Call 255-8400 or 1-800-252-1246. • Lifeline® - A personal emergency response service that offers 24 hours a day, seven days a week reassurance to remain at home. • Transitional Care Unit (TCU) - Bridges the gap between your hospital stay and return home. TCU is less intense care than provided in a traditional hospital setting and more intense care than offered in a skilled nursing facility. • Geriatric Assessment Program and Clinic - Please call 238-5437 to make an appointment for a comprehensive evaluation of seniors and their individual situation to assist with the correct diagnosis and treatment plan for your loved one.
