Noted Concerns Multiple workers raising concerns that it is

becoming increasingly slippery while walking down the hill.

Workers concerned that traversing up and down the slope with equipment will lead to near miss/incident.

These poor conditions are impacting housekeeping on site with equipment (ie; Cords) being buried under mud increasing the potential for a near miss/incident.

Water Pooling On Site

Difficult to traverse site even in areas with less mud

Page 5: B11 pad hazards

Difficult To Work in These Conditions

Water up to 2 feet deep in some areas

Page 6: B11 pad hazards

Workers would like mats to walk on in order to reduce possibility of near miss/incident

Page 7: B11 pad hazards

Desired Solutions Better egress/access to work area using

stairs. Walkways from stairway to and around

pipeline. Pump out water around pipeline.

Cenovus Safety Handbook Hazard Controls 23.2 First elimination of the hazard and if

elimination is not possible, then controlled by implementing engineering controls or substitution.
