Attachment B: WZTC QA Evaluation User Guide
The purpose of the WZTC QA Evaluation form user guide is to provide the evaluator with and overview of the WZTC QA Evaluation form.
WZTC QA Evaluation Form
• WZTC QA Evaluation (M-7002) updated 1/2017
• Applicable to any Temporary Traffic Control (TTC) Operations– Department– Construction– Permit– Utility– Municipal
• Applicable to any work duration – Short-Term Work– Long-Term Work – Mobile Work
WZTC QA Evaluation Form
• New form is designed to be objective
• Questions use verbiage to give the evaluator a clear choice and remove areas of interpretation
• Weighted scoring system
• Pass/Fail is 85%– On previous maintenance form 4.25=85%
• Auto-calculated scoring

• Scoring sections– Flagging (25%)– Signing (20%)– Channelizing Devices (20%)– PCMS/Arrow Board (10%)– Vehicles/Protection (10%)– Pedestrians (5%)– Markings/Delineation (5%)– Act 229 (5%)
Scoring Summary
• Each section has a weighted factor determined by level of importance
• Total Weighted Factor is adjusted by number of sections that are scored on the form– See next page for example
• Overall Scoring– Satisfactory: 85%-100%– Unsatisfactory: 0%-84%

– Pink is the score given and score possible– Yellow is the weighted factor– Blue is the weighted score– Green is the calculated total score
QA Form: Questions With Automatic Unsatisfactory (AU) Rating
• Overall, 13 questions on the QA Form have an Automatic Unsatisfactory (AU) rating designation– Operational Requirements
• 4 questions– Flagging
• 6 questions– Vehicles/Worksite Protection
• 3 questions
• The high level safety questions are designated with an AU rating

• This section gives the WZTC Evaluator additional information to conduct the WZTC QA Evaluation
QA Form: Operational Requirements Section
• Consists of four basic Yes/No questions– These questions do not count towards the overall score
but will result in an AU if answered ‘No’

Question 1:Is there a TTC Plan present on the operations?
Answer:A Custom TTC Plan, Publication 213 (or applicable pages from Publication 213), or Manual on Uniform Traffic Control Devices (MUTCD)(or applicable pages from MUTCD).Note: Can be hard copy or electronic

Question 2:Is the appropriate setup being utilized?
Answer:This question is not asking if the correct PATA or TA is being utilized. It is asking in the correct setup is being utilized given the location and type of work.• Example: A crew working on an interstate should
not be using a conventional highway PATA or TA.

Question 3:Does the Custom TTC Plan and all modifications to the Custom TTC Plan have approval from the Department's District Traffic Unit?
Answer:All customs plans and modifications must have DTU approval.Note: Signature is not required for approval

Answer:Devices must be found in Publication 35 (Bulletin 15) or approved by some other source (e.g. FHWA)

Section I: Flagging
• Weighted Factor 25%
• 10 Yes/No questions worth 50 points– Scoring example for Yes/No questions on next page
• 6 of 10 questions have an AU Rating Designation

• Answer:– Yes (5): Enter 5/5 in boxes to the right– No (AU): Enter 0/5 in boxes to the right (AU = 0)
• Check AU box when answering ‘No’ on AU questions– Provide comments regardless of answer– Check N/A if question does not apply
• All other Yes/No questions follow same procedure
Section II: Signs
• Weighted Factor 20%
• 12 Percentage questions worth 60 points total– Scoring example for percentage questions on next page
• 0 of 12 questions have an AU Rating Designation

• Answer:– If five signs are present in the field and six signs are
required as per the TTC plan, scoring is as follows:• 5(signs present)÷6(signs required)=83.3% Score = 3
– Enter 3/5 in boxes to the right– Provide comments regardless of answer– Check N/A if question does not apply

Section III: Channelizing Devices
• Weighted Factor 20%
• 12 Percentage questions worth 60 points– Scoring example for percentage questions on next page

• Answer:– If the TTC plan requires 2 tapers with 6 devices per taper
there would need to be 12 devices set up in the field. However, if only 11 devices are present in the field, scoring is as follows:
• 11(devices present)÷12(device required)=91.7% Score = 4– Enter 4/5 in boxes to the right– Provide comments regardless of answer– Check N/A if question does not apply
• All other percentage questions follow the same procedure

• Weighted Factor 10%
• 12 Yes/No questions worth 60 points
• 0 of 12 questions have an AU Rating Designation
Section V: Vehicles/Worksite Protections
• Weighted Factor 10%
• 7 Yes/No questions worth 35 points
• 3 of 7 questions have an AU Rating Designation
Section VI: Pedestrians
• Weighted Factor 5%

• 0 of 2 questions have an AU Rating Designation
