report, Town of Yucca Valley, and the District' s specifications. Task No. 2 - Materials Sampling

Sample representative material on- site and transport them to our laboratory

for testing. Results will be emailed to the Construction Manager and the

forwarded to the Town' s Inspector. Test results will include statio0n, date, time,

and testing data at a minimumTask No. 3 - Geotechnical Support

Provide geotechnical support at the request of the Construction Manager on

an " as -needed" basis.

Rask No. 4 - Final Compaction and Analysis Report

Prepare and submit a Final " Soils and Materials Testing Report" with

documentation of all reports and analysis results created under the quality

assurance monitoring during the construction of the project. The submittal will

rnncict of nne harm copy and one electronic Pnp file with the nnrrespnndingexcel file.

AGI is knowledgeable, experienced, and capable of providing responsive services to theDistrict. Please feel free to contact the undersigned, who will be the single person for

Part 1 - Insurance Requirements

An example Certificate of Insurance ( COI) is presented in the Attachments as instructed

by the PSA and meet the minimum levels of coverage. A detailed and project specific COIwill be provided if selected. This insurance will be maintained for the duration of this andfuture projects.

Part 2 - Professional Services Agreement ( PSA) Comments

AGI does not express any exceptions or conditions to the form of the District' s PSA.

Part 3 - Exhibit " B" Soils and Materials Testing Fee Schedule

A completed copy of the " Soils and Materials Testing Fee Schedule" is presented in theAttachments, entitled Exhibit " B".

Part 4 - Statement of Offer and Signature

This proposal was signed by an individual authorized to sign contracts, signature shownon prior page. Proposal pricing as presented for the requested scope of work shall remainfirm for a minimum of 90 days.

Considerations

1. This is a prevailing wage project. 2. work hours shall be estimated using a five ( S) day work week while assuming a rate of 400- 800 linear feet of pipeline

per day with applicable appurtenance installation. The typical rate of collection system pipeline installation is 200- 400

feet per day depending upon the depth of pipe and the contractor will be utilizing two construction headings toprovide the 400- 800 linear feet listed above. Depth of pipe should be considered when determining hours worked.

3. Overtime inspections performed in excess of 8 hours per day shall be paid at 1. 5 times the employee' s current rate. 4. The District will not pay for travel time, per diem, or mileage charges independent of the charges listed above. S. For Support Staff, Project Manager, Principal Engineer, and Final Geotechnical Report, Respondent shall complete the

number of hours as required and/ or lump sum rate as provided.
