APPLICATION CRITERIA The Brookfield Apartment Homes supports the Fair Housing Act as amended, and prohibits discrimination based on race, color, religion, sex, national origin, handicap or familial status. Applicants and co-applicants must be 18 or over unless protected by the familial status per the Fair Housing amendment. All adult members of the household must complete a TAA Application and sign a Tenant Release and Consent. Third party written verification will be obtained to certify household claim. The Brookfield Apartment Homes requires all applicants to meet the following criteria in order to qualify for housing at our community. Income: Each household must show income that is at least 2.5 times the resident paid portion of the rent. Income Verifications: The Brookfield Apartment Homes requires each applicant to provide us with at least four consecutive pay stubs for each occupant 18 and older prior to having their application approved. If self-employed, a copy of your last Income Tax return is needed. Occupancy: (2) Two occupants per bedroom + (1) One additional Occupant = Maximum # of Occupants One bedroom = 3 occupants Two bedroom = 5 occupants Credit/Criminal History: We perform credit and criminal background checks in accordance with applicable federal and state laws. An unsatisfactory criminal background check revealing a serious charge including, but not limited to, conviction of a felony or misdemeanor for crimes against persons or property, drugs, assaults, weapons or sex crimes or deferred adjudication/status of a felony or misdemeanor for crimes against persons or property, drugs, assaults, weapons or sex crimes may result in denial of your application. Rental History: All occupants 18 and over are required to sign the Lease Agreement. Applicants may be denied occupancy for non-payment or frequent late payment of rent, evictions, drug use, poor housekeeping, poor supervision of applicant’s children, and unruly or destructive behavior by applicants and/or their guests. Waiting List: Leasing is based on a first come, first service basis in accordance with the qualifying criteria of THE Brookfield Apartment Homes. When there are no vacant or available rental units, eligible applications will be placed on a waiting list. The Brookfield Apartment Homes waiting list is close/open considering the number of applications for each size or type of unit to fill anticipated vacancies for the coming 12 months. The Brookfield Apartment Homes will publicly announce the close or open of the waiting list. Utilities: All residents will be responsible for their electricity/gas, water/sewer, and trash and pest control. NO CASH ACCEPTED FOR ANY TRANSACTIONS. NOTICE: By signing this document you acknowledge that you have had the opportunity to review the Applicant Criteria. Privacy Policy for Personal Information and Rental Applicants and Residents

We are dedicated to protecting the privacy of your personal information, including your social security or other governmental identification numbers. We have adopted a privacy policy to help ensure that your information is kept secure. We follow all federal and state laws regarding the protection of your personal information.

How information is collected: You will be furnishing some of your personal information (such as your social security or other governmental identification numbers) at the time you apply for residency at our community. This information will be on the rental application form or other documents that you provide to us or to an apartment locator service; either on paper or electronically.

How the information is used: We may use this information in the process of verifying statements made on your rental application, such as your rental, credit or employment history. We may use the information when reviewing any lease renewal. We may also use it to assist us in obtaining payment from you or any money you may owe us in the future.

How the information is protected and who has access: In our company, only authorized persons have access to your social security or other governmental identification numbers. We keep all documents containing this information in a secure area, accessible only by authorized persons. We limit access to electronic versions of the information to authorized persons only.

How the information is disposed of: After we no longer need your social security or other governmental identification numbers, we will store or destroy the information in a manner that ensures that no unauthorized person will have access to it. Our disposal method may include physical destruction or obliteration of paper documents or electronic files containing such information.

Locator Services: If you found us through a locator service, please be aware that locator services are independent contractors and are not our employees – even though they may initially process rental applications and fill out lease forms. You should require any locator service you use to furnish you their privacy policy as well.

Electric Connection Fee $50.00 fee, plus usage, if electricity is not connected in residents name by time of move in, or is ever disconnected during the lease term. Electricity MUST stay in your name for the duration of your lease contract responsibility.

Monthly Billing Fee $4.19 Administrative Fee added to monthly Water/Gas Bill Trash $10.00 added to monthly Water/Gas Bill Pest Control $3.00 added to monthly Water/Gas Bill

Pet Limits Limit of (2) pets per apartment – pet pictures and veterinary records must be provided for our files.

Pet Restrictions Acceptable pets include: cats, dogs, fish (50 gallon tank limit size limit with manager’s prior written approval) and select birds only. In regards to dogs, restricted breeds (full or mixed breeds) not acceptable include: Chow, Doberman Pincher, Pit Bull, Stafordshire, Terrier, Rottweiler, German Shepard, Wolf and Wolf Hybrid, Prohibited pets include, but not limited to monkeys, snakes, ferrets, iguanas, potbelly pigs, and rabbits.

Pet Depoist/Fee Pet Fee: $300.00 (non- refundable) per pet

Pet Deposit: $100.00 (refundable) depending on move out inspection

Pet Waste Violation $100.00 per violation if pet waste is not picked up by pet owner

Returned Check Fee $50.00 plus $50.00 late fee if paid within 24 hours, if not paid within 24 hours all late fees apply retroactive back to the 4th day of the month.

Late Charge $50.00 initial late charge on the 4th plus $10 per day thereafter, (maximum of 15 days for daily late charge). Total late fees that can accumulated equal $200.00 per month.

Month to Month Fee Market Rent + $150.00 per month expired leases will be subject to MTM fees automatically.

Lock Change Fee $25.00 per lock and request must be in writing and payment made prior to locks being changed.

Trash Violation $25.00 Fine for each bag and/or item that is left by front door, on patios/balconies, or not properly disposed of in the trash dumpster.

Apartment Transfer Fee $300.00 transfer fee (waived if within 30 days from lease expiration). New pet deposit(s) and pet fee(s) are required for new apartment. Requires a written 30 day notice, and the resident must have lived in the current apartment for a minimum of 6 months.

Roommate Fee $100.00 per change, due at the time of change is submitted.

Addition – new applicant must qualify per the applicant criteria

Deletion – Remaining residents must qualify on their own
