Facilities and training sitesCleveland Clinic Main Campus, a 1,300-bed, internationally renowned, tertiary care hospital, serves as the primary training site of the Psychiatry Residency Program. The campus is located on historical Euclid Avenue, in close proximity to the Cleveland Museum of Art, Severance Hall and the Cleveland Botanical Gardens. Residents in our Adult and Pediatric Psychiatric Consultation Services learn to manage psychosomatic issues in a wide array of medical and surgical conditions.

Lutheran Hospital, a 204-bed acute-care facility, houses the primary adult inpatient psychiatric unit of the Cleveland Clinic Department of Psychiatry. Lutheran Hospital has 84 behavioral health beds distributed among general psychiatry, geriatric psychiatry and mood disorders inpatient subspecialty units.

Fairview Hospital, a 500-bed facility which specializes in pediatric emergency and subspecialty care, houses the new 13-bed child and adolescent psychiatry unit.

Other training sites include the Louis Stokes Cleveland VA Medical Center, North Coast Behavioral Forensics Unit, Connections Community Mental Health Center and Stephanie Tubbs Jones Health Center as well as the hospitals and family health centers that make up the Cleveland Clinic Health System.

Unique training opportunities and affiliated centersAlcohol and Drug Recovery Center and Chronic Pain Rehabilitation Center The Alcohol and Drug Recovery Center and Chronic Pain Rehabilitation Center are comprehensive, multidisciplinary treatment programs designed to manage the most challenging patients, many of whom have undergone numerous failed treatments. The emphasis in these programs is on total health, including mental, physical, social and spiritual well-being. The spectrum of care includes inpatient, partial hospitalization and intensive outpatient programs. Residents spend one to two months in each of these centers.

Psychiatric Neuromodulation Service The Psychiatric Neuromodulation Service provides exposure to management of treatment-resistant mood and anxiety disorders. Treatment modalities available include electroconvulsive therapy (ECT) and vagal nerve stimulation (VNS). Ongoing research in the area of deep brain stimulation (DBS) for intractable psychiatric disorders highlights the novel opportunities for involvement.

Mood Disorders Research Unit The Mood Disorders Research Unit is an innovative program that incorporates research and clinical care through a multidisciplinary approach in an inpatient setting. It is among the first programs in northeast Ohio to offer both inpatient and outpatient programs dedicated to the treatment of mood disorders. The program has the dual goal of providing an optimal treatment experience for systematically investigating their problems of otherwise high-functioning patients with disabling mood disorders.

ElectivesWe welcome elective rotations from allopathic (MD) and osteopathic (DO) medical schools across the country. Elective rotations are scheduled through the elective program at CCLCM. Cleveland Clinic is affiliated with the Clevealnd Clinic Lerner College of Medicine (CCLCM).

MoonlightingResidents may moonlight after successful completion of PGY–2 and satisfactory performance in all areas of training. Final approval for moonlighting is at the discretion of the program director.

Post-Graduate InformationProgram graduates are well rounded and

prepared for the future. Past residents have chosen careers ranging from academic psychiatry, community psychiatry and private practice to administrative leadership positions, consultation psychiatry and clinical research, among others.

Board Pass Rate The average board pass-rate for Part I between 2000 and 2010 has been 91 percent, compared to the national average of 85 percent.

CurriculumFrom the beginning of their training, residents are encouraged to plan for the future beyond training. Each resident is expected to develop an individualized learning plan that creates a foundation for lifelong learning. While the first two years of residency focus on prescribed training experiences, the third and fourth years offer various elective opportunities. At all levels residents are encouraged to test their potential for scholarship and research. Each resident

is expected to develop an in-depth study of a problem or issue relevant to his or her area of interest, such as private practice, epidemiology or the future directions of psychiatry. Residents then present the results of their scholarly and research projects in venues such as a noon conference, grand rounds or local and national professional meetings.

PGY-1An orientation course held during the summer facilitates a successful transition to residency. The PGY-1 resident spends four modules in primary care (combination of inpatient, outpatient and emergency medicine) and two modules in neurology. Neurology rotations involve exposure to and management of neurological disorders on inpatient and consultative services. Residents participate in neurology conferences, grand rounds and other educational activities to gain additional exposure in preparation for the PRITE and board certification.

For the additional seven modules of the first year, the resident rotates through the adult psychiatric inpatient units. During the inpatient experience, residents learn diagnosis and management of severe mental illness under the supervision of highly qualified faculty. The residents will continue to hone their interviewing and diagnostic skills on the adult inpatient units via direct supervision from the senior resident.

PGY-2The PGY-2 resident trains on more specialized inpatient services while beginning longitudinal care for outpatients. Residents rotate through the Alcohol and Drug Recovery Center Unit, the Chronic Pain Rehabilitation Program and the Child and Adolescent Unit, gaining both inpatient and outpatient experience. The resident is an integral part of the treatment team and receives thorough exposure to multidisciplinary treatment of adult, child and adolescent patients.

Four modules in PGY-2 are spent on our flagship service, Consultation-Liaison Psychiatry, which plays a key role in the multidisciplinary treatment of the Cleveland Clinic’s nearly 80,000 annual hospital admissions. Residents will round off their experience with rotations in child psychiatry, geriatric psychiatry and emergency psychiatry.

PGY-3The third year of residency focuses primarily on outpatient care with emphasis on integration of psychopharmacology and psychotherapy, including cognitive behavioral, family therapy, group therapy and couples counseling. Weekly longitudinal outpatient clinics started in PGY-2 are continued as well. Residents meet with their medication management and psychotherapy supervisors weekly.

PGY-4During the fourth year, the resident extends training in outpatient care in addition to serving as a senior supervisor on the inpatient adult unit. Residents and medical students also assume teaching responsibilities.

PGY-4 residents are able to participate in a variety of electives to help with career planning and continuing research. Residents are encouraged to seek out weekly specialized clinics such as psycho-oncology, psycho-cardiology, epilepsy, chronic pain, movement disorders and psychiatric neuromodulation, to solidify areas of interest.

DidacticsThe didactic curriculum is presented within the

framework of the Academic Half-Day (modeled

after the Duke Psychiatry Residency Training

Program). Each Thursday morning all residents

(PGY-1 to PGY-4) are excused from clinical

duties to focus on academic objectives. From

8:30 to 11:30 a.m. residents attend two one-

hour-and thirty minute lectures, seminars, or

workshops designed and presented according

to PGY training level. After a half-hour break,

all residents and faculty attend Grand Rounds

from 12 noon to 1 p.m. Residents are then

expected to return to their service obligations.

A general outline of the 2012-2013 curriculum

is presented below. For the PGY-1s and -2s the

Summer is devoted to teaching and learning

the essentials, that is, “what you need to know

to get through they day . . . and the night”.

The PGY-3s and -4s receive focused instruction

in Learning to Teach, and play an active role

in lecturing to their new and returning junior

colleagues. General Psychiatry (PGY-1 and

PGY-2) and Psychiatric Neurosciences (PGY-3

and PGY-4) begin in October and are presented

in respective one-hour-and-thirty-minute blocks

each Thursday for the rest of the academic.

During the second one-hour-and-thirty-minute

block the PGY-1s have a weekly workshop in

learning to interview (Clinical Skills Training

(CST)), and the PGY-2s continue their weekly

psychotherapy (PT) course learning the

principles and practice of cognitive-behavioral

therapy (CBT), psychodynamic psychotherapy,

dialectical behavior therapy (DBT) as well as

group and family therapies. A four-week course

on Community Mental Health is offered to PGY-

2s and -3s). PGY-3s and -4s supplement PSN

with courses in advanced topics (e.g., Forensic

psychiatry), career development, and a monthly

journal club focused on learning to read and

interpret scientific literature.

ResearchResearch opportunities exist across the department and the institution. Each resident is required to have an academic project, consisting of a poster presentation and/or journal submission. A number of trainees choose a research project that they design; others elect to become part of a larger, ongoing study at Cleveland Clinic. Current and recent trainees have participated in projects on chronic pain, suicide, delirium, pseudo-seizures, ovarian cancer, women’s health, sleep disorders, post-myocardial infarction depression and transplant psychiatry. In addition, residents are expected to present at Case Conference annually, beginning in the second year. Every year, several residents go on to present at local and national meetings.

Neurological Institute Research Day (NIRD) is held annually at the Intercontinental Hotel and Conference Center, adjacent to Cleveland Clinic Main Campus. Abstracts may be submitted by all Neurological Institute trainees, research fellows, post-doctoral fellows and graduate students in the Lerner Department of Neuroscience, and by Cleveland Clinic Lerner College of Medicine medical students who complete Neurological Institute-mentored research projects. RetreatsThroughout their training, residents participate in various resident-led activities such as journal club, movie club, book club, picnics and other social events. A residency retreat is held twice each year, offering opportunities for residents to come together as a class. Retreats also help residents form lasting relationships and improve communication skills that foster positive work-life balance, teamwork and stress relief.
