Dear Friends,

We use this opportunity to inform you about the activities that were implemented by the Austrian Doctors for Disabled and Union SUMERO during the implementation of the project „Support of people with severe disabilities in poverty in B&H“. This Report should give you a picture into the problems of disabled persons in B&H and inform you about our beneficiaries improvement during this period and the continuation of our activities in order to raise quality of life of the beneficiaries.

Reporting period: April 2009 – March 2010

ABSTRACT In 2009 we have provided direct help for people with severe disabilities through our project, we have also provided and continued with the service of personal asisstance through trainings and the engagement of personal asisstant staff, we worked on the model for the assesment of disability, and influenced and lobby the government representatives to make new laws and system solutions. Through the service of personal asisstance, depending on the needs of each beneficiary, we made the socialization and the everyday life easier for people with disabilities. The beneficiaries where provides with dirct financial support from a huge number of sponsors from Austria and Bosnia and Herzegowina. A large number of meetings wit the government representatives from Republic of Srpska and the Federation of Bosnia and Herzrgowina were held. End of 2009 we started a Pilot-project called „Inclusion and assesment of people with severe disabilities“, wich we are implementing together with the Institute for medical Expertise of health conditions, Sarajevo, the centre for social work, Banja Luka and the Ministry of health and social protection of RS. During the last year we also worked on the lobbyingof government representatives and a media campaign, respecitvely on raising public awareness. December 2009 we had a meeting with the Director of the World bank, Mr. Marco Mantovanelli. At the meeting was agreed that the World bank will try to work together with DfD/Sumero on some of their projects in the field of social protection. It was also agreed some future activities and meetings with the representatives from the World bank and their consultants, so wo could easier agree a better cooperation with the World bank.

ACTIVITIES IN THE LOCAL COMMUNITY Thanks to the SUMERO members and the established project network of NGO's of people with disabilities (Union of paraplegics of Kanton Sarajevo, Union of citizens fallen to cerebral paralysis and dystrophy Goražde, Union „Sun“ Mostar, Union „Steps of Hope“ Tuzla, Union of people with disabilities Vlasenica, Union MNRL Banja Luka, Union of dystrophycs Doboj, Union „The Sun is Mutual“ Trebinje and Union „The Joy of Friendship“ Bihać) it was easier to implement our project. Besides that we had an easier visiting and communication with our beneficiaries, monitoring of work of the personal asisstants and providing support.

PROBLEM BACKGROUND AND THE POLITICAL SITUATION IN 2009 Financial compensations for people with disabilities in Bosnia and Herzegovina, respectively in the both entities, are very high wich shows also a analysis of the World bank, where we can see that the percentage of the compensations in B&H (together with Croatia) is one of the highest in Europe! However, the money is not directed towards real needs nor the persons for whom the support is the most important. The key for this inadequate aimed support is the asessment of disability. The asessment system in B&H is disorganised, so that we have different asessment of disabled war veterans, work disabled and in the end the civil disabled persons. If we add the differences in the asessment in both entities, we can conclude a evident problem wich is reflected in the lump-sum allocations for these categories, wich ultimately leads to unsustainable socialwellfare system as the budget and the financing of the state in general. Currently is in forcw in all aspects of the asessment the so called „medical aproach“ of asessment wich is not based on the needs of people with disabilities, but on the fact how much they are disabled, respecively on the fact what they cannot work. In the field of social protection there is still discrimination present based on the place where people live. The new law of social protection in the Republic of Srpska went through discussions in the Government of RS as well as through public discussions in local communities. It was expected that this law will come on the daily task of the National Assembly in march 2009. According to the new law, the beneficiaries should receive a amount of 150 KM, but the law was still not voted so that persons with disabilities in RS receive for Long-term Care only 41 KM! In the Federation od B&H a law for social protection is beeing implemented wich is adopted in march 2009. This law has partly improved the position of people with severe disabilities. The developements in and around the Parliament of FB&H have shown how difficult changes of the socialwellfare system are. The representatives of Sumero were present at one session of the Parliament and made lobbying to the government representatives to adopt this law for social protection. People with severe disabilities in FB&H been through a revision in the Institute for disability asessment and are receiving at this time over the Federal Ministry of Labour and Social affairs round 360 KM. In the field of culture and arts are people with disabilities also discriminated because of the maladjustments of technics, cultural and religious facilities to their needs. The existing legislative framework about the space planing and the construction works contibutes only partly to the removal of architectual barriers, prescribing a obligation that newly built facilities have to be adopted, while the sanctions are not specified. Sport and leisure laws are also disciriminatig towards people with disabilities, especially they exclude women and children with disabilities. In B&H ther is no systematic support for the realization of family living of people with disabilities, wich is especially reflected in the realzation of motherhood. Systematic solutions do not provide support services of personal asisstance, because of that are people with disabilities fully addressed to the system of social protection, care and sozialisation within their families.

COOPERATION WITH GOVERNMENT REPRESENTATIVES AND PUBLIC AWARENESS RAISING During this period a large number of meeting were held with the government representatives. We met two times with the vicepresident of the Federation of Bosnia and Herzegowina Mirsad Kebo, and started negotitions with the World bank for a Project co-operation.

10 SUPPORT TO PEOPLE WITH SEVERE DISABILITY IN BOSNIA AND HERCEGOWINA

SERVICE OF PERSONAL ASISSTANCE (PA) Service of Personal asisstance allows independent and quality life, that gives support to a full participation of people with disabilities inside the social flows of the local community as well as support with the basic functioning. The goal of the personal asisstance is the support of people with disability in the socialization and involvement in social flows as well as doing the every-day's needs in the household. Personal asisstants training In this year we appointed 14 personal asisstants. We held two trainings for personal asisstants where people with disabilities also participated. We visited all personal asistants and the beneficiaries, respectively the persons with disability for whom these asisstants are engaged. The trainings had the purpose to improve the knowledge about personal asisstance, and as well as about the rights that the people with disabilities have, and with the need of socialization of people with disabilities. These activities were funded by the Swiss organisation „SDC – Swiss Agency for Developement and Cooperation“ in the period January-April 2009. The trainer was Tanja Mandić from Banja Luka and the subject of the training was „Perspectives of the personal asistance in B&H“. Training of personal asisstants in Banja Luka

In 2009, by users who have had personal assistants, unfortunately, are deceased Lejla Efendic Amina Efendic and from Sarajevo and Nikola drawn from Banja Luka. Work description of personal asisstants The scope of tasks of personal assistants depends on the needs and interests of users.Although the coordinator has material liabilities for the operation of the project, users, and the persons with disabilities are those who give personal instructions to the assistants, and define and control their business. Personal assistant can become part of the user's life for more bases.For example, can assist in personal care and can also pomogati around the user's socialization (socializing, walks, visits to the school or college, etc.). The tasks of personal assistants perform the following: assistance in moving, help with placement in a wheelchair and out of them, and changing positions in bed, help with dressing and removal, help with personal hygiene (bathing, care and beautification of hair, skin, etc .), massage and exercise, house cleaning, laundry, clearing away the beds, preparing food and assistance in taking, buying, Administrative tasks (correspondence, invoices and letters, help you manage your household budget), maintenance of space around the house (garden), repairsand maintenance of equipment (wheelchairs, lifts and other technical aids). The tasks of personal assistants associated with the user's socialization are: assistance in participating in hobbies and leisure activities, help with meetings with friends, help in business meetings or faculty, assistance in achieving communication, help with work on the computer, reading, help with homework, etc.

The specific schedule of duties and tasks of personal assistants are regulated in concert with the user and project manager. In addition, users' needs can vary from day to day, and the duties and tasks of personal assistants change accordingly. Considering that a person with disability has different needs depending on the diagnosis, age, family status, etc., of different types and activities of personal assistants. Founded in the difference between personal assistants are the ones who are working with customers who can express their needs and coordinate activities of personal assistants and those working with customers who are not able. Another categorization of the activities of personal assistants are those activities that a personal assistant works directly with the user (to help in maintaining hygiene, dressing, feeding, helping prilkom movement, etc.) and personal assistant to the activities performed at home (home maintenance, or helping their parents in case the user that the user refuses contact with everyone except the parents / guardians). A few examples: Amela V. can express her needs and wants and coordinate the activities of personal assistants. She has two personal assistants one is in charge of home help, she helps Amela with personal hygiene, dressing, make-up, etc while the other one is responsible for takeing her to university and writing lessons for certain subjects instead of her. Amela is a regular student of Philosophy, University of Sarajevo Department German language.

Džemila O., Denis Č. and Azra K. can not communicate and are not even able to coordinate the activities of their personal assistants, their parents are doing this for them. Work of personal assistants is mainly based on home care assistance in maintaining hygiene, feeding, clothing, etc. Beneficiary Nikola R. from Banja Luka, with his personal assistant. Unfortunally becuse of the diagnosis and difficult health condition he died in 2009.

Ilma D. is engaged as a personal assistant on two hours per day of home help. Her commitment is based on help with housework, shopping, performing administrative tasks for the user. Her work is done very professionally . Zlatan mother is weak an old so she very much relies on his personal assistant.

Vaso B. is very pleased with the work of a personal assistant, he said that Srdjan is his right arm, both in performing household chores, assistance in leaving the doctor, keeping home budget and purchasing. Srdjan and Vaso became great friends, they often go out, whether it comes to walk through the city, or perform some jobs, and when they are able they love to watch a movie at the cinema, or go on a trip, so the user socialization is at a satisfactory level.

Dario C. is engaged as personal asistant to Denis C. for two hours of help in the house, he take care of Denis hygiene and helps him around the dressing and feeding. Also, does housework, takes care of the doctor findings and if necessary go to municipal, bank, etc.

Anđela R. Spends every day in company with Senada J. who is engaged in two hours of her home help. Senada assist her in dressing, feeding, maintain personal hygiene, even in performing household chores, supplying food, etc. Angel is pleased with Senada work and diligence, her quality of life improved significantly. After evaluation of the work and of the visits is noticeably better mood at Anđela.

Jasmin new personal assistant of Dino M., who is a physiotherapist by profession, is very good in performing obligations that he has as a personal assistant. In the opinion of Dino mother, Jasmin is very seriously about his obligations, with a noticeable dose of resposiblity in the approach to work and sacrifice. Dino is very good friend with Jasmin, due to the fact that they are same generation, and have the same interests and preferences. The work of personal assistant is based on Dinos socialization, he helps Dino with feeding, dressing, bathing, and in the active implementation of physical therapy, because of that Dino is a lot better with using his right hand.

Personal assistant Lejla Miladin is very well experienced in her work. She takes Amela to university and attend lectures with her. Amela V. is satisfied with the personal assistant. Amela is a regular student of Philosophy, University of Sarajevo Department Germanistik. Maja Velic is engaged as home help and she take care of Amela hygiene, wear and make up, so Amela can not imagine even one day without Lejla and Maya as her everyday help.

Amela V. With her PA Mirsad Z. is a good example of how personal assistance can contribute to the socialization of users and economic independence. Thanks to his personal assistant, Mirsad is outgoing and positive . He began to go out and meet new people which previously was impossible. Also he wants to go on computer courses, which is a big improvement for Mirsad future.

Mevludin S. Has positive changes in behavior and success in school since Adisa I. was engaged as her PA. Mevludin is now outgoing and cheerful. Adisas work is mostly based on help Mevludin S. with writing homework and with learning. She also helps with housework, with doctors apointment, she makes purchases in town, etc. She also represent a great help to mother, because now she can manage to do most of the for which previously she did not have time. Adisa I. is engaged in two hours of home help. Mevludin S. and Adisa I. became good friends.

Dzemila O. is in serious health condition that worsens over time, She require constant care. Taiba H. is engaged in two hours of home help, she helps in Džemila feeding, dressing, personal hygiene, etc. Taiba is of great help to Džemilas mother. She also often read stories that Džemila likes to listen. Džemila is aware of everything but can not communicate. She is accustomed to Taiba and she is always happy to see her.

Because of her severe health state Azra K. spent most of her time laying down. She is entirely directed at nursing of another person. Azra is a cheerful and social girl. Ines is engaged in two home help hours. Ines T. assists Azra in feeding clothing, and personal hygiene. Azra mother can now work carelessly to ensure the existence of family, because she knows that Azra is in good hands.

DIRECT HELP TO PEOPLE WITH SEVERE DISABILITY AND SPONSORSHIPS Given the difficult living conditions in which beneficiaries of the project are they need financial assistance, therefore, in the past year we provided financial support for a large number of users from the whole territory of Bosnia and Herzegovina. The money from the sponsorship was mainly spent on food, medicines and other essential supplies, which greatly facilitate life to the users and their families.

Financial assistance that Niksa P. receives allows him to buy school supplies and clothes and shoes for school. Niksa is graduate and would like to go

to college.

Not one member in Nenad K. family does not work, so the money he receive is of

great importance. Nenad use money mainly to purchase food and hygiene supplies.

Given the very bad financial situation, help that Mirsad O. gets is very important and it makes life easier to him and his family. The money is mostlyl spent on food and they do not know

Thanks to this financial aid Momo K. from Banja Luka has a better and quality life, the money that gets he spent on the medicines, and the purchase of school supplies. Mother of 26 year old Sanja P. is very grateful to the donor, she says that is very important for her the knowledge that she and Sanja are not alone and that there are people who are willing to help. Marko I. parents says that thanks to donor they were able to afford his son, physical therapy, which helped him to stand on his feet.

DISABILITY ASESSMENT IN B&H The purpose of our involvement in this segment is to define an adequate model estimate of disability suitable conditions and BiH legislation, harmonized system of assessment between the entities, and to improve knowledge of international standards, assessment of disability. One of the key documents is a document of the World Health Organization (WHO) entitled "ICF (International Classification of Functionality, Disability and Health). Adequate assessment of disability, Bosnia and Herzegovina and its entities to harmonize their systems evaluation and to have the ability to direct resources towards the real needs of people with disabilities. In both entities of Bosnia and Herzegovina lacks documentation for the assessment of disability. Also, there are no adequate data about the number of people with severe disabilities. During many discussions with the authorities in both entities, indicating the interest in a joint project, which would have the task of drafting a questionnaire to assess disability. By doing so, used the Austrian and international experience. Goals

• Development of questionnaires for the inclusion of persons with severe disabilities. The basis for this assessment models used in the RS and the Federation, ICF International model and the Austrian model of the required degree of care and Austria Model "for evaluation of disability.

• Iinclusion of persons with severe disabilities in two municipalities in Bosnia - one in the Republik of Srpska (Banja Luka), one in the Federation (Novo Sarajevo). Calculation of people with severe disabilities for the entire BiH.

• Proposals to create a model of assessment, which would be suitable for these modern times, international (European) standards of social rights

Work of expert teams During the project, has organized numerous meetings with government representatives and experts from both entities. Established an expert commission to test the model estimates (fbih, rs, icf / mkf, au model, barthel test), on the basis of these results and data exchange, she began to create a model of assessment that would fit the conditions of bih and the international and european standards . The composition of the commission is balanced between the two entities (republika srpska, the federation).

Colaboration

Through the activities in developing the model estimates of disability was further intensified cooperation with the Ministry of Health Federation, the Ministry of Health and Social Welfare of RS, Institute for Medical Expert health status of Sarajevo and the Centre for Social Work Banja Luka.

Our cooperation with the Ministry of Health and Social Welfare of the Republic of Srpska is officially certified with the agreement on cooperation signed by the Minister Ranko Škrbić personally, and with the Institute for Medical Expert health Sarajevo, where we also signed an agreement on cooperation.

REPORTS AND DOCUMENTATION Sumero team is regularly monitoring and documenting all activities from the filed. All the reports, whether the reports of work from filed or the ones received from our local partners, go to DfD and other organization that support this project. Communication and reporting are performing by regularly phone lines, Skype contacts as well as other forms of reporting. All project documentation is kept in archive of Union Sumero in Sarajevo. FINANCES Since this project was fully implemented in B&H, all project finances are in accordance with the provided procedures and lows for under the BiH. Indira Špiljak and Haris Haverić were in charge for all finances, allowances and their checking, cooperation with banks and financial reports and other as well. Financial report in attachment is in accordance with other financial reports that are as well as other important data availably to NGO ‘Doctors for disabled’ in Vienna and Union Sumero in Sarajevo.
