• Medical Conditions – is it temporary
• Applicant awaiting surgery
• Will this affect their mobility for at least 12 months• Is there sufficient information available regarding the
equipment a child needs

Clinic
• Occupational Therapists cover this on a rota basis. Clinic held one morning a week
• Occupational Therapist has the application forms in advance and access to all information. Discusses functional difficulties.
• 2 recent situations where the decision was reached not to issue a badge
Applicant had back pain but little functional difficulty
Referred to PT pain clinic and applicant will be reviewed again
Applicant stated she had eye conditions
Resource worker for Visually Impaired available and able to give practical assistance
