AFFIDAVIT OF ACKNOWLEDGMENT AND CONSENT

I, YSMAEL MAALI, of legal age, Filipino, married and a resident of Tagum City, Davao del Norte, Philippines, after being sworn in accordance with law, hereby depose and state that:

1. I am the biological father of the minor child, KHAELLANE MAALI, who was born on November 29, 2006 at Tagum City, Davao del Norte, Philippines;

2. At the time of the birth of said minor child, I was not legally married to his biological mother ANELYN P. MALLAS, who now resides and works at Jordan;

3. I hereby acknowledge my paternity/ filiation of the minor child, KHAELLANE MAALI and that I am giving my consent for him to use my surname as the same is reflected in his birth certificate;

4. His mother, ANELYN P. MALLAS wishes to take our minor child to Jordan to live with her and I hereby give my consent to such;

5. I am however going to continue to give the support to our minor child, as it is my obligation as a parent;

6. I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal purpose it may serve.
