SESSION ABSTRACT: The patient experience has become critically important to health care organizations impacting value-based purchasing payments. Patient expectations are evolving and patients are becoming more engaged and educated health care consumers. Case management leaders play an important role in operationalizing their organization’s patient experience strategy. SESSION ABSTRACT: In this session, a crisis social worker will share his experiences from years of work conducting crisis assessments in an emergency room setting and illuminate the events that led to him to seek out more in-depth training in suicide assessment. Learn about a research-based clinical approach the speaker adopted in assessing suicide in a hospital setting.

LEARNING OBJECTIVES:1. Identify Thomas Joiner’s three commonalities among people who died of suicide.2. Define thwarted belongingness, burdensomeness and acquired ability to inflict lethal self-harm.3. Discuss the importance of including the role of social media in suicide assessment.

SESSION ABSTRACT: In 2015, 47 million people were reported with dementia and the number continues to rise. The extended timeframe spanning the advancing stages of dementia poses challenges for family and caregiver decision making and for health care professionals within the advanced care planning process. During this interactive session, we will explore: Are there limits to what an Advance Healthcare Directive can specify for the future care of a patient with dementia? Can food and water given by hand be declined in advance even though the future demented self may want it?

LEARNING OBJECTIVES:1. Discuss the role of Advance Healthcare Directives in the context of dementia.2. Identify the ethical issues related to precedent autonomy and its clinical and social roles.3. Explore common challenging situations and perspectives related to providing or withholding care, food and/or water

for a demented patient.

SESSION ABSTRACT: Discuss the purpose of the Child Maltreatment Screening Guidelines in Minnesota. Ramsey County Child Protection provides direction to local child welfare agencies to promote statewide consistency in definition and practice. In addition, they educate the public about types of child safety concerns that should be reported. Families and communities are best served when child maltreatment screening guidelines are clearly understood and readily available.

LEARNING OBJECTIVES:1. Identify the role and expectations and role of mandated reporters.2. Review role of the Child Protection Services.3. Recognize signs of child abuse.
