Application Instructions I. Funding Objectives To provide clinically-focused scholarship opportunities for breast cancer specialists from outside the United States to receive advanced training at a leading U.S. breast cancer center affiliated with the Avon Foundation for Women. The Avon Global Scholars will take advanced skills back to their home countries to improve treatment and care of women facing breast cancer.

II. Guidelines on Use of Award The program will bring up to 20 outstanding clinician scholars to Avon Foundation-funded breast cancer centers in the U.S. for a 4-week intensive training/preceptorship for them to learn or become expert at new clinical skills. Due to medical malpractice laws in the U.S., these preceptorships will primarily be observational in nature. The exact work plan for the Scholar’s activities and goals will be developed jointly by the Scholar and Mentor during the course of the application process and beyond.

All Scholars will begin April 27, 2016 (tentative) in the U.S. with introductory course work and dialogue with leading breast cancer experts on April 28-29, 2016.

Scholars will travel to the city of their host institution and attend clinical training from May 2 – June 1, 2016.

Scholars will attend the ASCO Annual Meeting, June 3-7, 2016 in Chicago.

Following ASCO, Scholars will return to their home countries.

December 1, 2016: 6-month progress report due to Avon Foundation.

June 1, 2017: Final progress report due to Avon Foundation.

III. Eligibility Criteria Scholars will be physicians (M.D., D.O., F.A.C.P., or equivalent) working in breast cancer clinical care, who are primarily early to mid-career and who will benefit from a short program of advanced, intensive training at a leading U.S. medical center. Eligible applicants are experienced licensed physicians or other health care professionals of any nation who devote a majority of their professional activity to breast cancer patient care. Ideal candidates will have an interest in improving breast cancer screening, treatment and outcomes in their countries. Scholars must have approval from their home institutions and a commitment from their home institution that they have an employment position upon their return home. Scholars must commit to arriving in the U.S. to begin the program on April 27, 2016, attending the entire Avon program, provide progress reports at 6 months and 1 year and agree to share and disseminate their new skills with other experts upon their return home.

V. Application Details / Selection Process All application materials must be in English and must be submitted using the Avon Foundation’s online application program. Application Selection Process A review panel composed of Avon Foundation Staff, Scientific Advisory Board members and ad-hoc experts as needed will rank applications and select candidates. Attempts will be made to geographically balance scholars. Finalists will be asked to undergo an interview by Avon via phone or skype. Final Selection Process The final selection for Avon Global Scholars awards will be based on a review of the application packages and interviews.

VI. Scholars’ Activities Scholars will be expected to develop a special project or curriculum of activities, in coordination with their assigned host mentor, relating to their interests and desired advanced training. A goal is to have defined skills, defined learning or a defined work-product by the end of the four-week training that can be taken by the scholar back to his/her home institution.

Examples of work products:

Become expert at pathology techniques and quantifying ER/PR or HER2 marker staining

Develop skills in core needle breast biopsy performance, interpretation and patient management

Increase understanding of how to structure and conduct multi-disciplinary review of patient cases and proposed treatment plan

Understand how to develop a system of patient navigation or tracking in areas where barriers exist to delivery of long term follow up clinical care

Increase skills in public health approaches to reducing stigma to breast cancer diagnosis, understanding benefits to cancer screening

VII. Award Notification All applicants will be notified of their selection status in early October 2015 (tentatively October 1, 2015). Applicants selected will have until October 16th to decide whether they will accept or decline the award. In the event a Recipient declines the award, an alternate will be contacted and offered the award. All award decisions are communicated via email and are final.

VIII. Stipend The Avon Global Scholars stipend is not intended to completely cover all travel or daily expenses during the course of the program. For example, the award does not cover medical care, medical insurance, travel insurance or taxes. All incidentals (room service, telephone, laundry, movies, etc.) will be paid for by the recipient. The recipient is financially responsible for all travel, living and related expenses not covered as part of the Global Scholars program described here.

A weekly stipend of $400.00 USD will be provided to assist with living costs while attending the Avon scholarship. The stipend will be calculated based on the number of weeks the Scholar is studying at the host institution.

In addition to the stipend:

Avon will directly cover the costs of economy class air transportation from Scholar’s home country to New York; from New York to the city of the U.S. host institution; from the city of the U.S. host institution to Chicago; and from Chicago back to Scholar’s home country.

o Scholar must arrange travel through Avon’s travel partner.

Avon will arrange and cover the cost of Scholar’s initial ground transportation upon Scholar’s arrival in New York, to travel from the airport to the host hotel. Scholars will be responsible for arranging and paying for all transportation needs other than when they first arrive in New York from their home country.

Avon will arrange and directly pay the costs of room and tax for housing during scholars’ stay, including a hotel room in New York, housing for the four-week training, and hotel in Chicago. Housing is intended for the Scholar only and is not intended to accommodate any other occupants.

It is expected that most scholars will be on a sabbatical leave from their home institution, and therefore will receive salary support from their home institution and be able to maintain any benefits (health insurance, retirement benefits) at their home institution during the time period that they are Avon Global Scholars.

IX. Global Scholar Participation and Reporting Requirements Avon Global Scholars are required to communicate with their mentors prior to departure for the U.S. Recipients are also encouraged to maintain contact with Mentors and fellow participants following their experience. Scholars are required to arrive in the U.S. no later than April 27, 2016 and attend the entire Global Scholar program. Scholars are required to submit a 6-month progress report due to Avon Foundation on or about December 1, 2016.

APPENDIX C: Host Institutions and Mentors Baylor College of Medicine Houston, TX Matthew Ellis, BSc. MB, BChir, PhD, FRCP

Baylor College of Medicine (BCM) was founded in 1900, affiliated with Baylor University in 1903, and became an independent institution in 1969. It was ranked as 1 of the Top 25 medical schools in US News & World Report. Its mission is "To provide state-of-the-art medical care and customer service in an environment dedicated to teaching these principles and discovering new ways to achieve them." BCM is affiliated with 9 hospitals, including Ben Taub General Hospital and its new Smith Clinic of the Harris Health System (HHS). HHS had more than one million outpatient visits in 2011. HHS and BCM have coordinated care for almost 50 years. Dr. Matthew James Ellis is a native of the United Kingdom. He completed his medical degree at Queens’ College & School of Clinical Medicine at the University of Cambridge in England, postgraduate clinical training at the Royal College of Physicians in London and gained a Ph.D. at the Royal Postgraduate Medical School and Imperial Cancer Research Fund at the University of London. After a medical oncology fellowship at the Lombardi Cancer Center, Georgetown University, Washington DC, he was an Assistant Professor there until

moving to Duke University in 2000 and subsequently to Washington University in St Louis where he served as professor of medicine and section head of breast oncology until 2014. Ellis was recently recruited to Baylor College of Medicine to serve as the Director of the Lester and Sue Smith Breast Center and to hold the C. Kent Osborne Chair of Breast Oncology. Both the McNair Foundation and the Cancer Prevention Research Institute of Texas recently awarded him scholarships. He has been instrumental in developing a Genome Atlas and Therapeutic Road Map for estrogen receptor positive breast cancer by applying genomic techniques to samples accrued through a series of neoadjuvant endocrine therapy trials. Most recently, he has found that metastatic breast tumors harbor mutations and translocations in the estrogen receptor gene that render the tumor resistant to therapies used to block estrogen receptor function. He also pioneered research into the clinical relevance of activating mutations in HER2 and in the deployment of patient-derived xenografts for the pharmacological annotation of breast cancer genomes. He is currently Co-Chair of the translational medicine committee for the NRG cooperative group, co-leader for The Cancer Genome Atlas (TCGA) Breast Project and also serves as a Co-PI for the Clinical Proteomic Tumor Analysis Consortium that endeavors to translate TCGA genomic discoveries into protein-based biomarkers with clinical utility. Dr. Ellis has a successful track record in international clinical and translational research, with recent trainees from Brazil, Chile, Poland and Turkey.

Boston Medical Center Boston, MA Tracy Battaglia, MD, MPH

Boston Medical Center (BMC) is a 496-bed academic medical center located in Boston’s historic South End. BMC provides a full range of pediatric and adult care services, from primary to family medicine to advanced specialty care. Emphasizing community-based care, BMC is committed to providing consistently excellent and accessible health services to all—and is the largest safety-net hospital in New England. BMC’s Breast Health Program incorporates a coordinated multi-specialty approach to care, linking internal medicine, surgical oncology, radiology/mammography, medical oncology, radiation oncology, pathology and other breast health specialists together as one connected team. This patient-centered approach results in rapid access, personalized care and streamlined patient visits. All new patients are given expedited appointments in the Belkin Breast Health Center, where they are seen by either a medical or surgical breast specialist. A cancer care nurse is available to all breast cancer patients as a central contact to offer support and ensure compliance throughout their multispecialty cancer care. They are conveniently located next door to the Belkin Breast Imaging Center if further diagnostic evaluation is needed. An essential part of the program is a weekly multidisciplinary breast cancer conference in which the entire team collaborates on a comprehensive evaluation, and plans the best, most individualized treatment regimen and follow-up care for patients with breast cancer. Dr. Tracy Battaglia is a graduate of Boston University School of Medicine and completed her residency at the Beth Israel Deaconess Medical Center. Additionally, she holds a Masters of Public Health from the Boston University School of Public Health. Dr. Battaglia’s interests include breast and cervical cancer screening in minority and underserved women, reducing errors in follow-up to abnormal mammography, as well as risk perception and screening behaviors in multi-ethnic populations. She is Associate Program Director of the Boston University Preventive Medicine Residency Program and Co-Director of the Avon Breast Health Initiative at Boston Medical Center. Additionally, Dr. Battaglia is a former recipient of Career Development Awards from both NIH and the American Cancer Society and now serves as a Principal Investigator on the Patient Navigation Research Program, a community-based cancer control project supported by the Center to Reduce Cancer Health Disparities at the National Cancer Institute.

Armando E. Giuliano, MD, is Executive Vice Chair of Surgery for Surgical Oncology, Co-director of the Saul and

Joyce Brandman Breast Center - A Project of Women's Guild and Associate Director for Surgical Oncology at

the Samuel Oschin Comprehensive Cancer Institute. Additionally he is the Linda and Jim Lippman Chair in

Surgical Oncology and serves as Program Director for both the Complex Surgical Oncology Fellowship and

Breast Oncology Fellowship. He is a recognized authority in breast cancer, thyroid cancer, and parathyroid

disease.

After graduating from Fordham University, Dr. Giuliano earned his medical degree from the University of

Chicago. He trained in surgery at the University of California, San Francisco and surgical oncology at the

University of California, Los Angeles.

Dr. Giuliano has served on the editorial boards of prestigious peer-reviewed journals, including the Annals of

Surgical Oncology, Journal of the American College of Surgeons and Breast Cancer Online. A prolific

researcher, he is the author of more than 450 scientific papers and has written book chapters and a textbook.

Dr. Giuliano has earned numerous teaching awards as well as the Hadassah Medical Organization

Outstanding Lifetime Achievement Award in Oncology, the Fellowship of the Royal College of Surgeons in

Edinburgh ad hominem, the National Summit on Breast Cancer Outstanding Medical Advances in Breast

Cancer Award and the Umberto Veronesi Award for international recognition of contributions to the field of

breast cancer and the introduction of sentinel node biopsy for the treatment of breast cancer. In 2011, he

was one of only three physicians to receive the national clinical Excellence Award from Castle Connolly.

Recently he was awarded the Susan G. Komen for the Cure® Brinker Award for Scientific Distinction in Clinical

Research, their highest award for clinical research.

In addition to serving as Chairman of the American College of Surgeons Oncology Group Breast Organ Site

Research Committee, Dr. Giuliano served as Chairman of the Breast Fellowship Program Directors Committee

for the Society of Surgical Oncology. He recently completed his term as President of the International

Sentinel Node Society and currently serves as Secretary for the Society of Surgical Oncology.

Dr. Giuliano's work on nodal metastases has changed the treatment of breast cancer worldwide.

In 2005, Dr. Sheryl Gabram was recruited by Dr. Otis Brawley to lead the AVON Comprehensive Breast Center where she became the principal investigator on the AVON Grant in 2009. In September 2009, Dr. Gabram appointed to Deputy Director of Cancer Center for Excellence at Grady. Then, in October 2011 she was named Surgeon-in-Chief at Grady Memorial Hospital. Dr. Gabram current research focuses on decreasing disparities for breast cancer patients. She serves as the medical liaison for the AVON community outreach and navigation program and is responsible for clinical access to high quality care for patients seen in the AVON Comprehensive Breast Center at Grady. From a predictive health perspective, she collaborates with researchers from Georgia Tech on the examination of breath biomarkers in exhaled specimens as a potential tool for breast cancer disease detection and monitoring. To date Dr. Gabram has authored over 65 peer reviewed journal publications. Dr. Gabram is the recipient of numerous awards and honors. She has been named by the patient resource guide Castle Connolly as one of America’s Top Doctors for seven consecutive years and American’s Top Cancer Doctors for five consecutive years. She has received the Arthur G. Michel, MD Award for Excellence in Breast Care from the Y-ME National Breast Cancer Organization and was selected as one of Atlanta Magazine’s Top Doctors in 2007 and 2009. In the November 2008 Women’s Health magazine she was named one of 5 top oncologists for women in the South. Dr. Jenny C. Chang is the Emily Herrmann Chair in Cancer Research and Director of the Cancer Center at Houston Methodist Hospital in Houston, Texas. She obtained her medical degree at Cambridge University in England, and then completed fellowship training in medical oncology at the Royal Marsden Hospital/Institute for Cancer Research in the United Kingdom. She was also awarded a research doctorate from the University of London. There, she developed an interest in breast cancer, particularly in the area of prognostic and predictive markers. She used the clinical situation of locally advanced breast cancer, in which patients are traditionally treated with pre-operative therapy, to assess the use of such markers in predicting treatment response. Her recent work has focused on the intrinsic therapy resistance of cancer stem cells, which has lead to several publications and international presentations. In addition, she has been awarded several federal grants to evaluate novel biologic agents, and holds patents on new technologic advances including high throughput molecular profiling.

His lab utilizes sophisticated genetic models to identify, validate and develop targeted therapies for breast cancer. Currently the lab is focused on the PI3 Kinase/AKT signaling pathway, based on the lab's initial finding that the PIK3CA gene is mutated at high frequency in human breast cancers. Dr. Park remains actively involved with all aspects of biomedical research training including teaching of graduate and medical students, post-doctoral fellows, interns, residents and clinical fellows. In addition, Dr. Park continues clinical duties including attending on the inpatient oncology service and seeing breast cancer patients in the outpatient setting. Massachusetts General Hospital Boston, MA Paul Goss, MD, PhD

Dr. Goss was educated at the University of Witwatersrand in Johannesburg, South Africa before pursuing postgraduate training in oncology and a doctorate in hormonal mechanisms of breast cancer at the University of London. He subsequently became Professor of Medicine at the University of Toronto and Director of the Breast Cancer Research Program at the Princess Margaret Hospital before joining Massachusetts General Hospital in September 2004. Dr. Goss' principal research interest has been to explore the pivotal role of estrogen in the pathogenesis of breast cancer. He chairs a multitude of international clinical trials. His recently completed MA17 study of letrozole after tamoxifen led to FDA approval and changed the practice of oncology worldwide. His translational laboratory program focuses on tumor signatures and mechanisms of endocrine resistance. Dr. Goss has published over 200 manuscripts, sits on numerous international scientific committees and serves as ad hoc reviewer for many scientific and medical journals.

New York-Presbyterian Hospital, based in New York City, is the nation's largest not-for-profit, non-sectarian hospital, with 2,409 beds. These collaborations are important. They allow patients to receive comprehensive care and have access to emerging treatments, typically available in clinical trials. Dr. Dawn Hershman completed her medical degree at The Albert Einstein College of Medicine where she was Alpha Omega Alpha, and completed her internal medicine and oncology fellowship training at Columbia University Medical Center, where she served as Chief Resident. During that time she completed a Masters degree in Biostatistics at the Mailman School of Public Health. She was appointed to a faculty position at Columbia University in 2001 and since that time has developed an expertise in the areas of breast cancer treatment, prevention, survivorship, late-effects of cancer therapy, health outcomes and health disparities research. She has published over 150 scientific articles and has received numerous awards including the Eqig Award for Teaching Excellence, the Avon Foundation Medical Advancement Award and the American Society of Clinical Oncology Advanced Clinical Research Award. Northwestern University Chicago, IL Seema Khan, MD

cutting-edge of breast cancer research in assessing the association between nipple fluid hormones, gene methylation levels and risk of breast malignancy, Khan also conducts clinical trials of cancer chemoprevention agents. She is Northwestern's principal investigator for the breast cancer trials of the National Surgical Adjuvant Breast and Bowel Project and the American College of Surgeons Oncology Group. She chairs a National Cancer Institute Division of Cancer Prevention protocol investigating the effects of a soy supplement on the breast epithelium of women at high risk of breast cancer. She is co-investigator on a study of the role of alpha-B crystallin expression in breast cancer biology and was principal investigator on a recent study of ductal lavage biomarkers in high-risk women. Khan is an associate member of the Early Detection Research Network and has served on the National Consortium of Breast Centers board of directors and the endocrinology review panel of the Department of Defense. She has also served on peer-review panels of the National Cancer Institute, the Avon Foundation, and the Susan G. Komen Foundation. She is a founding member of the board of directors of the International Society of Cancer Risk Assessment and Management and is a member of the breast cancer screening panel of the National Comprehensive Cancer Center Network. She belongs to many professional societies, including the American College of Surgeons and the Society for the Study of Breast Disease. Khan has published more than 70 peer-reviewed articles as well as six book chapters and 90 abstracts. She is on the program committee of the American Society of Clinical Oncology and will serve as track leader for breast cancer for its 2009 annual meeting. She is a member of the editorial board of the Journal of Clinical Oncology and serves as a reviewer for many professional journals.
