At what point is something important enough to

warrant an investigation?

Sometimes people do investigations on

everything and end up doing poor investigations...

If you have a large number of incidents but don't have the

resources to investigate all of them, you have to draw a line.

But it goes beyond incidents. You should investigate things

that trend, have a root cause, can be corrected...

Look for things like minor incidents, near misses, audit findings or BBS

observations.

Collect the data, find the trend, then investigate the PROCESS not the incident.

You may not be able to investigate everything,

but remember...
