Spotlight:Patient Safety, Care Coordination &Cutting-Edge UpdatesPROGRAM INTRODUCTION

The 41st Annual CALTCM meeting shines a Spotlight on Patient Safety, Care Coordination, and Cutting-Edge Updates. Attendees will be challenged to !nd ways to integrate advances in medical and post-acute care practices into current care systems, while preparing for the transition to a more e"cient and streamlined Post-Acute and Long-Term Care (PA/LTC) delivery system. We have designed our meeting for practical training of essential members of the interdisciplinary team, in areas where PA/LTC has struggled to improve quality.

The !rst half-day workshop, Cutting-Edge Updates, is presented in a format that enables physicians and other interdisciplinary team members to keep up with the ever-expanding medical literature. Our Keynote Speaker, Joseph Ouslander, MD, sets the stage for an invigorating and interactive second half-day workshop on Care Coordination. Patient Safety is an ever-present issue in PA/LTC, and constitutes the !nal half-day workshop, with a lively and informative Mock Trial presented as the grand !nale.

TARGET AUDIENCE

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation specialists, social services designees, and other members of the interdisciplinary team who care for older adults in the PA/LTC setting.

LEARNING OBJECTIVES

By participation in the annual meeting, participants will have the ability to:

1. Explain models and incentives for improving care coordination, and take appropriate steps to improve care integration with their partners;

2. Develop at least one QAPI Performance Improvement Project (PIP) for implementation in the coming year;

3. Understand how to e#ectively integrate recent advances in medical knowledge into their practice within the post-acute continuum;

4. Identify and implement two new tools to improve transitions of care and care coordination;

5. Identify four preventive steps a facility can take to improve care and thereby reduce its exposure to regulatory or civil actions.

PRE$CONFERENCE: TEAM$BASED INTERPROFESSIONAL COMPETENCY TRAINING IN DEMENTIA SCREENING & MANAGEMENT

The California Geriatric Education Center (CGEC)* presents the 2015 CALTCM Pre-Conference: Team-Based Interprofessional Competency Training in Dementia Screening and Management

BackgroundThe Institute of Medicine (IOM) 2008 Report, “Retooling for an Aging America: Building the Health Care Workforce”, identi!ed signi!cant gaps in training for all health care professions to deliver safe and high quality patient care to an expanding older population. In order to address this gap and thus improve safety and quality of care, specialty-speci!c competencies in care for older adults need to be developed, taught and assessed. A challenge facing gerontologic educators is how to ensure that geriatrics competencies are being taught and assessed in their respective professions.To address the gaps in health workforce training in dementia identi!ed by the IOM, health practitioners need to learn and demonstrate the ability to evaluate older patients for dementia and address their needs within their respective scopes of practice. Healthcare practitioners who directly deliver care need to additionally demonstrate competency in formulating a treatment plan for patients with abnormal cognitive function with special attention to patient safety.

This workshop is divided into 3 activities:1. A brief introduction/overview of the workshop, including a comparison of minimum

competencies for di#erent health disciplines;2. Rotation through competency assessment stations using case study simulations using

actors, and;3. A summary and ‘action plan’ to promote competency in cognitive and behavioral disorders

at the learners’ home institutions.

PRE$CONFERENCE LEARNING OBJECTIVESAt the completion of this training participants will be able to: 1. Demonstrate competency in cognitive and behavioral health assessment through

administration of validated screening or assessment tools.2. Formulate a di#erential diagnosis and work with an interprofessional team to recommend an

evaluation and treatment plan for an adult with a cognitive/behavioral problem.3. Identify caregiver stress and appropriate community support and services for patients with

dementia.

Spotlight:Patient Safety, Care Coordination &Cutting-Edge UpdatesPROGRAM INTRODUCTION

The 41st Annual CALTCM meeting shines a Spotlight on Patient Safety, Care Coordination, and Cutting-Edge Updates. Attendees will be challenged to !nd ways to integrate advances in medical and post-acute care practices into current care systems, while preparing for the transition to a more e"cient and streamlined Post-Acute and Long-Term Care (PA/LTC) delivery system. We have designed our meeting for practical training of essential members of the interdisciplinary team, in areas where PA/LTC has struggled to improve quality.

The !rst half-day workshop, Cutting-Edge Updates, is presented in a format that enables physicians and other interdisciplinary team members to keep up with the ever-expanding medical literature. Our Keynote Speaker, Joseph Ouslander, MD, sets the stage for an invigorating and interactive second half-day workshop on Care Coordination. Patient Safety is an ever-present issue in PA/LTC, and constitutes the !nal half-day workshop, with a lively and informative Mock Trial presented as the grand !nale.

TARGET AUDIENCE

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation specialists, social services designees, and other members of the interdisciplinary team who care for older adults in the PA/LTC setting.

LEARNING OBJECTIVES

By participation in the annual meeting, participants will have the ability to:

1. Explain models and incentives for improving care coordination, and take appropriate steps to improve care integration with their partners;

2. Develop at least one QAPI Performance Improvement Project (PIP) for implementation in the coming year;

3. Understand how to e#ectively integrate recent advances in medical knowledge into their practice within the post-acute continuum;

4. Identify and implement two new tools to improve transitions of care and care coordination;

5. Identify four preventive steps a facility can take to improve care and thereby reduce its exposure to regulatory or civil actions.

PRE$CONFERENCE: TEAM$BASED INTERPROFESSIONAL COMPETENCY TRAINING IN DEMENTIA SCREENING & MANAGEMENT

The California Geriatric Education Center (CGEC)* presents the 2015 CALTCM Pre-Conference: Team-Based Interprofessional Competency Training in Dementia Screening and Management

BackgroundThe Institute of Medicine (IOM) 2008 Report, “Retooling for an Aging America: Building the Health Care Workforce”, identi!ed signi!cant gaps in training for all health care professions to deliver safe and high quality patient care to an expanding older population. In order to address this gap and thus improve safety and quality of care, specialty-speci!c competencies in care for older adults need to be developed, taught and assessed. A challenge facing gerontologic educators is how to ensure that geriatrics competencies are being taught and assessed in their respective professions.To address the gaps in health workforce training in dementia identi!ed by the IOM, health practitioners need to learn and demonstrate the ability to evaluate older patients for dementia and address their needs within their respective scopes of practice. Healthcare practitioners who directly deliver care need to additionally demonstrate competency in formulating a treatment plan for patients with abnormal cognitive function with special attention to patient safety.

This workshop is divided into 3 activities:1. A brief introduction/overview of the workshop, including a comparison of minimum

competencies for di#erent health disciplines;2. Rotation through competency assessment stations using case study simulations using

actors, and;3. A summary and ‘action plan’ to promote competency in cognitive and behavioral disorders

at the learners’ home institutions.

PRE$CONFERENCE LEARNING OBJECTIVESAt the completion of this training participants will be able to: 1. Demonstrate competency in cognitive and behavioral health assessment through

administration of validated screening or assessment tools.2. Formulate a di#erential diagnosis and work with an interprofessional team to recommend an

Spotlight:Patient Safety, Care Coordination &Cutting-Edge UpdatesPROGRAM INTRODUCTION

The 41st Annual CALTCM meeting shines a Spotlight on Patient Safety, Care Coordination, and Cutting-Edge Updates. Attendees will be challenged to !nd ways to integrate advances in medical and post-acute care practices into current care systems, while preparing for the transition to a more e"cient and streamlined Post-Acute and Long-Term Care (PA/LTC) delivery system. We have designed our meeting for practical training of essential members of the interdisciplinary team, in areas where PA/LTC has struggled to improve quality.

The !rst half-day workshop, Cutting-Edge Updates, is presented in a format that enables physicians and other interdisciplinary team members to keep up with the ever-expanding medical literature. Our Keynote Speaker, Joseph Ouslander, MD, sets the stage for an invigorating and interactive second half-day workshop on Care Coordination. Patient Safety is an ever-present issue in PA/LTC, and constitutes the !nal half-day workshop, with a lively and informative Mock Trial presented as the grand !nale.

TARGET AUDIENCE

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation specialists, social services designees, and other members of the interdisciplinary team who care for older adults in the PA/LTC setting.

LEARNING OBJECTIVES

By participation in the annual meeting, participants will have the ability to:

1. Explain models and incentives for improving care coordination, and take appropriate steps to improve care integration with their partners;

2. Develop at least one QAPI Performance Improvement Project (PIP) for implementation in the coming year;

3. Understand how to e#ectively integrate recent advances in medical knowledge into their practice within the post-acute continuum;

4. Identify and implement two new tools to improve transitions of care and care coordination;

5. Identify four preventive steps a facility can take to improve care and thereby reduce its exposure to regulatory or civil actions.

PRE$CONFERENCE: TEAM$BASED INTERPROFESSIONAL COMPETENCY TRAINING IN DEMENTIA SCREENING & MANAGEMENT

The California Geriatric Education Center (CGEC)* presents the 2015 CALTCM Pre-Conference: Team-Based Interprofessional Competency Training in Dementia Screening and Management

BackgroundThe Institute of Medicine (IOM) 2008 Report, “Retooling for an Aging America: Building the Health Care Workforce”, identi!ed signi!cant gaps in training for all health care professions to deliver safe and high quality patient care to an expanding older population. In order to address this gap and thus improve safety and quality of care, specialty-speci!c competencies in care for older adults need to be developed, taught and assessed. A challenge facing gerontologic educators is how to ensure that geriatrics competencies are being taught and assessed in their respective professions.To address the gaps in health workforce training in dementia identi!ed by the IOM, health practitioners need to learn and demonstrate the ability to evaluate older patients for dementia and address their needs within their respective scopes of practice. Healthcare practitioners who directly deliver care need to additionally demonstrate competency in formulating a treatment plan for patients with abnormal cognitive function with special attention to patient safety.

This workshop is divided into 3 activities:1. A brief introduction/overview of the workshop, including a comparison of minimum

competencies for di#erent health disciplines;2. Rotation through competency assessment stations using case study simulations using

actors, and;3. A summary and ‘action plan’ to promote competency in cognitive and behavioral disorders

at the learners’ home institutions.

PRE$CONFERENCE LEARNING OBJECTIVESAt the completion of this training participants will be able to: 1. Demonstrate competency in cognitive and behavioral health assessment through

administration of validated screening or assessment tools.2. Formulate a di#erential diagnosis and work with an interprofessional team to recommend an

evaluation and treatment plan for an adult with a cognitive/behavioral problem.3. Identify caregiver stress and appropriate community support and services for patients with

dementia.

Spotlight:Patient Safety, Care Coordination &Cutting-Edge UpdatesPROGRAM INTRODUCTION

The 41st Annual CALTCM meeting shines a Spotlight on Patient Safety, Care Coordination, and Cutting-Edge Updates. Attendees will be challenged to !nd ways to integrate advances in medical and post-acute care practices into current care systems, while preparing for the transition to a more e"cient and streamlined Post-Acute and Long-Term Care (PA/LTC) delivery system. We have designed our meeting for practical training of essential members of the interdisciplinary team, in areas where PA/LTC has struggled to improve quality.

The !rst half-day workshop, Cutting-Edge Updates, is presented in a format that enables physicians and other interdisciplinary team members to keep up with the ever-expanding medical literature. Our Keynote Speaker, Joseph Ouslander, MD, sets the stage for an invigorating and interactive second half-day workshop on Care Coordination. Patient Safety is an ever-present issue in PA/LTC, and constitutes the !nal half-day workshop, with a lively and informative Mock Trial presented as the grand !nale.

TARGET AUDIENCE

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation specialists, social services designees, and other members of the interdisciplinary team who care for older adults in the PA/LTC setting.

LEARNING OBJECTIVES

By participation in the annual meeting, participants will have the ability to:

1. Explain models and incentives for improving care coordination, and take appropriate steps to improve care integration with their partners;

2. Develop at least one QAPI Performance Improvement Project (PIP) for implementation in the coming year;

3. Understand how to e#ectively integrate recent advances in medical knowledge into their practice within the post-acute continuum;

4. Identify and implement two new tools to improve transitions of care and care coordination;

5. Identify four preventive steps a facility can take to improve care and thereby reduce its exposure to regulatory or civil actions.

PRE$CONFERENCE: TEAM$BASED INTERPROFESSIONAL COMPETENCY TRAINING IN DEMENTIA SCREENING & MANAGEMENT

The California Geriatric Education Center (CGEC)* presents the 2015 CALTCM Pre-Conference: Team-Based Interprofessional Competency Training in Dementia Screening and Management

BackgroundThe Institute of Medicine (IOM) 2008 Report, “Retooling for an Aging America: Building the Health Care Workforce”, identi!ed signi!cant gaps in training for all health care professions to deliver safe and high quality patient care to an expanding older population. In order to address this gap and thus improve safety and quality of care, specialty-speci!c competencies in care for older adults need to be developed, taught and assessed. A challenge facing gerontologic educators is how to ensure that geriatrics competencies are being taught and assessed in their respective professions.To address the gaps in health workforce training in dementia identi!ed by the IOM, health practitioners need to learn and demonstrate the ability to evaluate older patients for dementia and address their needs within their respective scopes of practice. Healthcare practitioners who directly deliver care need to additionally demonstrate competency in formulating a treatment plan for patients with abnormal cognitive function with special attention to patient safety.

This workshop is divided into 3 activities:1. A brief introduction/overview of the workshop, including a comparison of minimum

competencies for di#erent health disciplines;2. Rotation through competency assessment stations using case study simulations using

actors, and;3. A summary and ‘action plan’ to promote competency in cognitive and behavioral disorders

at the learners’ home institutions.

PRE$CONFERENCE LEARNING OBJECTIVESAt the completion of this training participants will be able to: 1. Demonstrate competency in cognitive and behavioral health assessment through

administration of validated screening or assessment tools.2. Formulate a di#erential diagnosis and work with an interprofessional team to recommend an

evaluation and treatment plan for an adult with a cognitive/behavioral problem.3. Identify caregiver stress and appropriate community support and services for patients with

dementia.
