A HEALING GARDEN FOR OUR COMMUNITYThe mission of our Healing Garden is to provide people in the community a place to reflect on the loss of a loved one and restore a sense of well-being. Our garden engages all the senses in a soothing yet rejuvenating natural sanctuary – with flowers and Zen-garden patterns for the eye, aromatic herbs and traditionally therapeutic plants for the nose, and a centerpiece waterfall sculpture for the ear.

Hello Friends,29 years of serving Southeastern Connecticut… 11,000 patients cared for… thousands more families given bereavement care… these are measures of the kind of impact we make in our community – and we are proud of them. But they are not what we mean when we say we are your community hospice.We mean that we provide care to anyone who needs it and meets the qualifications for care, even if they are un or underinsured and unable to pay. We mean that we guarantee 24/7 call response, that we listen carefully and help patients and families make critical decisions, and that we provide deep care which helps patients sustain their quality of life and die in comfort, with dignity. We mean that we consider the entire family to be our unit of care, and that we support family members through their experience before and after the death of their loved one - including bereavement counseling, free of charge. Our commitment to exceed our patients’ and their families’ expectations of the hospice experience is what distinguishes us as your community hospice.But our community is growing and evolving, and we are committed to rising to the challenge of meeting its growing needs. This is why last year we developed our 2018 Vision, which is a 5-year plan to grow our core hospice program and create new services and facilities. We are very pleased to report to you about the substantial progress we made this year in accomplishing key parts of our plan.First, to help patients and their families before they need hospice care, we launched our Palliative Care Program at Backus Hospital. In partnership with Backus, our Palliative Care Team provides advanced pain and symptom management to hospital patients with life-limiting illnesses. We also help patients and their families to understand their options for care and to make decisions that help enhance quality of life. The goal of the program is to enable patients to go home with good care, and to stay out of the hospital.To enable cardiac patients to live wherever they call home and optimize their quality of life, we are piloting Heart 2 Heart: A Program for Cardiac Care. This unique initiative is the first of several disease-specific programs we are planning.To provide grieving people in the community a place to reflect on their loss and recover their sense of well-being, we created and opened our Community Healing Garden. In addition to being a community resource, our garden will play a key role in our Bereavement Counseling programs, including grieving children in our Expressive Arts program. We are especially grateful to the many generous people who donated more than $125,000 to build our garden.To serve families with grieving children and teens in the region’s shoreline towns, we partnered with Mystic Arts Center to launch our Expressive Arts program there. This means that many more children will benefit from a unique program that helps them in their mourning process by providing physical, emotional, and cognitive outlets to express themselves. As you will see in coming years, our 2018 Vision looks ahead to many more successes like these. Your support makes these initiatives possible, and we are deeply grateful to you.

EVOLVING TO MEET THE CHANGING NEEDS OF OUR COMMUNITY

Community Healing Garden: A place for people from our community to reflect on the loss of a loved one and restore a sense of well-being. We are the first and only hospice in Connecticut to create a Healing Garden.

Palliative Care Program at Backus Hospital: A consultative practice including a physician, APRN, nurse and a social worker in partnership with Backus Hospital that combines advanced management of pain and symptoms, with enhanced quality of life for hospitalized patients and their families. We help patients and families to understand their options and make informed choices of the care they decide to receive – including the option of returning home with our agency. This is the first and only program of its kind in Southeastern Connecticut.

Heart 2 Heart: A Program for Cardiac Care: A pilot program that is providing deep care at home to patients with end-stage heart disease, enabling them to stay out of the hospital while enhancing their quality of life at home. Heart 2 Heart is the only program of its kind in the region.

Expressive Arts @ Mystic Arts Center: A partnership with the Mystic Arts Center to provide our unique program for grieving children and teens in Mystic, Stonington, Pawcatuck, and Groton, enabling them to use creative outlets to express their grief and cope with the loss of a loved one. Expressive Arts also provides a safe setting in which children can bond with other grieving children.

Expressive Arts in the School: An adaptation of our Expressive Arts Program that provides grief workshops and programs in the region’s schools and other community settings.

THE ROSE A VOLUNTEER’S STORYCenter for Hospice Care delivers a rose to the primary caregiver after a patient has died. Volunteer Ann went to her usual florist to pick up the donated rose. Usually a red rose is given, but on this day there were none so Ann took a yellow rose.

When Ann knocked on the door, she was greeted by the wife of the patient who had been on our program. Ann offered the rose and said, “This is on behalf of Center for Hospice Care.” At this, the widow began to cry. Ann herself was moved to tears, and said, “I’m so sorry for your loss.” The widow responded, “No, it’s not that. You see, today is my birthday, and my husband always gave me yellow roses on my birthday.”
