COMENTARIO (WENCHY)
En este informe McBurney presento 8 casos entre el 21 de Mayo de 1988 19 octubre de 1989 (16 meses), el último caso falleció al 4to día de peritonitis postoperatoria y ello hace una mortalidad del 12.5 %. McBurney sólo contaba con el procedimiento quirúrgico y la morfina para el dolor, por otro lado, el marco conceptual de la enfermedad no se había desarrollado.

En 1986, Alvarado publicó el clásico score su epónimo para el diagnóstico de AA.El score demostró una sensibilidad de 0,87, especificidad de 0,94 y exactitud diagnóstica de 0,95. El score diagnóstico de apendicitis demuestra una elevada especificidad, sensibilidad y exactitud diagnóstica para el diagnóstico correcto de apendicitis

Gut translocation bacterial is defined as the passage of gatrointestinal microflora across of the lamina propia to local mesenteric lymph nodes (MLN) and from there to extranodal sites. Is has been proposed that bacterial translocation may initiate a citokine response from macrophages encountered in mesenteric nodes. In certain condition this may predispose the host to the development of septic morbidity.

EXPERIMENTAL RESULTS. When staphylococci were injected 3 hours before antibiotics were given, the resultant lesion was hardly smaller than the control. Penicillin injected longer than 3 hours after staphylococci had been introduced had no effect on ultimate lesion size.
Incisions made and contaminated 3 hours before the intravenous injection of penicillin, chloramphenicol, erythromycin, or Achromycin in the doses used in treating the dermal lesions resulted in lesions indistinguishable from the control contaminated incisions not treated with antibiotics.

The fact the time of the earliest opportunity for incisional contamination can exactly determined arouses speculation about the absolute failure of the so-called ¨prophylactic antibiotics.¨ Frequently ¨ prophylactic antibiotics ¨ are started after operation.
