Alzheimer’s Community Care was founded in 1996, when a group of local donors, caregivers and community

leaders came together in response to the growing number of local residents diagnosed with Alzheimer’s

disease. Many of these individuals had family members who were being affected by Alzheimer’s. They

established a local dementia-specific service organization exclusively dedicated to the needs of patients

and caregivers living with the daily challenges of Alzheimer’s disease and related neurocognitive disorders.

The organization’s mission has always included a dual focus on helping both the patient and the caregiver.

All funds raised are committed to supporting its community-based programs and services.

Alzheimer’s Community Care’s first programs included two Family Nurse Consultants and three dementia-

specific Care Centers located in Boca Raton, Delray Beach and Pahokee. Through its 18 years of service the

organization has grown to include a comprehensive safety net of community-based services and resources

for Alzheimer’s patients, caregivers, and the Palm Beach, Martin and St. Lucie communities. Programs and

services now include nine (soon to be 11) Specialized Alzheimer’s Care and Service Centers, seven Family

Nurse Consultants, 24-hour Alzheimer’s Crisis Line, ten Caregiver Support Groups, Case Management,

Professional and Community Education and Specialized Disaster Preparedness.

To our Supporters and Friends, The 2013-14 Annual Report reflects our commitment to provide the highest quality of dementia-specific care within an efficient and cost effective model-of-care. We are so proud of our numerous accomplishments during this past fiscal year, as listed below:

The first achievement was opening the newly renovated Phyllis and Julius Siegel Alzheimer’s Care and Service Center in Boca Raton. For over 18 years, our Specialized Alzheimer Care and Service Center was housed within Advent Lutheran Church’s fellowship hall. Now, it is located in its own free-standing building thanks to the partnership with the church and the philanthropic generosity of Ron Siegel. We now operate nine community based centers and all are licensed Specialized Alzheimer’s Day Care and Service Centers.

The second achievement is becoming accredited by the Joint Commission under Behavioral Health. Being accredited is a major accomplishment, and one that was a complete organizational effort. Everyone at Alzheimer’s Community Care contributed to this process and we all deserve to take credit for this hallmark accomplishment.

The third accomplishment was the establishment of our Education and Quality Assurance Department, led by Vice President Karen Gilbert. Paula Menigoz, previously the Family Nurse Consultant for Palm Beach County’s southern area, has become the new project manager for the ID Locator Bracelet Program.

There are other achievements that are too numerous to mention. However, we are indeed proud of our ability to maximize our resources, as evidenced by the fact that 90 cents of each donated dollar is used for direct services and resources. Family Nurse ConsultantsThe Family Nurse Consultant (FNC) Program serves as a life line for families in Palm Beach,

Martin and St. Lucie Counties who are living with Alzheimer’s disease and related disorders.

The program’s goal is to provide caregivers with the support, tools and resources needed

to continue providing home-based care for their loved ones.

The Family Nurse Consultant program incorporates

best practices for home-based dementia-specific care

as demonstrated through research by the Fisher Center

for Alzheimer’s disease. The best practices include

educating caregivers about the disease, counseling

and supporting the caregiver, and strengthening

social supports and family relationships. As a result,

caregivers are well informed, less isolated and better

equipped to provide care at home.

The results equate to improved quality of life and safety

for both the patient and the caregiver. In 2014, 92%

of patients served by our FNCs were able to continue

living at home with family members rather than be

admitted to nursing homes.

Seven Registered Nurses serve as our FNCs. They are

specially trained in treating dementia and provide

support 24/7, 365 days a year. Our FNCs provide inter-

ventions, home visits, needs assessments and care

plans designed to meet the needs of the patient and

family caregiver throughout all stages of the disease.

These interventions have proven to reduce caregiver

stress, and delay or eliminate the need for nursing

home placement. The FNC program helps reduce

physician office and emergency room visits. FNC

services are provided without charge to Alzheimer’s

patients and caregivers and are entirely supported

by donations and grants.

Specialized Alzheimer’s Care and Service CentersSpecialized Alzheimer’s Care and Service Centers are essential in intervening and providing

respite care for caregivers while simultaneously providing dementia-specific, community-

based services for patients.

Specialized Alzheimer’s Care and Service Centers are

designed to provide the tools for Alzheimer’s patients

and family caregivers to maintain their quality of life. In

order to accomplish this, patients receive comprehen-

sive dementia-specific care, which lessens the impact

of behaviors associated with Alzheimer’s disease.

Caregivers benefit with essential daily intervention

that results in improved personal health, stress

reduction and respite from the 24/7 burden of caring

for their loved one at home. Each Care and Service

Center provides a safe, supportive and individualized

environment for Alzheimer’s patients.

Physical exercise, socialization, cognitive stimulation

and medical monitoring are some of the benefits

patients receive. Alzheimer’s Community Care admin-

isters nine dementia-specific centers, seven in Palm

Beach County, one in Martin County and one in St.

Lucie County.

Assurance of quality control is provided by the Vice

President of Community Care Services, a registered

nurse with extensive knowledge and experience in

dementia-specific care. The Care and Service Centers

maintain a 5:1 patient-to-staff ratio and are licensed by

the Agency for Health Care Administration in accord-

ance with Chapter 400, Part V, Florida Statutes, and

Chapter 58 A-6, Florida Administrative Code.

Education & Quality Assurance

Alzheimer’s Community Care’s Education and Quality Assurance Department provides dementia-

specific education and training, training in regulatory compliance, staff orientation and training,

policy and procedure development and maintenance, as well as oversight of special projects.

The Department monitors and maintains ongoing compliance with the organization’s quality,

performance, and state and regulatory standards, and performs evaluation and analysis of

planned objectives and outcomes.

In this past year, Education and Quality Assurance has

standardized the orientation of Alzheimer’s Community

Care’s clinical staff. These staff include: Family Nurse

Consultants, Program Managers, Program Nurses,

Program Assistants and Activity Coordinators. New staff

members are also required to demonstrate education

and competency in their first days of employment,

and in compliance with the latest, high standards of

our accrediting body, The Joint Commission.

Alzheimer’s Community Care also developed a plan for

enhancing the education of local Registered Nursing

students by establishing procedures for providing

clinical experiences for these future nurses.

This plan included structured orientation and formal-

ized agendas for active student participation in the

activities of our Specialized Alzheimer’s Care and

Service Centers, with the goal of orienting future

nurses to our community-based model of care and

demonstrating effective interventions for patients

with Alzheimer’s disease and related neurocognitive

disorders.

The initial plan provided for clinical rotations for

students of the Palm Beach Atlantic University Bachelor

of Science in Nursing Program. Future plans include

extending the program to additional area universities.

The Education and Quality Assurance Department

also expanded education in the community, providing

numerous disease-specific educational programs to

community groups, law enforcement, emergency first

responders, and related health care providers, such

as skilled nursing facilities, assisted living facilities

and adult day centers.

Toward the end of the fiscal year, the Department

began to frame goals and objectives for achieving

Disease-Specific Care Certification from The Joint

Commission, a level of recognition that goes beyond

our Accreditation in measurably demonstrating the

effectiveness of Alzheimer’s Community Care’s various

programs and services, and validating the outcomes

that we help our patients and caregivers achieve.

24-Hour Alzheimer’s Crisis LineThe 24-Hour Crisis Line provides the caregiver with a lifeline of understanding and

guidance that helps them survive the many stages of Alzheimer’s disease and other

related neurocognitive disorders.

The 24-hour Alzheimer’s Crisis Line serves Palm Beach, Martin and St. Lucie counties. Family Nurse Consultants

(FNC), on-call 24/7, answer calls that include crisis calls, responding to questions and relieving caregiver anxiety.

A nurse will arrange for emergency respite care when needed. Since 60% of Alzheimer’s patients are at risk for

wandering, the Crisis Line connects our on-duty nurse to law enforcement for assistance during search and

rescues. The Crisis Line is also an excellent crisis management resource for caregivers.

In 2013-2014, our FNCs received 4,441 calls for assitance and referrals from family caregivers. In all cases, the

on-call FNC responded to the inquiry, followed-up as appropriate, and linked the family to essential services

and resources.

ID Locator Bracelet The Alzheimer’s Community Care ID Locator Bracelet program provides an additional safety net

tool for patients with dementia who may wander away from safe and supervised settings. This

program partners with SafetyNet by LoJack and local law enforcement to assist the wandering

patient and their family. Our pilot project is working with 75 patients. An additional 75 bracelets

will be added to this project in 2015 thanks to generous support from The Jim Moran Foundation.

This is a vital program, as 125,000 persons with Alzheimer’s disease or related neurocognitive disorders wander

away from safety each year. Prompt location of the patient is essential because 80 percent of those lost for more

than 72 hours will not survive the event.

Digital FootprintThere are over 3 billion internet users in the world according to Statista. Approximately

40% of the world population has an internet connection. As caregivers become younger it

is vital to provide a credible and easily accessible resource. We pride ourselves on always

providing the most up-to-date information, and our registered nurses are always vetting

new techniques that may be of help to caregivers. Alzheimer’s Community Care has also

created a mobile-specific website that is tailored to your mobile phone. As the digital age

continues to evolve we are very proud of the following 2013 – 2014 accomplishments:

• 74,650 unique page views. That’s over 6,220 page views per month!

• Average time spent on website: Three minutes and twenty seconds

Google AdWords

Google Ad Grants is the nonprofit edition of AdWords, Google’s online advertising tool. Google

Ad Grants empowers nonprofit organizations, through $10,000 per month in in-kind AdWords™

advertising, to promote their missions and initiatives on Google search result pages. This equals

$120,000 over a calendar year. Google AdWords allows us to create ads for Google’s search engine

and increase the visitors coming to our website.

• Alzheimer’s Community Care had 1,300 visits to our website via Google AdWords during

the 2014 Fiscal Year.
