PP RR EE FF AA CC EE In the pursuit of its mission and vision, the AFP Medical Center
continues to evolve and develop as the premier health service facility of
the Armed Forces of the Philippines. It exists amidst thriving
government and private health service systems that provide the
benchmark in health care delivery. Through the years, it has proven to
be capable and competitive in catering to the health needs of its patients
and the entire Armed Forces. Its services are availed of primarily by
military personnel, their dependents to include authorized civilians.
AFP Medical Center as a tertiary health care facility adheres to the
standards set by regulatory and accrediting agencies - the Department
of Health, the Philippine Health Insurance Corporation, the Philippine
Hospital Association, various Specialty and Sub-Specialty Society
Boards. The Services rendered by our healthcare personnel are
governed and guided by policies; conforming with standards, rules &
regulations set by the AFP as well as those of the Department of Health
and the Civil Service Commission.
This primer provides information about procedures in the delivery
of initial services rendered by the hospital. It serves as an easy guide to
personnel and patients who would avail of such services. It also
indicates responsible persons and offices specific to the activity or type
of service. This will provide built-in mechanisms to avoid red tape and
abuse of authority and privilege.

(b) Active members of the Citizens Armed Forces Geographical Units (CAFGU);
(c) Probationary Officers of the AFP Cadets (PMA, OCS, PAF Flying School: ROTC Cadets/Cadettes on Summer Camp Training), Candidate Soldiers while undergoing military training and Reservist on Annual Active Duty Training (AADT)/Assembly or Mobilization Test;
(d) Persons in military custody or confinement; (e) Informers, Guides and Civilian Volunteers while actually
working with AFP; (f) Civilian Employees of the Office of the Secretary of National
Defense and AFP; (g) Surviving Spouse and their Direct Dependents; (h) Veterans as determined by the Secretary of National Defense; (i) PNP personnel who sustained injuries while supporting AFP
personnel during Counter-Insurgency (COIN) Operations; (j) Other Civilian Patients whose treatment or admission in AFP
Medical and Dental Facilities/Clinics are authorized or directed by the Commander-in-Chief, AFP, Secretary of National Defense, or the Chief of Staff, AFP and patients whose illness are exceptional educational value to the AFP Medical/Dental profession as determined by The Surgeon General, AFP / The Chief Dental Service, AFP.

Civilian Employees – GHQ ID
Authorized Civilian – Approved Authority Form Duly signed by guarantor and the Dep Cmdr for Prof’l Svc, AFPMC authenticated by the Adjutant, AFPMC
Official Receipt (Deposit)
Duration: 30 minutes for simple request 1 hour and 30 minutes for reports which need to be referred to other
agencies
