How the service came into existence?

enable people, who are medically fit,to be discharged home safely

support people, post discharge, while they regain their confidence

support them to regain skills for activities of daily living that may have been lost during the hospital stay

establish a true picture of any on-going support needs

• Service commenced in May 2014• Some changes made along the way• Now working with the Integrated

Discharge Team• Assessing patients in A&E, CDU and on

wards• Over the past 10 months have supported

over 70 people through preventing admissions and reducing lengths of stay.

People need night sitting services (safety/confidence)

More effective assessment in the Clients own home

Providing a viable alternative to permanent care

Bridging Service - another point of call if things get challenging
