Option A: Pay all premiums (1st month and monthly renewals) by Electronic Funds Transfer (EFT). Automatic Clearing House (ACH)isusedforinitialpaymentandBank Service Plan(BSP)isusedforrenewalpayments.WhenchoosingtopayboththeinitialandmonthlyrenewalsbyEFT,theapplicantmustcompletetheformandsubmititwiththeapplication.DONOTsubmitasignedcheckforpaymentunderthisoption.Toavoidpotentialdelaysinprocessing,submitavoidedcheckandcompletetheaccountinformation(routing/accountnumbers,nameoffinancialinstitution)ontheform.
