I hereby declare that I am aware of and accept the provisions of the Design of Identity/Identity of Design Competition Rules and Regulations by Teatr Wielki – Opera Narodowa. I authorise the Competition Organizer to process my personal data in accordance with the Personal Data Protection Act of 29 August 1997 (uniform text: Journal of Laws 2002, No. 101, item 926, as amended) for the purposes of holding and announcing the winners of the Competi- tion specified in paragraph 2, including publishing those data in mass media. I authorise BMW Vertriebs GmbH Sp. z o.o. oddział w Polsce to process my personal data in accordance with the Personal Data Protection Act of 29 August 1997 (uniform text published in Journal of Laws 2002, No. 101, item 926, as amended).
