HSE ALERT PPD DUBAI Description of incident – In one of our PPD site, cable cutting/removal work was assigned to a technician who cut two rums of cable extra during removal of intended four runs of LV cables using power operated portable cable cutting machine. All cables were isolated and no injury took place. Due to this incident, client revoked all work permit and complete stoppage of work enforced. What Went Wrong – 1. No identification (trace marking) of cable to remove. 2. No protection to nearby cables to protect from probable damage during cutting of other cables. 3. Lone working – no direct supervision. 4. Human Error – No Hold Point to reduce human error. 5. The method of cable cutting was not detailed in the Method statement. Instead it was noted as removal of incoming and outgoing power & control cables. Technician cut extra two cables
