 41 10 00 Additional Insured – Residence Premises
Pools and Ponds
• Swimming pools and/or ponds must be protected by a self-latching fence at least four feet high.
• Dwellings with empty pools are not eligible.
Prior Insurance
• No lapse in coverage between FCIC coverage and prior insurance, unless the risk is a new purchase or underwriting has given prior approval.
• Agents should retain proof of prior insurance in their files for future underwriting audits.
Prior Loss History
• Prior weather related or catastrophe claims will not be used to non-renew or increase the premium on a policy.
• Risks with any previous fire or liability claim are not eligible.
• Risks with one claim of any other kind in the last three years, excluding catastrophe or weather related claims must be submitted to underwriting for approval.
• Risks with three or more claims reported in last three years are unacceptable.
• The above guidelines will not apply to deny applicant coverage if a claim was caused by an act of domestic violence by another person covered under the applicant’s prior policy if the applicant did not contribute to the loss and cooperates in providing any reasonable additional information which may be requested.
Roof Roof must be in good condition
• Composition shingle roofs must be less than 15 years old – mandatory on New Business.
• Maximum of one overlay of composition shingles (none allowed over wood shingles).
• Tile and metal roofs must be less than 30 years old.
• Metal roofs must have no signs of rusting or curling.
• All roofs must have at least five (5) years of useful life remaining.
• Dwellings with flat roofs, corrugated roofs, tin, rolled tar paper, built up tar and gravel and roofs with wood shingles or any roof with less than five (5) years of useful life are not eligible.
• Flat roofs are not eligible unless they are made of poured, reinforced concrete.

Seasonal / Secondary Residences A dwelling unoccupied by the owner for more than three (3) months during any one (1) year period and not rented to others is considered seasonal or secondary. A minimum All Other Perils deductible of $1,000 is required. Coverage E – Liability is limited to premises liability only. Water damage for Dwelling and Personal Property is excluded for Seasonal / Secondary residences unoccupied for more than 30 days unless the water supply is shut off. Seasonal / Secondary Residences are not eligible unless:
• The dwelling is located in a Secured Community and overseen on a regular basis by the insured or a professional management firm; or a reputable neighbor, friend or relative living within 50 miles of the residence. Name, address and phone number of professional management firm or individual overseeing the property must be maintained in agents’ file for future audit.
OR
• The dwelling is not located in a Secured Community but is overseen on a regular basis by the insured or a professional management firm; or a reputable neighbor, friend or relative living within 50 miles AND has a central reporting burglar-alarm system. Name, address and phone number of professional management firm or individual overseeing the property must be maintained in the agent’s office for future audit.
Dwellings unoccupied more than nine (9) months in any one year period are not eligible. Seasonal / Secondary residences with rental exposure are not permitted in any program.

● BINDING ●
Agent Binding Authority Agents may bind coverage on risks meeting the following criteria without contacting the company:
• Risk must meet Eligibility / Underwriting Guidelines.
• Risks must be within the amount of insurance binding authority limits.
• The completed Internet Homeowners Application must be signed and dated by the insured and the agent.
• Bound risks must be submitted to the company within 72 hours. If not submitted within 72 hours of binding, the company reserves the right to take appropriate action, including non-acceptance of the risk.
• Completed and signed application must be maintained in the agent’s files and is subject to audit review by the company.
• New Business on the HO3 form is not eligible for this program.
Binding Restrictions Agent binding authority automatically terminates for new business and any coverage changes including decreases in deductible when a “Hurricane or Tropical Storm Warning” has been issued by the National Weather Bureau based on the following rules: Hurricane and Tropical Storm Binding Authority:
• Binding is restricted when a tropical storm or hurricane is located west of 72° West Longitude and north of 15° North Latitude and south of 35° North Latitude.
• Agents may not bind coverage for 72 hours after the warning has been lifted or upon notification from FCIC.
• FCIC reserves the right to further restrict binding authority as conditions warrant.
• Any exceptions to the Hurricane and Tropical Storm Binding Authority will be communicated by the company.
Binding information is available at: www.bankersinsurance.com Severe Weather Binding Authority Agent binding authority automatically terminates for new business and coverage changes including decreases in deductibles when a wind or hail storm, tornado, or other severe weather watch or warning has been posted. Authority to bind is reinstated after the storm and threat of damage has passed. Other Binding Authority Restriction FCIC reserves the right to immediately suspend underwriting and binding authority during periods of potential floods, mud slides, brush fires, wildfires and other natural disasters or state and national emergencies. Broker of Record Changes Broker of Record changes must be effective on a renewal effective date. Changes cannot be made midterm. If policy is being non-renewed, it must be re-quoted and meet current eligibility guidelines.
Building Code Effectiveness Grading The Building Code Effectiveness Grading Schedule (BCEGS) develops a grade of one (1) to ten (10) for a community based on the adequacy of its building code and the effectiveness of its enforcement of that code. Policies that include hurricane coverage may be eligible for special rating treatment, subject to the criteria in the following sections:
• Community Grading o The Building Code Effectiveness Grade applies to any building that has an original certificate
of occupancy dated the year of the effective date of the community grading, or later. A rating factor has been developed for each community grade.
o If a community is re-graded subsequent to its initial grading, the factor for the revised grade applies to buildings that have an original certificate of occupancy dated the year of the effective date of the revised grading, or later.
o Where certificates of occupancy are not issued, equivalent documentation acceptable to the company may be used.
o If, due to an addition or alteration, the original building is changed to comply with the latest building code, the factor for the community grading applicable at the time the reconstruction is completed will apply to such building.
• Individual Grading Where buildings have been built in full conformance with the natural hazard mitigation elements of one of the nationally recognized building codes, even though the community grade is greater than one, exception rating procedures may apply
o Any building may be classified as Grade One (1) for Windstorm/Hail upon certification by a registered or licensed design professional based on an on-site inspection that such building is in compliance with one of the three nationally recognized building codes with respect to mitigation of the windstorm or hail hazard. This classification is effective only from the date of the certification.
NOTE: There will be a surcharge for non-participating communities.

Cancellation or Reductions in Limits of Liability or Coverages
● Mandatory coverages may not be canceled unless the entire policy is canceled. ● If insurance is canceled or reduced at the request of the insured or insurer, the additional or return
premium shall be computed on a pro-rata basis. ● The insured may cancel the policy at any time for any reason. We will refund any unearned premium. ● Flat cancellations after the effective date of the policy will require proof of other insurance and must be
made within 60 days of the effective date.
Change Endorsement Changes to the policy may be requested by providing the company with all pertinent coverage and rating information needed to complete the change. Additional premiums for policy changes occurring during the current policy term shall be computed pro-rata of the annual premium using the rates in effect at the time the policy was written or renewed.
Construction Definitions Brick, Stone or Masonry – Exterior walls constructed of masonry materials such as adobe, brick, concrete gypsum block, hollow concrete block, stone tile or similar materials and floors and roof of combustible construction. Brick, Stone or Masonry Veneer – Exterior walls of combustible construction veneered with brick or stone. Frame – Exterior wall of wood or other combustible construction, including wood-iron clad, stucco on wood, plaster on combustible supports, or aluminum or plastic siding over frame. Hardiplank or Hardiboard over Frame Construction – Exterior walls of frame construction with fiber cement siding, hardiplank or hardiboard. Superior Construction – Exterior walls, floors, and roof constructed of masonry or other non-combustible or fire resistive materials. NOTE: Mixed (Masonry/Frame) – A combination of both frame and masonry construction shall be classified and coded as frame when the exterior walls of frame construction (including gables) exceed 33 1/3% of the total exterior wall area; otherwise classify and code as masonry.

Manual Premium Revision A manual premium revision shall be made in accordance with the following procedures:
• The effective date of such revision shall be as announced.
• The revision shall apply to any policy or endorsement in the manner outlined in the announcement of the revision.
• Unless otherwise provided at the time of the announcement of the premium revision, the revision shall not affect in-force policy forms, endorsements or premiums, until the policy is renewed.
Multiple Company Insurance Other insurance is not permitted. We will not provide coverage for a dwelling that is also covered by another insurance policy.
Payment Plans The payment plan applies to the policy premium only. All policy fees are due at policy inception. The selected payment plan must be indicated on the application. All future renewals will be billed by the same plan unless otherwise instructed by insured or agent. Minimum Premium for Two-Pay, Three-Pay and Four-Pay Payment Plans is $400. Annual Payment: Policy is required to be paid in full either with application or annual billing statement. No service fees. Two-Pay Payment Plan: 50% down due at new business One payment of 50% due on the 180th day of the policy Three-Pay Payment Plan: 40% down due at new business Two payments of 30% due in two equal installments at 60 day intervals Four-Pay Payment Plan 25% due at new business Three payments of 25% due in three equal installments at 90 day intervals All installment payments are subject to a $3.00 payment fee. Policy Term All policies must be written for a period of one (1) year and may be extended based upon premiums, forms and endorsements applicable on the effective date of the policy term.
Protection Class Information Protection Class determinations are defined by the Public Protection Classification Manual and apply to all risks insured under the Homeowners Program. Use PC 10 for unclassified areas. The Protection Class indicated applies for risks or municipalities where a single class of fire protection is available. In an area where there are two or more classifications, the classification is determined as follows:

NOTE: Hydrant distance does not apply when the responding fire department is equipped to transport or pump water from an alternative water supply within 1,000 feet of the dwelling. Upon request, the agent shall submit a certification of credible water supply from the responding fire department. Protected Subdivision Rule:
• A dwelling located in a qualifying restricted subdivision will receive the rating of the responding fire department if:
o The subdivision is under development with a recorded plat for 10 or more dwellings; and o Dwellings are built by licensed contractors subject to building restrictions as to type and square
footage.
• The dwelling is clearly visible to at least four other residences.
• The dwelling is located within seven road miles of a responding, recognized Public Protection Class 1-8 rated fire department.
• The dwelling is located on an improved, maintained and year-round passable road.
• The dwelling is not located in a severe brush fire area.
• The home is located within 1,000 feet of a credible year-round water source that can supply a minimum of 5,000 gallons of water.
o A year-round water source may be any of the following: ▪ An operating public fire hydrant, or ▪ A lake, pond, reservoir, or ▪ A tanker/pumper truck supplied by the responding fire department.
• Upon request, the agent shall submit a certification of credible water supply from the responding fire department.
Renewal Plan – Loss Free Discount If the company elects to offer a renewal, a policy may be renewed by the policyholder by paying the renewal premium prior to the expiration of the current policy. A renewal policy will be issued based on the premiums, forms and endorsements in effect at time of renewal. A new declaration page will be issued. A Renewal policies have been loss free in the prior policy year (Catastrophic and Weather Related Losses Excluded) will receive a 10% Loss Free Renewal Discount off the Non-Hurricane Premium. Multiply the NON-HURRICANE BASE PREMIUM by 0.90.

Single Building Definition
• All buildings or sections of buildings that communicate through unprotected openings shall be considered as a single building.
• Buildings that are separated by space shall be considered separate buildings.
• Buildings or sections of buildings which are separated by an 8-inch masonry party wall that pierces or rises to the underside of the roof and pierces or extends to the inner side of the exterior wall shall be considered separate buildings. Communication between buildings with independent walls or through masonry party walls described above shall be protected by at least a Class A Fire Door installed in a masonry wall section.
Supporting Documentation Supporting Documentation must be maintained in the Agent’s file unless noted otherwise. Failure to provide requested documentation for future audits may result in the removal of the credit or application of a surcharge and an invoice to the insured.
• Application – Completed application, signed by insured and agent.
• Multiple Policy Discount – Dec pages for companion policy.
• Protective Devices Discount – Certificate for reporting or central reporting burglar and/or fire alarms and sprinkler systems.
• Prior Insurance – Copy of prior declaration page(s) for prior 12 months.
• Seasonal / Secondary – Name, address and phone number of Professional Management firm or name, address and phone number of reliable friend or neighbor.
• Wind Mitigation Discounts – A Windstorm Mitigation Survey completed and signed by a licensed contractor or other acceptable documentation must be submitted within 15 days of the policy inception date.
Townhouse, Rowhouse and Duplex Definition Each unit must be separated by parapet walls or adequate masonry firewalls. Acceptable units must have a minimum of 1-hour occupancy suppression wall (firewall) which extends from the base floor to the roof sheathing. If there is an attic, the wall must extend through the attic to the roof sheathing. Firewalls must exist between each unit in the townhouse, rowhouse or duplex.
Transfer or Assignment No transfers or assignments are permitted. Each insured and/or property will require submission of a new application.
Waiver of Premium When a policy is first issued or endorsed after the inception date, additional or return premium of less than $5.00 is waived. However, if a policyholder requests the return, we will refund the unearned premium.
Whole Dollar Premium Rule Each premium shown on the policy and endorsements shall be rounded to the nearest whole dollar. A premium of 50 cents or more shall be rounded to the next higher whole dollar.

Hurricane Occurrence:
• Begins at the time a hurricane watch or warning is issued for any part of Florida by the National Hurricane Center of the National Weather Service;
• Continues for the time period during which the hurricane watch or warning conditions exist anywhere in Florida; and
• Ends 72 hours following the termination of the last hurricane watch or warning issued for any part of Florida by the National Hurricane Center of the National Weather Service.
The Calendar Year Hurricane Deductible is calculated anew in each calendar year. If the policy period does not coincide with the calendar year, then a separate Calendar Year Hurricane Deductible will apply to loss that occurs during each calendar year in which the policy is in force. For example, if the policy period is from July 1 of calendar year one (1) to June 30 of calendar year two (2), a separate Calendar Year Hurricane Deductible applies to loss occurring from July 1 to December 31 of calendar year one (1) and to loss occurring from January 1 to June 30 of calendar year two (2).
• The Calendar Year Hurricane Deductible can be exhausted only once during each calendar year.

• The Calendar Year Hurricane Deductible applies to loss to covered property caused by one or more named storms during each calendar year.
Single Hurricane Occurrence During Calendar Year Application: In the event of the first windstorm loss to covered property caused by a single hurricane occurrence during a calendar year, the dollar amount of the Calendar Year Hurricane Deductible is deducted from the total of the loss for all coverages.
Multiple Hurricane Occurrences During Calendar Year: With respect to a windstorm loss caused by the second, and each subsequent, hurricane occurrence during the same calendar year, the company will pay only that part of any loss that exceeds the greater of:
• The remaining dollar amount of the Calendar Year Hurricane Deductible; or
• The deductible that applies to fire that is in effect at the time of the loss.
The remaining dollar amount of the Calendar Year Hurricane Deductible is determined by subtracting all previous windstorm losses caused by named storms during the calendar year from the Calendar Year Hurricane Deductible.
The Calendar Year Hurricane Deductible is NOT available midterm. Deductible options may only be changed at renewal. The Calendar Year Named Storm Deductible applies to the first loss in a calendar year. Any subsequent loss is the greater of the remaining amount of the Calendar Year Hurricane Deductible or the amount that applies to the other perils deductible. Underwriting Tier – All Forms
Credit history is ordered and an Insurance Score is determined on the First Named Insured for all new and renewal business policies. Agent must obtain permission from the applicant prior to requesting the credit report. Tier rating is used to properly rate each risk. Consumer data gathered from the applicant and third party providers determines tier placement. Criteria used for tier placement include insurance score and prior claims. Claims caused by an “Act of God” are not counted towards the number of claims in tier criteria. Non catastrophe claims older than three years old will not be counted against the insured’s tier placement. Credit will be re-run every two years as if the policy were new. Based on the new insurance score, the policyholder will be placed in the tier for which they would have been eligible at the time of new business. Policyholders resulting in No Hit, No Score, or Permission to obtain credit not granted will be placed in the “Other” tier. For New Business it is mandatory that a credit history is ordered.

Credit Guidelines The credit of the First Named Insured will be reviewed in compliance with section 626.9741, F.S. and Florida Rule 69B.125.004.
• Credit history is ordered and an Insurance Score is determined on the First Named Insured only for all new business policies. Agents must obtain permission from the applicant prior to requesting the credit report.
• FCIC does not make adverse underwriting decisions due to credit when the Insurance Score cannot be generated for either a No Hit or lack of data.
• For insureds that have been adversely affected by their credit history, credit will be reviewed, with policyholder approval, once every two years, or at the request of the insured, whichever is sooner.
• The Insurance Score and prior claims history are used to determine the Underwriting Tier (pages 37-39). The credit history will be run at time of new business and every two years thereafter, or at the request of the insured, whichever is sooner.
Limited Water Damage – HO 00 03 & HO 00 06 Only
Dwellings older than 40 years of age are written with the Water Damage Exclusion. For an additional premium, the policy may be endorsed to provide coverage for sudden and accidental discharge or overflow of water or steam from within a plumbing, heating, A/C, automatic sprinkler system or from within a household appliance. Multiple Policy Discount – All Forms
A five percent (5%) discount will apply if the insured has another policy written with a company that is part of Bankers Insurance Group. The policy number must be listed on the application or forwarded to Underwriting. Only one Multiple Policy Discount may be applied. Multiply the NON-HURRICANE BASE PREMIUM by a factor of 0.95
No Prior Insurance/Lapse in Coverage Surcharge – All Forms
All applicants not providing proof of prior insurance will be subject to a 10% surcharge. This surcharge will also be applicable if there has been a lapse in coverage which exceeds thirty (30) days from the effective date of the FCIC renewal policy. The surcharge will remain in effect for one year from the policy effective date to which the surcharge applies. Any lapse or short term coverage during the compliance period will cause the mandatory surcharge period to begin anew. Proof of prior insurance shall be in the form of a copy of a declarations page or other acceptable documentation as determined by FCIC and shall be retained in the agent’s office for a future audit. Proof of Updates – All Systems – HO 00 03 and HO 00 06 Only
A 10% discount will be provided for homes 15 years and older if evidence of acceptable condition and proof of updates is received. A 4-point inspection with accompanying photos or other acceptable evidence must be submitted to the company. If evidence of updates is not submitted at binding, the discount will be removed until proof of updates has been submitted. Multiply the NON-HURRICANE Base Premium by 0.90.
Protective Device(s) Discount – All Forms
Approved and properly maintained installations of burglar alarms, fire alarms and automatic sprinklers in the dwelling may be recognized for a reduced premium developed by applying the appropriate factor(s). Water Loss Prevention Discount – HO 00 03 and HO 00 06 Only
A 5% discount will be provided for dwellings that have had the plumbing certified to have been checked for soundness and to be free of any visible defects. All appliances connected thereto and the attachments between the plumbing system and the appliances must be checked for soundness and lack of defects. This certification requires a licensed plumber to complete the WATER LOSS PREVENTION DISCOUNT CERTIFICATION form. This discount applies automatically to all newly constructed homes completed in the me calendar year as the initial effective date of the policy. For new construction, the requirement for the Water Loss Prevention Discount Certification form is waived. This discount expires after three (3) consecutive policy terms unless a new Water Loss Prevention Discount Certification form certifying the soundness of the plumbing system is provided. Cannot be applied when Proof of Updates - All Systems discount is present on the policy. Multiply the NON-HURRICANE base premium by 0.95
Windstorm Loss Reduction Credit – All Forms
Eligibility When a policy covers the perils of Windstorm, a risk may be eligible for a premium credit if one (1) or more of the windstorm loss reduction features are properly constructed or installed and meet specific guidelines. Windstorm loss reduction construction and devices include:
• Roof Cover
• Roof Deck Attachment
• Roof-Wall Connection
• Opening Protection
• Roof Shape (Can be verified without submitting the verification form)
• Secondary Water Resistance
Windstorm loss reduction credits are available for new and existing construction. If the insured is a rental property and a windstorm mitigation credit is given for Opening Protection, with the protection being Storm Shutter, we require the insured to provide proof the arrangements have been made with another party to close and secure all Storm Shutters if the insured is unable to do so. Proof of Compliance Applicable wind mitigation credits shall apply if proof of compliance is provided by the insured.
New Business – When a mitigation credit feature is selected, a Uniform Mitigation Verification Form (OIR-B1-1802 Rev 2-10) with at least one (1) photo documenting the existence of each visible and accessible construction or mitigation attribute must be submitted to underwriting within 15 days of the policy effective date. The insured is responsible for the expenses associated with verifying the existence of the selected mitigation features. If acceptable documentation is not received within 15 days of the policy effective date, unsubstantiated credits will be removed and premium adjustments will be made. (Homes built to the 2001 Florida Building Code will receive the minimum Wind Premium Credit for New Construction without submitting the verification form).
Existing Business – Credits shall apply after the insured has submitted the Windstorm Mitigation Verification Form with at least one (1) photo documenting the existence of each visible and accessible construction or mitigation attribute. The form is available from the FCIC web site.

If we receive proof of windstorm loss reduction construction or devices during the policy period, and the devices were installed prior to the start of the current policy term and meet the required guidelines, we will apply the premium credit back to the beginning of the current policy term.
If we received proof of windstorm loss reduction construction or devices during the policy period, and the devices were installed during the current policy period and meet the required guidelines, we will pro-rate the applicable premium credit for the current policy period and apply the discount at the next renewal term.

Windstorm or Hail Exclusion Wind, Hail, Tornado, Hurricane and Tropical Storm coverage MUST be excluded if the property is eligible for such coverage from Citizens Property Insurance Corporation. The perils of Windstorm or Hail, including Hurricane and Tropical Storm, may be excluded from the policy as an OPTION if the property is NOT eligible for a wind-only policy from Citizens Property Insurance Corporation. The insured must provide a handwritten statement required by Florida Statute, signed and dated by ALL named insureds and ALL additional insureds. Provides coverage for additional expenses incurred from ordinances or laws which regulate construction, repair or demolition of property, provided loss is from a covered peril and if actual damage and ordinance or law expenses exceed the limit of liability. Increased Replacement Cost on Dwelling – HO 00 03 Only
Dwelling which are insured to 100% replacement value may be endorsed to provide an additional amount of insurance for Coverage A – Dwelling only when loss to the dwelling exceeds the limit of liability shown on the Declarations Page. This endorsement is intended to cover additional costs of construction that are incurred due to increases in the cost of construction that occur during the policy period. Provides coverage for loss caused by hurricane to aluminum framing for screened enclosures, pool cages and carports permanently attached to the dwelling, this endorsement does not provide coverage for screen material or associated with removing or replacing screens. This endorsement does not increase the limit of liability for Coverage A. The maximum limit of liability for this coverage is $50,000. The policy provides loss settlement on an actual cash value basis for certain types of property. For an additional premium, the policy may be endorsed to extend the limit of liability for Coverage C – Personal Property to include Replacement Cost Loss Settlement HO 00 03 Multiply the NON-HURRICANE BASE PREMIUM by and the HURRICANE BASE PREMIUM by 0.10. Sinkhole Coverage and Catastrophic Ground Cover Collapse – HO3 and HO6 Catastrophic Ground Coverage Collapse Florida law requires coverage in all Homeowners Policies for Catastrophic Ground Cover Collapse, which is provided under Endorsement BHO 09.119 Special Provisions - Florida. Catastrophic Ground Cover Collapse provides coverage for direct physical loss or damage to Covered Property caused by or resulting from catastrophic ground cover collapse, meaning geological activity that result in all of the following:
• The abrupt collapse of the ground cover;
• A depression in the ground cover clearly visible to the naked eye;
• Structural damage to the building, including the foundation; and

• The insured structure being condemned and ordered to be vacated by the governmental agency authorized by law to issue such an order for that structure.
Catastrophic Ground Cover Collapse Coverage is automatically included in all FCIC Homeowners policies. Catastrophic Ground Cover Collapse Coverage does not cover sinkhole losses that do not meet the criteria of catastrophic ground cover collapse. Optional coverage for Sinkhole Loss is addressed in the next section. Sinkhole Loss Coverage Florida law requires the insurer to make available coverage for Sinkhole Loss. Sinkhole Loss coverage is optional and is a broader coverage than Catastrophic Ground Cover Collapse Coverage. Sinkhole Loss means loss of or damage to Covered Property when structural damage to a building, including the foundation, is caused by settlement or systematic weakening of the earth supporting the building, only when such settlement or systematic weakening results from movement or raveling of soils, sediments, or rock materials into subterranean voids created by the effect of water on a limestone or similar rock formation. Sinkhole Loss Coverage is provided under Endorsement BHO 09 176 Sinkhole Loss Coverage – Florida – for Form HO 00 03; or BHO 09 191 Sinkhole Loss Coverage – Florida – for Form HO 00 06 10% Deductible on Sinkhole Loss Coverage is mandatory. See Deductible Section of manual (pp. 22-23). Eligibility – New Business (All Counties) Any new application or endorsement request that includes Sinkhole Loss coverage may not be bound and must be submitted for approval if:
• The insured location, including the residence premises other structures, and grounds to be insured by FCIC, has ever experienced a partial Sinkhole Loss or Catastrophic Ground Cover Collapse Loss.
• In the event of such a loss, the applicant/insured must provide the following documentation for appropriate underwriting review to determine if Sinkhole Loss Coverage may be bound:
o The professional engineer’s investigative finding report, and o Scope of completed repairs; and o Proof that the recommended repairs were made, or o Proof that alternate repairs were made.
• If the property HAS BEEN repaired in accordance with the engineering recommendations upon which any payment or policy proceeds were based, First Community Insurance Company may at its own expense retain the services of a professional engineer to certify that:
o The insured location has been repaired; o The insured location has been stabilized; and o There is no evidence of any sinkhole activity or structural damage to the building, including the
foundation, at the insured location.
• If the property has experienced a sinkhole loss and full repairs HAVE NOT been completed in accordance with the professional engineer's investigative findings report and/or certification from the professional engineer HAS NOT been issued to attest that the property to be insured has been repaired, has been stabilized, and there is no evidence of further sinkhole activity, the risk would be DECLINED for sinkhole loss coverage.
• If the property has experienced a sinkhole loss that has resulted in payment of full policy limits, the risk would be DECLINED for sinkhole loss coverage.
• New business risks proximate to sinkhole activity, as determined by a third party vendor’s mapping software that indicates any sinkhole activity within one mile, may be bound only with Catastrophic Ground Cover Collapse. If the policyholder elects the option for Sinkhole Loss Coverage, an approved structural inspection of the property to be insured must be completed prior to adding the coverage. The inspection will document existing damage, evaluate the structural integrity of the property to be insured

and verify that there has been no past or current sinkhole activity at the property to be insured that has not been disclosed.
• An “Approved” inspection service is one that has been designated by us as competent to perform the evaluation and whose report format meets our informational requirements. The insured will contract directly with the “Approved” inspection service and pay an arranged fee. Both parties will receive a copy of the inspection. The fee will not be refundable no matter how the underwriting decision is reached.
• An inspection from an inspection service that has not been designated by us as “Approved” may be submitted by the insured for consideration in meeting this requirement. Such an inspection must have been completed by a professional engineer, professional geologist or other individual or entity recognized by us as possessing the necessary qualifications to properly complete the inspection, and must meet all requirements outlined above with regard to timing, content and format.
Pasco and Hernando – Renewal Business
• First Community Insurance Company will be exercising its option under section 627.706, F.S. to non-renew the policies of current insureds maintaining sinkhole loss coverage in Pasco and Hernando counties. First Community Insurance Company will be issuing renewal offers to these insureds that include catastrophic ground cover collapse but EXCLUDE sinkhole loss coverage.
• When requesting to add Sinkhole Loss Coverage to those policies non-renewed in Pasco and Hernando counties, the insured must comply with the new business requirements listed under rule Eligibility –New Business (All Counties), shown above.
• Subsequent changes to add or remove Sinkhole Loss Coverage in these counties can only be done on the policy’s renewal date.
All Remaining Counties – Renewal Business
• All renewal policies that CURRENTLY maintain sinkhole loss coverage in all remaining counties will renew WITH sinkhole loss coverage unless rejected by the insured.
• When requesting to add Sinkhole Loss Coverage to the policy at renewal, the insured must comply with the new business requirements listed under rule Eligibility – New Business (All Counties), shown above.

Unit Owners Rental To Others – HO 00 06 Only
There is no coverage for theft under Coverage “C” – Personal Property or Section II Liability when the residence premises is regularly rented or held for rental to others. If the property is rented, then the policy must be endorsed to provide such coverage. The Condominium unit must be located in a limited access complex or building with local management or supervision. Theft coverage is limited by the endorsement while the unit is rented to others.

Coverage is available for expenses due to loss caused by identity fraud. Expenses covered by this endorsement include:
• Notarizing cost for affidavits.

Additional Coverage may be added to the Homeowners policy for specifically scheduled items of personal property subject to the following requirements:
• Each item must be individually described including manufacturer, model, serial # (if applicable) and value.
• Covers property of an individual or spouse who reside together, or members of the Insured’s family who reside in the same household.
• Owner occupied dwellings only; no seasonal/secondary homes.
• An appraisal or bill of sale no older than three years must be submitted to underwriting before a request to add a scheduled item on new business or an endorsement request for each single item worth $5,000 or more.
• Any single item exceeding $10,000 or total schedule exceeding $20,000 must be submitted along with dated photos and appraisals to the company, unbound, for prior approval.
• Central burglar alarm must be installed for schedules over $20,000.
• Jewelry items valued over $5,000 must be kept in a vault or safe when not worn.
• Minimum values of $500 per item, pair (jewelry), or set (silverware).
• Golf carts may not be scheduled as Golf Equipment.

Equipment Breakdown
The Equipment Breakdown Enhancement Endorsement provides the inclusion of equipment breakdown as a covered peril under Form BHO 99 500. This endorsement is not available to any other policy forms and can be added to the policy at the request of the policyholder.
The limit of coverage is $100,000 per occurrence. This limit is separate from and does not reduce any other coverage contained within the Section I Property Coverage. This endorsement has a separate deductible of $500. The premium for this endorsement is $50 annually.
