Background
Accident prevention checklist inspection could be applied as a part of guidelineto prevent incident/accident
(OSHA,2011).
Various types of hazard encountered in laboratory (Bennedict, 2004).
Safety Inspection related to Acc Prevention at Opt Lab in XYZ university hasn’t been conducted.

Purposes & Objectives
Verify the effectiveness of the protection which is offered to proffesionals and/or students invoved in laboratory activities.
Encourage safe work practices in laboratory.
Identify spesific programs at Optical Laboratory in XYZ University.

Conclusions
Optical Laboratory at XYZ university in Japan has comply 74 safety checklist inspection items, out of 79 items (94%).
Spesific Programs:
1. Safety in Laboratory is one of the compulsory courses (1 credit course).
2. New employee must be trained about safety induction before they start the job.
3. Yearly Medical Check up & Health Surveilance for all student & staff.
4. Emergency drill once a year: fire, chemical spill, and nature disaster.
