Background Check Instructions
Start by logging into your Affinity account
BACKGROUND CHECK WEBSITE
Click on the blue REGISTRATION button in
the top right
From here you will create a profile or login to
existing account
The next screen shows your family account –
click the continue button to begin the background check
disclosure.
Note – If you do not show yourself under the family account log out and create a profile. If it states you have a profile contact your
club or [email protected] to access your account.
Choose which family member is completing the
disclosure and click “Register as coach/admin”
On the next screen select Play Level, enter
all personal information, and
answer all disclosure questions.
Click Save
Accept all ELA’s(electronic legal
agreement)
Click Agree & Continue
Click CONTINUE to bypass payment screen
NO PAYMENT IS
REQUIRED
The next screen will take you to where you will enter your
social security number
Click SUBMIT BACKGROUND CHECK
YOU ARE NOT DONE!!
This will take you to the background check screen
where you will enter YOUR social security number and
click SUBMIT BGC
JD PALATINE IS THE COMPANY WHO RUNS THE BACKGROUND CHECK. THE
ADMIN IS YOU
Click the green SUBMIT BGC to complete
Once entered you will see that the background
check was submitted and it is currently pending.
Background checks take 2-5 business days to
process.
Congrats you are done!
Email you will receive once completed.
*Please note if you didn’t input your SSN
you will still receive the email.
