A steering committee was formed in 1984 in response to the community lobbying for a home based palliative care service. Under the guidance of the Committee of Management with Allan Bath as Chair, Ballarat Hospice Care Inc. (Ballarat Hospice) was founded – incorporated in 1985 and we accepted our first patient on the 18th March 1987. We were supported by the Department of Health and Human Services and the community then and as we are now.

We continue to deliver specialist palliative care services within the Ballarat City, Hepburn Shire, North West of the Golden Plains Shire and the western part of the Moorabool Shire. Hospice offers holistic care to people with life threatening illness, and to their families and carers. We work closely with primary health services that service the local area and close to where patients reside. It is our responsibility to provide the best information to enable patients and their families to make the best decisions most appropriate for their care.

Our particular strength is being able to offer a flexible interdisciplinary service to our community, without cost to the patient and the family, through specialist nursing care, psychosocial, spiritual, emotional and welfare services, the loan of equipment and grief and bereavement support.

Consistent positive feedback from people utilising our service, and the same coming from partner community organisation networks, encourages us to believe that we are continuing to provide a high quality, relevant and necessary service in our region.

The Annual Report will be presented for adoption at the 28th Annual General Meeting to be held on Monday 19 October 2015 at 7.30 pm in the Chapter House of the Ballarat Anglican Cathedral.

Our 2012-2015 Strategic Plan kept us on track with our direction and it has given a solid foundation for 2015 – 2018. The Committee of Management has also been proactive in identifying the challenges and solutions in the context of future directions 2018 – 2030.

Review of the 2012 – 2015 Strategic Plan showed achievements in the areas of research with a publication in the RSA Journal, Renal Society of Australiasia, Integrating Renal and Palliative Care. Carer Education evaluations showed that the sessions increased preparedness and confidence in caring for their loved ones at home. This data also showed that carers’ education has a direct and positive impact for people being cared for and dying at home. Planning and anticipating patient care needs is also a component of supporting this choice. An audit undertaken through the After Hours project Phase 3 showed only 4.9% of patients who called to Ballarat Hospice afterhours number required emergency care. Research undertaken by Julie Fletcher PhD Candidate showed that all community palliative care staff practice at a spiritual level but there are different levels of skill. To enhance our spiritual / pastoral care component of our service Paula Robinson completed her Clinical Pastoral Education with clinical placement at St John of God Ballarat. We acknowledge Thomas Miller, Manager, Pastoral Services for supervising and supporting Paula.

The Chair and Executive Officer will present a joint report as this reflects our united approach to achieving our Strategic Goals. While we acknowledge our own strategic and operational responsibilities, together with the support of the Committee of Management, staff, volunteers and supporters, we can deliver on our Vision and Mission to provide specialist palliative care services to our community. We continue to work collaboratively through strengthening our relationships with our partner organisations. Hepburn Health Services and Ballarat Hospice, with support from the Grampians Regional Palliative Care Consortium, staged the play Four Funerals in One Day in Daylesford. This play helps promote palliative care. The play was well attended with great interaction from the audience participating in a question and answer panel following the production. We were invited by Palliative Care Victoria (PCV) to participate in the projects The Dreamers and the Two of Us. Patients, carers and volunteers were invited to share their stories. Each project, unique in its own way, is also a vehicle for promoting palliative care and conversations.

The Dreamers was produced by Pippa Wischer, a visual artist specialising in documentary portraiture. She asked 40 people receiving palliative care, questions about life, dreams and dying. To celebrate

National Volunteer Week and National Palliative Care Week in May 2015, PCV decided to ‘go behind the scenes’ and invite pairs of palliative care volunteers and the people they support to tell us about their relationships. These eight inspiring stories illustrate how these relationships are mutually valued and enriching. Ballarat Hospice was represented in these projects by Moira and George, Tamara and Carol – we thank and appreciate them for their inspirational courage.

We have undertaken a review of how we communicate in the modern world. A new information and resource folder has been developed in cooperation with past carers and volunteers of our service. A recognised challenge is the balance of information we give to people who are new to our service, without over whelming them. The input from the carers and volunteers was invaluable and we believe we now have a product that raises awareness of what is available and, when the need arises, further information is provided - right care at the right time. Our web page has been updated and we have ventured into the social media arena with a Facebook page for both Ballarat Hospice and the Friends of Hospice Op Shop. Thank you to Kevin Harper for his creativity and guidance in development of social media and web page.

The Op Shop has gone from strength to strength. We congratulate our Manager, Genevieve (Gen) Ware and the Department of Health and Human Services (DHHS). The grant given to us enabled this initiative and the employment of Gen. The Op Shop is showing a profit and is enhancing our fundraising activities which will provide more opportunities for innovation and reduce reliance on the DHHS. We would also like to congratulate all our volunteers, current and new, for embracing this idea. Gen with the support of Karen Taylor, Manager of Volunteers, has recruited an amazing group of people who are so willing to be a part of this venture. We appreciate their hard work, from managing stock, visualising, presentation and the professional manner in which they operate. They are all remarkable advocates for Ballarat Hospice and share the wider message of palliative care in our community.

We continue to review our workload, how we manage the increasing referrals and supporting those who need our care, be it through clinical improvements, working with other organisations and information management systems.

We are driven by our values and our service focuses on safety and regulatory compliance to ensure we practice in a safe environment for all. Accreditation and quality is paramount as shown by our strong Risk Management Framework. In this environment the human element is sometimes forgotten by bureaucracy but is often expressed in the many cards and letters received from the people we care for. Over 300 cards and letters we received from families of patients we have helped care for were recently reviewed to assess the value of our services in this context. Common themes that people expressed in these cards mentioned kindness, compassion, professional approach, information, the thought given to remembering the person’s loved one at the 12 month anniversary, love, communication and thoughtfulness. These cards and letters give a clear affirmation that we are delivering on our values. All feedback is appreciated as suggestions shape the opportunity for improvement to our service.

The community support is humbling from our corporate support, Guardianship, general donations and donations given in memory of those who are loved and remembered. Tanya Daniels continues to support us through The Good Guys Foundation and a big thank you to Steven Anderson and John Bourke at Regent Cinemas and all our sponsors for our successful Movie Night 2014 which enabled us to purchase a delivery van that delivers the equipment to our patients.

We would like to make special mention of Fiona Watson who retired from the Committee of Management in February 2015. Fiona continues to contribute to Ballarat Hospice and we thank her for her creativity, skill and sense of fun. We take this opportunity to acknowledge Olga Anderson, our first receptionist commencing work with Ballarat Hospice in 1990. We acknowledge her strong commitment and contribution to our organisation.

We also recognise Kath Connors and Rose O’Meara both of whom have been volunteers with Ballarat Hospice since the beginning. Rose retired from Friends of Hospice while Kath continues her role in supporting patients. We also farewelled Val McMaster and Arnold Treloar from Friends of Hospice and wish them well for their future adventures. Thank you to our Committee of Management, Staff and Volunteers, DHHS, patients’ GPs, Grampians Region Palliative Care Consortium, Grampians Regional Palliative Care Team (GRPCT), Ballarat Health Services, St John of God Hospital – especially to the Medical Administration for assisting with the After Hours Audit, District Nursing Services and Home and Community Care Services through the Councils in our service area, Very Special Kids, Commonwealth Carers Respite Services and the many other services that contribute to the care of the patients and their families that we care for at Ballarat Hospice. Mostly we would like to thank the patients and families who allow us into their lives at such a sensitive and life changing time.

This year the report is written against our Philosophy, Core Values and Primary Objectives. These values are the very essence of our service delivery; they highlight our thinking, our delivery and our approach to our relationships with our patients, their families, including carers and volunteers. Our Values were developed in consultation with staff who work diligently in all aspects of service delivery to ensure that these values are being met. We have included comments from our patients and carers to reflect the achievements of our philosophy, core values and primary objectives, which contribute to our evidence of quality of care.

mind that she was not alone. She was also given a great deal of kindness from you all.

• Our staff and volunteers display qualities not only of compassion but also of commitment and deep understanding of the complexities of life.

Our specialist palliative care nurses work across the Monday – Friday week with an after-hours and week end “on Call” service. The nurses are skilled, experienced and compassionate to the needs of patients and families. This year we farewelled Karen Davies and thank her sincerely for her contribution to palliative care. Our nurses are constantly working on systems to stream line our communication and coordination of care.

Quality and risk is essential for safe practice and Karen ensures that we meet our compliance with legislation and that standards are adhered to through the coordination of the Risk Management Sub-committee and the operational Quality and Risk Committee.

Our volunteering program offers opportunities for supportive care within the home, through carer respite, companionship, social interaction and transportation. There are also fundraising, retail, administration and governance roles.

Volunteers

Special Interest Group members made a donation to assist us to purchase a new bed with rails and an air mattress. To assist in raising this money a Garden Party was held last September – a great day and a great outcome for our patients. Thank you so much!

Friends of Hospice (FoH) continue to work tirelessly with an Open Garden Day held at Raglan, another successful Golf Day, attendance at the Memory Tree and sold tickets for the Rotary Club Christmas Raffle.
