Annotations:

1. The dashboard items are pre-filtered patient lists (screen PL1). The number of results from this filter equals the number shown.

2. Eventually, users will be able to create their own filters and save them in the dashboard for easy access. An example would be to save all patients tagged with a phrase. This will not be available in MVP 1.0.

Once patients are entered into the system, their profiles become searchable by their name and diagnosis.

Results autofill with suggestions as letters are typed. These results are segmented by name or cancer type.

From the results page, the user can filter using the sidebar to narrow down the results further.

1. The number of results searching / filtering has found. This number changes, as well as the patient list below it, as filters are adjusted.

The four most popular diagnoses appear, followed by “see more” which opens a full list of diagnoses. The number in parentheses always refers to the number of patients with this tag.

When a cancer type is picked, there is the option to select by “Stage” tag as well. When tags have zero results, they can not be checked.

Annotations:

1. When this icon is clicked from any page in the app, the Notifications page opens. The red circle on the icon disappears when the icon is clicked. It reappears when the user is on a different page and a new notification arrives. The number aligns with the number of new notifications since last visiting this page.

2. Notifications are arranged in chronological order with most recent actions appearing at the top.

3. Users and actions are linkable to their respective pages. For the MVP, staff links will not be linkable but patients and items such as commenting and tasks should.

4. The 15 most recent notifications are shown when landing on the page. More notifications lazy load when continuing to scroll down.

This section is where staff adds new patients into the system. There are two types of patients. “Brand New Patients” have no previous history at the office / just been referred. “Existing Patients” are in the middle of treatment at the office but just have not been added to the CareStatus database.

New patients, when a “reason for referral” is selected, will automatically receive their first checklist (the pre-consultation checklist). For example, when new patient Mary Smith is created here and her reason for referral / diagnosis is Breast Cancer, her treatment plan will have generated a list of tasks that need to be completed before her first consultation appointment. When a doctor authors a pre-consultation checklist once for a specific type of disease, all future patients with that disease will get this same checklist. Existing patients do not have this option because they are past the pre-consultation interval in their treatment.

Annotations:

1. Searching for a specific patient name shows only that patient in the results with the top-level task.

2, Filter by the current treatment interval the patient is in. This does not include previous or upcoming intervals such as if a patient has already finished chemo or will start chemo in the next week.

3. The status update of the task. There is a set number of status updates that tasks can be designated. These statuses are assigned by team members in one of two places, the patient profile / treatment plan (screen PT3) or the patient profile / task detail page (screen PT4)

4. Tasks can be checked off, rescheduled and updated from this section exactly like they can on a patient’s profile page.

Annotations:

1. The tab “Treatment Plan” is open in a patient profile.

2. The timeline is a work in progress. It will not be incorporated into MVP 1.0

3. A list of the treatment plans that have been authored by the doctor for the patient. Each treatment plan type (Chemo, Radiation, Oral Therapy, Watchful Waiting, Long Term Task, Additional Workup) has a color associated with it. The task list below this section is a combination of all tasks that were created for these treatment plans. The task list is not organized by treatment type but rather in chronological order over the next 30 days.

4. When tasks are completed, staff checks off this box.

5. This is the rule for when this task must occur in relation to the main event. These rules are determined in the authoring process.

6. When tasks with appointment dates associated with them have been scheduled, they appear in this state. Choosing whether a task does or does not have an appointment associated with it occurs during the authoring process.

7. When tasks with appointments have not been scheduled, they appear in this state with a target date indicated in place of an actual date as specified in the authoring process.

8. When tasks are created, they are assigned buckets or larger categories. Buckets allow for tasks to be easily organized in the Task Management tool (screen TM1). There are a set number of buckets to choose from. The bucket a task is assigned is noted here.

9. The current status of a task (“Pending Authorization”) with freehand comment “Called insurance on 2/21” also added.

10. Indicates that their are comments about this task. The number is the total number of comments including replies.

11. Opens treatment plan editing (screen A1)

12. Tapping on a treatment plan turns it “on” or “off” from the checklist below. When a treatment plan is “off” it does not show up in the task list. In the first screen, Radiation (green line) is “on” and in the second screen it is “off.”

PATIENT PROFILE - TREATMENT PLAN IN ACTION

Task editing can be performed directly on this page. Tasks can be completed, status updated and scheduled.

Annotations:

1. When the completed box is checked off (to signify that a task is complete), a confirmation window opens. Unchecking the box also opens a confirmation window asking “Is this task incomplete?”

2. Tasks can have status updates attached to them. This is not necessary but is an option. Tasks can be given multiple status updates over time but only the most recent shows. When the status area is clicked, a pop up window opens with a dropdown of status updates. These updates are from a fixed list of options. In addition to the dropdown, an option to leave a comment also is given. The comment can be a maximum of 140 characters. The number on the bottom left of the comment box decreases in real time as words are typed to represent the number of characters currently used.

3. When appointments are added, a pop up window with a calendar and time appears. The options of dates to choose on the calendar match the restrictions as indicated in the text (the user can only select days between April 15 and April 19 in this case). Selecting a time is optional. The time drop down shows every 30 minutes between 9 AM & 6 PM. As a date and time is selected, the results show in the lower left of the pop up window.

4. This is what blank status updates look like.

5. Once dates have been confirmed, they can always be rescheduled. Clicking on the date / time, opens a pop up window just like the one seen in #3.

6. Clicking the title of the task opens a new page with the task detail (screen PT4).

7. When tasks are completed, they appear in this state. Completed tasks can not be edited unless they are unchecked.

This is the page that shows when tasks are clicked from the task list. Each task has its own Task Detail page.

2. The current status with comment attached. Clicking on the gray box (“Awaiting Authorization” allows for a new status to be added in its place.

3. The date of the task. Clicking “Reschedule” opens a calendar and time drop down. This section only shows if the task has an appointment associated with iht.

4. Unlike general discussions about a patient which have multiple threads, there is only one thread for task discussion.

5. A history of all previous status updates. This list is ordered from most recent to least recent.

Treatment Plans can either be created from scratch or they can be selected from a stored list of previously created treatments. When a stored treatment is selected, the tasks that are designated can be edited or removed. However, these tasks revert back to their original form (when they were first created) when selected again for another patient.

4. A main event is indicated and can not be a changed. A calendar date must be added (via calendar popup) in order for the “View Task List” button to be enabled.

5. Once a regimen is clicked and a date has been added, clicking this button takes you to a page much like screen AC7 with all of the tasks and cycles pre-defined from when they were originally authored.

For Chemotherapy, treatments are performed in scheduled cycles. Different chemo drugs have different cycle lengths. Once this general area is created, it will be stored in the future as seen in the previous screen. If the name / cycles already have been authored, an error message will show with a link to this previously authored task list. (WORK IN PROGRESS). All fields in this modal window are required in order to continue building the task task list.

New Chemotherapy Regimen

Annotations:

1. The doctor must name the regimen. In this example the name is “TC” which stands for Taxotere / Cyotaxan - the two drugs which are used. There can be other treatments also named TC as long as the cycles are of a different length and duration.

2. The doctor can either click the plus / minus buttons to increase the number by one or can type a number value directly into the center box

3. Some cycles last indefinitely until there is signs of improvement. Clicking this button deactivates the number section.

4. Dropdown includes days, weeks, months, years

5. The first day of chemotherapy is the event which all of the tasks in the next page revolves around. By indicating a date, all tasks must now be completed before, during or after this date.

On this page, the doctor adds tasks that need to be completed during the chemotherapy treatment. Based on the previous screen, the number of cycles and projected dates have been created. The doctor indicated that there were 3 cycles that repeated every 3 weeks starting on March 23rd. So on this page, TC 1, 2 and 3 have been created. and the dates say March 25, April 15 & May 6.

Tasks can be added either before, the day or, or after the first day of chemotherapy. These are segmented.

When adding a task, there are a set of questions that need to be answered before the task is created. OThese expand in an accordion style which pushes all content below if down and pulls the content back up when the task is added.

Annotations:

1. The task name is a free text that is remembered the next time it is entered. This means previously typed tasks can are autofilled when typing begins.

2. The bucket the task belongs to is signified here. Previously created tasks autofill this area.

3, When adding a task before or after the first day of chemo, the doctor must indicate when specifically.. When “exactly” or “within” are selected, a day must be selected. The drop down for this section is days and weeks.

4. Clicking “all cycles” means the task will be slotted into the successive cycles in addition to the cycle the doctor is currently editing. Alternatively, the doctor can click one or more of the single cycle buttons.

5. Some tasks are just recommended as opposed to mandatory. This has no effect on the functionality of the task but just adds an extra tag indicating optional or required.

6. Appointment related tasks have schedules attached to them that are edited after the task list has been created. In-office appointments mean the staff can add a date and time. Out of office appointments means the staff can add a date but no time.

7. Completes the task creation and closes the accordion. Only becomes activated when all previous fields are filled.

8. Once tasks have been added, icons and text appear to quickly illustrate if they are repeating, optional, in or out of house appointments. Required tasks and those without appointments do not have respective icons - they are left blank.

When a plan is put on hold, it essentially restarts the cycle. So if some tasks in this cycle have been completed prior to pausing, they will become incomplete again once the cycle is resumed. When resuming a plan, treatment reverts back to the beginning of the current main event. By adding a date to resume treatment, the doctor is selecting a date for when the main event begins again.

When the doctor sets a reminder after holding a plan, it creates a reminder notification in the header. This is in addition to the “notifications” functionality also in the header.

Reminder Flow

Annotations:

1. Clicking the icon opens and closes the reminder window

2. This is a link directly to the patient’s discontinued plan page - screen HP5.

3. Clicking the icon or text creates a new reminder that is the exact same length as the previous reminder. If previous reminder was for 1 week, this will create another reminder for 1 week later and clear the current reminder.

4. The undo link reverts back to the Screen HP7 where the reminders still needs to be addressed.

5. A link to screen HP4 where the delayed treatment remains in the “On Hold” state.

5. Reminders are ordered in chronological order starting with the most recent reminder.

Reminders turn on once the time specified has elapsed. Like notifications, a number reflecting the number of new reminders is highlighted in a red circle on top of the reminder logo. Unlike notifications though, this number does not go away once the icon is clicked. The number of reminders signaled only decreases when a reminder is directly addressed.

TOOLS - TASK MANAGEMENT (wireframe only)

Task Management

Each task has a corresponding bucket it belongs to. This tool allows the staff to create and view tasks per bucket.

Annotations:

1. There is a set list of buckets that we will provide. Each bucket is presented in tabs. When a bucket is activated (such as “Imaging”), the tasks that have belong to this bucket show in the main window.

2. Tasks can be viewed by bucket or as a complete list.

3. Once inside of a specific bucket, tasks can be created. These can named at the user’s discretion.

4. Newly created tasks can be edited / deleted but once tasks have been assigned to task lists, they can not be deleted. This is a view only section.
