RCS-1: Big Payment Changes Ahead7:45 – 8:30 am CMS is intent on fixing SNF billing, and it’s proposing a new case-mix methodology called the Resident Classification System (RCS-1) that aligns reimbursement to a patient’s clinical characteristics and imposes new restrictions on therapy billing. Panelists will detail the breadth of this proposal, predict when it will go into effect and in what form, and discuss how RCS-1 is a bellweather for CMS’ desire to reward high-acuity nursing services and throttle back therapy service overuse.

Assessing the Outlook & Opportunities for Long Term Care1:05 – 1:50 pm The outlook for SNFs is sobering, as market realities create a dislocation in both the payment and delivery of care. Long term care will continue to face formidable challenges, including decreased reimbursement, redirection in care to alternative and lower-cost settings, and continued consolidation, as many operators struggle to keep pace with the transition to value-based reimbursement. This reality leaves little room for error, as the strategic decisions SNFs make now will be critical to their fate in the months and years to come. Veteran healthcare investment strategist Patrick Hurst will offer his candid assessment of the state of skilled nursing, including an outlook of which areas will be extremely challenging and which have the greatest growth potential. He will also outline recommended strategies for SNFs to survive – and thrive – amidst this unprecedented environment of change.

Patrick Hurst has more than two decades of experience in healthcare transactions, including his tenure as co-founder at investment banking firm Houlihan Lokey, where he advised on transactions involving nursing homes, hospitals, managed care companies, home care agencies, physician groups, and medical product manufacturers. Six Novel Fortune 500 Staffing IdeasIt’s a full-employment economy, and leading companies across all industries have made it a priority to address the challenge of finding and securing talent. Pursuing Extra Revenue for Long-Stay Medicaid PatientsNew Medicaid programs in a handful of states across the country are rewarding SNFs for high-quality long term care with annual additional payouts as much as $250k per facility. This session will showcase the Quality and Accountability Supplemental Payment Program (QASP) of California, one of a new wave of state-based Medicaid Value Based Purchasing initiatives rewarding “above average” providers. A California-based SNF will recount its QASP experience, lessons learned, and advise on the applicability of the program in other states.

The Keys to Exceptional Compliance ProtocolsRecent high-profile nursing home liability cases – including nine deaths at a Miami nursing home during Hurricane Irma last September – have caused many SNFs to reexamine their approach to regulatory compliance. This session will address optimal SNF compliance protocols, internal monitoring, and employee training – and offer advice on how to build a culture of trust so that employees can report potential infractions without fear of retribution.

Brookdale Moves into AL Respite Rehab Brookdale is now positioning its AL communities as an affordable alternative to SNFs for low-acuity rehab care. They will describe their partnership with bundled payment providers who are steering rehab patients to Brookdale communities for $150-per-diem AL respite stays. Brookdale will discuss the economics and scalability of the AL respite model, and make the case that when SNFs are accountable for readmissions penalties, there are times they too are likely to view AL as an attractive alternative step-down setting (compared to home).

Before the ACA was drafted, he sensed the impending shift in the reimbursement winds and sold off his entire 70-facility Florida SNF to pursue his vision for NuVista, a bold new post acute care navigation company whose reimbursement structure is tied to capitated Model 2 bundled payments. Paul will share his inspiration and frame out the critical pillars to the NuVista’s success, including seamless care coordination; a robust home-grown data platform; an ironclad network of providers (including a JV with hospital); and an unrelenting commitment to wellness. Paul’s presentation is an exciting, actionable blueprint for skilled nursing organizations interested in pursuing similar concepts in other markets.

Keys to Mastering the iSNP Opportunity SNFs with an appetite for risk-based reimbursement are becoming independent Medicare Advantage insurers through the creation of Institutional Special Needs Plans (iSNPs). Two SNFs with iSNP experience will detail the four core components to operating a successful Medicare Advantage program: clinical integration, care management, data, and culture.

Aligning with Health Systems through Joint VenturesTaking their preferred provider status to the next level, some SNFs are entering into joint venture partnerships with health systems to create deeper interdependence between the organizations. The architects of two recent JVs will outline the strategies and objectives of both parties, and discuss how post acute providers are managing the partnerships, including guaranteed LOS thresholds, episodic payment models, and post-SNF care coordination.

BPCI Advanced: The Opportunity for SNFsThe arrival of new voluntary bundles is the latest opportunity to either begin or expand one’s presence in risk-based payment. A panel of provider executives with extensive BPCI experience will deconstruct the new BPCI Advanced program to identify where it creates the greatest opportunities for SNFs, including which episodes align with facility-based rehabilitation, and how baseline pricing is a critical factor in determining which episodes present the greatest upside potential. In addition, the group will explore alternatives to conveners – including how post acute providers can co-own the risk – and describe how to optimize one’s care coordination structure in a capitated, episodic model.

SNF Networks Leverage Collective Bargaining Power A SNF alliance concept is emerging that aims to compete with large regional operators. Two regional alliances of small, independent SNFs, will explain how they are developing membership standards to make their collective whole more attractive to ACOs, health systems and managed care; and earn preferred provider status for individual SNF members. Panelists will also discuss the clinical protocols for member organizations and explain the unique selling proposition they use when negotiating with payers and referral sources.

Aidin: SNFs Competing Online for Hospital ReferralsAn emerging online service at the acute care level is changing the way hospitals discharge patients to post acute providers. Known as Aidin, the service determines where to send a patient based exclusively on a post acute provider’s performance and quality metrics. Executives from Aidin, which uses the self-described label of “care transitions marketplace,” will explain the measures used to determine top-performing PAC providers, as well as the ramifications this service has for patient volume to preferred providers.

Next-Gen SNF Alliance A 20-year-old Minneapolis-based alliance of 40 SNFs has ambitious plans to reinvent itself as an integrated post acute network with advanced medical capabilities and a Management Service Organization structure. CareChoice will share its evolution from a humble cooperative of SNFs sharing best practices to its current plan to compete against large regional SNFs for preferred provider status by requiring stringent clinical standards of its members; penning gainsharing contracts with local ACOs; and becoming one of the nation’s first Clinically Integrated Networks of SNFs.
